Anterior submuscular transposition of the ulnar nerve. For post-operative focal neuropathy at the elbow.

Outcome studies of revision surgical treatment for recurrent or persistent neuropathy of the ulnar nerve at the elbow are relatively rare and none involves patient self-assessment. In this study of 40 patients (41 elbows), a clear discrepancy is shown between clinical assessment and the patient's own view. From clinical assessment, 20% of patients had an excellent result, whereas only one (2.5%) patient self-reported a complete cure. More reports using patient self-assessment and validated scores are required.

Ultrasound of the elbow.

Ultrasound examination of the elbow should be performed in a comparative way, exploring joint spaces and fossas. Ultrasound is the method of choice for the detection of joint fluid and loose bodies. Tendinitis and microfissures can be demonstrated in case of tennis and golfer elbow. On the posterior aspect of the elbow, ultrasound shows the different types of bursitis, and allows evaluation of the ulnar nerve in cases of impingement.

[Intraoperative electrically evoked nerve action potentials in ulnar entrapment syndrome].

Many attempts have been made in the past to find predictive factors concerning patients operated on because of ulnar nerve entrapment at the elbow. The factors most frequently discussed in the literature are the patient's age, the importance of the preoperative neurological deficit, the duration of symptoms, accompanying diseases as diabetes mellitus or alcoholism and preoperative electrophysiological findings (EMG and conduction velocity measurements). With the exception of the electrophysiological findings, which uniformly are considered to be without predictive value, all other factors ment

Sports and peripheral nerve injury.

Peripheral nerve injury is one of the serious complications of athletic injuries; however, they have rarely been reported. According to the report by Takazawa et al., there were only 28 cases of peripheral nerve injury among 9,550 cases of sports injuries which had been treated in the previous 5 years at the clinic of the Japanese Athletic Association. The authors have encountered 1,167 cases of peripheral nerve injury during the past 18 years. Sixty-six of these cases were related to sports (5.7%). The nerves most frequently involved were: brachial plexus, radial nerve, ulnar, peroneal, and a

[Formation of a Subcutaneous Amputation Neuroma after Incomplete Endoscopic Carpal Tunnel Release Resulting in Complex Regional Pain Syndrome:A Case Report].

This 64-year-old woman had undergone endoscopic carpal tunnel release(ECTR)for right carpal tunnel syndrome 16 months earlier. Thereafter, she reported persistent dysesthesia in the thumb and index finger, developed burning pain in the middle and ring finger, paleness, coldness, and edema of the hand, a decreased range in hand motion, and a painful subcutaneous nodule just distal to the portal in the forearm. Based on physical, radiological, and electrophysiological studies, the diagnosis was incomplete carpal tunnel release associated with complex regional pain syndrome(CRPS). At open revisio

Medial epicondylectomy or ulnar-nerve transposition for ulnar neuropathy at the elbow?

We carried out a prospective randomised study comparing medial epicondylectomy with anterior transposition for the treatment of ulnar neuropathy at the elbow. The mean follow-up period was 4.5 years and we assessed the patients neurologically and orthopaedically. Neither procedure appeared to have a significant effect on elbow function. Our study showed better results after medial epicondylectomy; in particular patient satisfaction was higher than after ulnar nerve transposition. There were no significant differences in motor power or nerve-conduction rates and sensory fibres appeared to be mo

End-to-side anterior interosseous nerve transfer augmentation of ulnar nerve function in moderate and severe cubital tunnel syndrome; a randomised controlled trial protocol.

BACKGROUND: The anterior interosseous nerve (AIN) end-to-side (ETS) nerve transfer is a relatively recent treatment innovation in managing cubital tunnel syndrome (CuTS). Potential benefits must be balanced against the risks associated with the adoption of an innovative technique, evaluating procedure-specific risks, and considering additional costs in the safe delivery of care. The ETS technique has been proposed as a method of improving intrinsic muscle function in the hand in the setting of ulnar nerve compression at the elbow. The technique is controversial with emerging evidence from retr

Common sports-related nerve injuries seen by the electrodiagnostic medical consultant.

The high physiologic demands of sports create dynamic stress on joints, soft tissues, and nerves which may lead to injuries in the athlete. Electrodiagnostic (EDx) assessment is essential to identify the correct diagnosis, localization, and prognosis, to guide management of sports-related neuropathies. A comprehensive review was performed to provide the EDx medical consultant with a practical approach to the common peripheral nerve disorders seen in athletes. Sports-related neuropathies reviewed include transient traumatic irritation of the brachial plexus and/or cervical nerve roots ("Burners

Work-related risk factors for ulnar nerve entrapment in the Northern Finland Birth Cohort of 1966.

