Manifestations of left ventricular dysfunction and arrhythmia in patients with chronic hypoparathyroidism and pseudohypoparathyroidism: a preliminary study.

BACKGROUND: Cardiac damage triggered by severe hypocalcemia is well known. However, the role of chronic hypoparathyroidism (HP) and pseudohypoparathyroidism (PHP) in cardiac health is still unclear. We investigated the effect of chronic HP and PHP on cardiac structure and conductive function in patients compiling with treatment. METHODS: The study included 18 patients with HP and eight with PHP aged 45.4 ± 15.4 and 22.1 ± 6.4 years, respectively with a previously regular follow-up. In addition, 26 age- and sex-matched healthy controls were included. General characteristics and biochemical ind

Low-Grade Albuminuria Is Associated with Left Ventricular Hypertrophy and Diastolic Dysfunction in Patients with Hypertension.

INTRODUCTION: Microalbuminuria is a risk factor for cardiovascular morbidity and mortality in hypertensive patients. However, the relationship between low-grade albuminuria, a higher level of albuminuria below microalbuminuria threshold, and hypertension-related organ damage is unclear. Left ventricular (LV) hypertrophy (LVH) is well recognized to be a subclinical organ damage of hypertension, and LV diastolic dysfunction is also reported to be an early functional cardiac change of hypertension that predicts heart failure. The present study aimed to investigate the association of low-grade alb

Effect of sacubitril/valsartan on cardiac filling pressures in patients with left ventricular systolic dysfunction.

BACKGROUND: Sacubitril/valsartan is the newest neurohormonal agent approved for therapy in patients with heart failure with reduced ejection fraction (HFrEF). Little is known about its acute and incremental hemodynamic effects. We aimed to evaluate the change in hemodynamic profiles measured using an implanted monitoring device in HFrEF patients initiated on sacubitril/valsartan therapy. METHODS: We prospectively enrolled 13 subjects with HFrEF and pre-implanted CardioMEMS™ device on maximally tolerated guideline-directed medical therapy and no contraindications to sacubitril/valsartan therap

Left ventricular systolic dysfunction in two patients with ankylosing spondylitis: What is the role of corticosteroids?

Ankylosing spondylitis (AS) is a chronic inflammatory condition that most commonly affects the axial skeleton. The most common cardiac manifestation in patients with AS is the aortic root and valve disease, followed by conduction and rhythm abnormalities, decreased coronary flow reserve, myocardial infarction, and diastolic dysfunction. However, the presence of systolic dysfunction has been less described in patients with AS. Herein we present two cases of idiopathic dilated cardiomyopathy in patients with AS. These patients were noted to have an improvement of their ejection fraction followin

A case of right ventricular diastolic dysfunction due to a large hematoma posterior to the left ventricle.

We report a case of right ventricular (RV) diastolic dysfunction due to a large hematoma posterior to the left ventricle (LV) after cardiac surgery. An 80-year-old woman underwent cardiac surgery. After surgery, her physical findings revealed right heart failure. Localized hematoma posterior to the pericardial space and the RV compression to the sternum were shown by computed tomography. Transthoracic Doppler echocardiography demonstrated restrictive physiology of the RV although there was no evidence of constrictive pericarditis. These findings suggest that RV diastolic dysfunction could have

LEFT ventricular function assessed by echocardiography in cirrhosis: relationship to systemic hemodynamics and renal dysfunction.

BACKGROUND & AIMS: The current study aimed at assessing the potential role of cardiac abnormalities in the pathogenesis of circulatory and renal dysfunction in cirrhosis. METHODS: One hundred and fifty-two patients (34 without ascites, 95 with ascites without renal failure and 21 with hepatorenal syndrome) were evaluated using Doppler echocardiography. In 102 patients, diastolic function was assessed by measuring parameters related to ventricular filling velocity, mitral annulus velocity and left atrial dimensions. Cardiopulmonary pressures were also measured by cardiac catheterization in 54

Markers of preclinical vascular disease and left ventricular diastolic dysfunction in patients with HIV infection.

BACKGROUND: HIV infected patients have an increased cardiovascular risk that may be linked not only to the infection itself but also to the metabolic side effects of the antiretroviral therapy. AIM: The aim of our study was to determine markers of aortic arterial stiffness, carotid intima-media thickness (IMT) and parameters of left ventricular diastolic function and to establish the relationship between these vascular and cardiac parameters in HIV infected patients. MATERIAL AND METHOD: In this cross sectional case control study 43 patients with HIV infection and 25 healthy controls, matche

Left ventricular dysfunction in Klinefelter syndrome is associated to insulin resistance, abdominal adiposity and hypogonadism.

OBJECTIVE: Epidemiological data suggest there is an increased risk of dying from heart disease among patients with Klinefelter syndrome (KS). Due to high prevalence of hypogonadism and metabolic syndrome, we speculated that patients with KS may have subclinical changes in the left ventricular function. Therefore, the aim was to assess left ventricular long axis function by tissue Doppler echocardiography in patients with KS and relate these findings to the metabolic status and testosterone levels. DESIGN: Cross-sectional study. Out-patient clinic. PATIENTS: We investigated 25 unselected pati

Efficacy of internal cardioversion for chronic atrial fibrillation in patients with and without left ventricular dysfunction.

Internal cardioversion can restore sinus rhythm with energies below 6-10 J, often without anaesthesia/sedation. We investigated its safety and short-/medium-term efficacy in patients with persistent atrial fibrillation (AF) with left ventricular dysfunction (defined as ejection fraction < or = 40%). Among 34 patients with persistent AF who agreed to receive internal cardioversion, 16 had left ventricular dysfunction and 18 did not (the groups were similar as regards age, duration of AF and pretreatment with amiodarone). Internal CV was performed delivering 3.0/3.0-ms biphasic shocks between co

Acute aortic dissection associated with left ventricular dysfunction in a postpartum and normotensive young woman.

