5 to 25% of male adolescents present with a varicocele; only those cases inducing serious discomfort or affecting the development of the testis require surgical treatment. Nevertheless, the results of surgery are disappointing, with a high recurrence rate. Based on a better knowledge of testicular venous drainage, it appears that conventional high ligation of the spermatic vein (s) might not be sufficient to cure varicocele. The exact role of varicocele in human male infertility remains controversial. Fifty-five male partners of infertile couples randomly selected and 17 fertile semen donors were evaluated for semen quality, scrotal temperature, and presence of varicocele using clinical palpation and Doppler ultrasound. A new technique for surgical treatment of idiopathic varicocele is described. This technique consists of internal spermatic vein ligature and its resection and microvascular anastomosis between proximal tract of the internal spermatic and saphenous vein. In this way a communication is established between a high regimen pressure with a lower one. Fourteen idiopathic varicoceles are successfully operated with this new technique.

A retrospective analysis of 324 infertile patients who underwent the surgical treatment of varicocele was conducted. Patients were divided into groups according to the presence or not of other pathological conditions, associated with varicocele: genital inflammation, left testicular hypotrophy, elevated FSH, history of cryptorchidism and azoospermia. This study reports on the stagnation of blood within the microcirculatory vessels of the testes of patients with varicocele. Both fine structural and quantitative studies were carried out on testicular biopsies from 14 men with varicocele and a control group of three men. We describe a noninvasive modified radionuclide technique, the "comprehensive scrotal flow and scan," for the detection of varicoceles. The procedure utilizes in-vivo-labeled pyrophosphate red blood cells and 99mTc-pertechnetate as a blood pool scanning agent. Between May 1981 and October 1982, 259 varicoceles were identified by phlebography. It was possible to occlude 217 with Varicocid using a percutaneous Seldinger catheterisation technique under local anaesthesia. This method is an alternative to standard surgical techniques and has several advantages. It is cheap, it can be carried out on an out-patient basis, it lacks the risks associated with general anaesthesia and there are few complications.

Varicocele is one of the most common causes of male infertility. We have developed a technique for the treatment of this disorder in which occlusion of the internal spermatic vein is accomplished percutaneously using a detachable balloon. Twenty-one varicocceles in 20 patients have been treated successfully without complication. This procedure can be performed easily, without general anesthesia or surgical incision, on an outpatient basis.
