A retrospective analysis was conducted of 778 patients who underwent highly selective vagotomy between 1980 and 1990. Personal ten-year-long surgical and clinical experience with the application of truncal vagotomy in 51 patients with complicated duodenal ulcers-23 with hemorrhage, 19 with perforation and nine with stenosis, is shared. Emphasis is laid on the superiorities and comparatively low lethality (5.8 per cent) of the application of this type of vagotomy during the surgical management of complicated duodenal ulcers, as well as on the great diagnostic relevance of fiber gastroscopy applied on the apex of hemorrhage in case of bleeding duodenal ulcers.

Patients (1,643) undergoing vagotomy for peptic ulcer at York District Hospital were traced through the Office of Populations Censuses and Surveys (OPCS) and cause of mortality obtained for the 577 who had died. Post-prandial serum levels of gastrin, the main hormonal stimulator of acid secretion, have been shown to be significantly elevated after HSV compared with controls. The mechanism for this elevation is not known but could be secondary to an increased antral gastrin concentration (AGC). Highly selective vagotomy (HSV) was performed in 509 patients over 12 years for the surgical management of duodenal ulceration; 103 HSVs were carried out during the treatment of complications. The overall rate of ulcer recurrence was 7%, ranging from 10% in the first 4 years to 4% in the 6 years between 1975 and 1980. An analysis is presented of the ulcer recurrence rate following parietal cell vagotomy in 333 patients with duodenal ulcer. The median period of observation for patients without ulcer recurrence was 54 months (range 1 to 128 months). Forty-two patients developed proven recurrent ulcer. Our aim was to develop proximal gastric vagotomy with mucosal antrectomy as an operative approach to duodenal ulcer. We performed mucosal antrectomy in four dogs by excising the antral mucosa via a circular corporal myotomy, removing a circumferential band of corporal seromuscularis, anastomosing the corporal and pyloric mucosas endoantrally, and reapproximating the corporal and antral seromuscularis.