Aortic dissection is uncommon in young women and is associated with clinical conditions such as pregnancy and Marfan's syndrome. Owing to the low incidence, diagnosis of acute aortic dissection in young women might be missed or delayed in patients who have neither risk factors nor typical clinical manifestations. We report the case of a 28-year-old postpartum woman with aortic dissection. The patient complained of abdominal discomfort, transient back pain, and general malaise at our emergency department 1 week after delivery of a healthy baby. She had no history of hypertension, connective tis

Valve replacement for regurgitant lesions of the aortic or mitral valve in advanced left ventricular dysfunction.

Patients with aortic regurgitation and severe left ventricular dysfunction remain candidates for aortic valve replacement, as long as the risks of late left ventricular dysfunction and congestive heart failure have been fully discussed with the patient, the patient's family, and the referring physician. In contrast, patients with mitral regurgitation and severe systolic dysfunction are at considerable risk of more severe left ventricular dysfunction after operation, especially if mitral valve repair or chordal-sparing procedure cannot be performed. In patients who are candidates for such proce

Prognostic importance of the immediate hemodynamic response to nifedipine in patients with severe left ventricular dysfunction.

To determine the clinical significance of the occurrence of hemodynamic deterioration after the administration of calcium channel blocking drugs, nifedipine (20 mg orally) was administered to 29 patients with severe left ventricular dysfunction. Thirteen patients showed hemodynamic improvement with the drug (Group 1), as shown by a notable increase in cardiac index associated with a modest decrease in mean arterial pressure. The other 16 patients exhibited hemodynamic deterioration after nifedipine (Group 2), as reflected by a decline in right and left ventricular stroke work indexes accompani

Haemodynamic effects of salbutamol in patients with acute myocardial infarction and severe left ventricular dysfunction.

The haemodynamic effects of salbutamol infusions at rates of 10,20, and 40 micrograms/min were measured in 11 patients with acute myocardial infarction complicated by left ventricular failure. Four patients also had cardiogenic shock. Consistent increases were observed in cardiac outputs at all doses (up to 56% at 40 micrograms/min), while the mean systemic arterial pressure fell slightly (average 5 mm Hg), implying a reduction in peripheral vascular resistance. Changes in right atrial pressure and indirect left atrial pressure (measured as pulmonary artery end-diastolic pressure) were small a

Relation of left ventricular twist and global strain with right ventricular dysfunction in patients after operative "correction" of tetralogy of fallot.

In patients with corrected tetralogy of Fallot (cToF), left ventricular (LV) dysfunction is closely related to right ventricular (RV) dysfunction, indicating adverse ventricular-ventricular interactions. However, the mechanism that links RV dysfunction to LV dysfunction remains unclear. In this prospective study, 32 patients with cToF and 19 controls were enrolled. With cardiac magnetic resonance imaging, biventricular ejection fractions were assessed. Using 2-dimensional speckle tracking, global and regional RV and LV strains and LV twist were assessed. To detect and characterize ventricular-

Single lead electrocardiographic detection of left ventricular systolic dysfunction in pediatric and congenital heart disease.

Portable, scalable, and accessible artificial intelligence (AI)-enabled smartwatch technology shows promise as a cardiovascular risk stratification strategy in the general adult population. The challenges of lifelong care in congenital heart disease (CHD)-inclusive of regional and socioeconomic disparities worldwide-underscore the need for similar solutions tailored to this population. Herein, we present the first noise-adapted single-lead ECG model for predicting left ventricular systolic dysfunction (left ventricular ejection fraction [LVEF] ≤ 40%) in pediatric and CHD patients. The internal

Postpartum Left Ventricular Dysfunction in Preeclampsia With Incidental Patent Ductus Arteriosus: A Multimodality Hemodynamic Assessment.

Hypertensive disorders of pregnancy, particularly preeclampsia, are associated with increased risk of postpartum cardiovascular complications. We report a case of a 43-year-old female with known mild-to-moderate aortic regurgitation and thoracic aortic dilation who developed new left ventricular systolic dysfunction shortly after cesarean delivery for severe preeclampsia. On postoperative day three, she presented with chest pain and was found to have a reduced left ventricular ejection fraction (43%) on transthoracic echocardiography. Computed tomography angiography excluded aortic dissection

Artificial intelligence electrocardiography for left ventricular systolic dysfunction demonstrates preserved performance across demographic training imbalances.

AIMS: Artificial intelligence (AI)-enabled electrocardiograms (AI-ECG) can detect left ventricular systolic dysfunction (LVSD), but demographic imbalance in training datasets may introduce bias. Foundational models, pretrained on large and diverse datasets, may mitigate such concerns. We aimed to assess the impact of demographic composition in training datasets on the performance of an ECG Foundational Model (ECGFM) for diagnosing LVSD. METHODS AND RESULTS: We developed an ECG foundational model (ECGFM) using transformer architecture and self-supervised pretraining on 983 200 ECGs. Using 44 8

Bedside predictors of left ventricular systolic dysfunction in children with suspected infection undergoing cardiac ultrasound.

BACKGROUND: Left ventricular systolic dysfunction in children with suspected infection can worsen outcomes and complicate early management of sepsis. Cardiac ultrasound enables bedside assessment of ventricular function, but it remains unclear which bedside markers are associated with an increased risk of myocardial dysfunction. OBJECTIVE: To identify which bedside markers commonly incorporated into pediatric sepsis assessment are associated with reduced left ventricular systolic function on cardiac ultrasound in children with suspected infection. METHODS: In this retrospective cohort study,