Ulnar nerve entrapment (UNE) is the second most common entrapment neuropathy in the upper extremity. The aetiology of UNE is multifactorial and is still not fully understood. The aim of the study was to identify occupational risk factors for UNE and to determine whether smoking modifies the effects of work-related factors on UNE. The study population consisted of the Northern Finland Birth Cohort of 1966 (NFBC1966). In total, 6325 individuals active in working life participated at baseline in 1997. Occupational risk factors were evaluated by a questionnaire at baseline. The data on hospitaliza

Compressive neuropathies related to ganglions of the wrist and hand.

Ganglions of the wrist and hand causing compressive neuropathies are rare clinical entities. Compression of the ulnar and median nerves in their respective fibro-osseous tunnels lead to characteristic patterns of motor and/or sensory deficits, which are directly related to the location of the lesion. We present a unique case of a "dumbbell" shaped ganglion invading both Guyon's canal and the carpal tunnel causing a dual compressive neuropathy of the ulnar and median nerve. We discuss the patho-anatomy, clinical assessment, investigation and surgical treatment of this condition.

Long term results using a modified King's method for cubital tunnel syndrome.

Forty-six elbows in 40 patients with cubital tunnel syndrome were evaluated after using a modified King's method. The average postoperative followup period was 79 months. The severity of cubital tunnel syndrome was graded before and after operation by the modified method of Stuffer et al which evaluated the amount of numbness, sensation, muscle strength, and muscle atrophy. The results were positively correlated with the preoperative severity of the disease and the ulnar nerve sulcus angle as measured in the axial view of the elbow. When the amount of the medial epicondylectomy was large, a go

Synovial Osteochondromatosis of the elbow presenting with ulnar nerve neuropathy.

Synovial chondromatosis is an uncommon disorder in which cartilaginous material is formed within synovial tissue. The cartilaginous nodules may undergo enchondral ossification, described as synovial osteochondromatosis. The nodules may be shed from the synovium and become intra-articular loose bodies. The presenting symptoms are usually diffuse discomfort in the affected joint and decreased range of motion with an accompanying gritty or locking sensation. The authors present the case of a young man with elbow synovial osteochondromatosis associated with ulnar nerve neuropathy.

[The medial epicondyle of the humerus, an anatomical study].

A large variation in the shape of the medial epicondyle is encountered. This study was performed to investigate any role of the shape of medial humeral epicondyle in the etiology of osteoarthritis in the elbow, which can lead to cubital tunnel syndrome. A total of 71 humerus from 36 cadaveric specimens (mean age 75, ranging from 52 to 93) were used to study the morphological characterization of the medial humeral epicondyle. The specimens were divided into four types according to the index of medial epicondylar breadth and medial epicondylar length. A correlation was found between the angle of

Regional differences in ulnar nerve excitability may predispose to the development of entrapment neuropathy.

OBJECTIVE: To assess whether there are differences in nerve excitability properties between proximal and distal stimulation sites in the ulnar nerve in healthy controls, which may provide information on whether alteration in ion channel function predisposes to the development of ulnar neuropathy at the elbow. METHODS: Nerve excitability studies were undertaken in 11 healthy controls. Studies were undertaken with stimulation of the ulnar nerve at the elbow and wrist. Recordings were obtained from abductor digiti minimi in both sets of studies. RESULTS: Recordings obtained following stimulatio

Dissecting neural anomalies and clinical impact of aberrant median and ulnar nerve pathways in the forearm: A systematic review and meta analysis.

Aberrant communications between the median and ulnar nerves in the forearm and hand are clinically important anatomical variants that can significantly influence electrophysiological interpretation and surgical decision-making. This systematic review and meta-analysis aimed to synthesize available anatomical and electrophysiological evidence on the prevalence, subtypes, and clinical implications of median-ulnar nerve anastomoses, including Martin-Gruber, Marinacci, Riche-Cannieu, and Berrettini connections. A comprehensive search of major databases identified 20 eligible studies encompassing e

Large patent median arteries and their relation to the superficial palmar arch with respect to history, size consideration and clinic consequences.

BACKGROUND: A large patent median artery can be involved in several clinical disorders like carpal tunnel syndrome, anterior interosseous nerve syndrome and pronator syndrome. METHODS: The frequency and variability in the expression of the median artery and the expression of the other forearm arteries were recorded during two dissection courses. The topography of the arteries with their ramifications was documented on diagrams and photographs. The outer diameters of forearm arteries were measured. RESULTS: A large median artery was found in 4 of 54 arms (7.4%). The median arteries took their

Carpal tunnel syndrome in pregnancy.

One thousand consecutive postpartum patients were interviewed by questionnaire to establish the incidence of carpal tunnel syndrome in pregnancy. Hand symptoms had been noted by 34%: 25% had had symptoms of carpal tunnel syndrome, 2% symptoms of ulnar nerve compression and 7% ill defined hand symptoms. Maternal and fetal age, parity and weight change did not correlate with the presence of symptoms. The rate of ring removal because of swelling was twice as great for the symptomatic women (73%) as for the asymptomatic women (36%), and the rates of pre-eclampsia, hypertension and edema were highe
