Aging-associated decline in peripheral vestibular function is linked to deficits in behaviors and cognitive abilities that are known to rely on the sensorimotor and frontal cortices, but the precise neural pathways are unknown. To fill this knowledge gap, this cross-sectional study investigates the relationship between age-related variation in vestibular function and surface shape alterations of the frontal and sensorimotor cortices, considering age, intracranial volume, and sex. Data from 117 older adults (aged 60+) from the Baltimore Longitudinal Study of Aging, who underwent end-organ-speci

OBJECTIVE: Previous research suggested that panic disorder with agoraphobia is associated with abnormalities on vestibular and balance function tests. The purpose of this study was to further examine psychiatric correlates of vestibular/balance dysfunction in patients with anxiety disorders and the specific nature of the correlated vestibular abnormalities. The psychiatric variables considered included anxiety disorder versus normal control status, panic disorder versus non-panic anxiety disorder diagnosis, presence or absence of comorbid fear of heights, and degree of space and motion discomf

OBJECTIVES: CANVAS is an acronym for cerebellar ataxia, neuropathy and vestibular areflexia syndrome. Limited autopsy data has suggested that CANVAS is caused by a focal dorsal root ganglionopathy that damages Scarpa's (vestibular) ganglion, but spares the Spiral (hearing) ganglion. If the vestibular areflexia of CANVAS is in fact due to ganglionopathy, then there should be global reduction of all vestibular responses.
MATERIALS AND METHODS: With this hypothesis in mind, a retrospective review of 5 subjects who met the clinical criteria for CANVAS was performed. Recent advances in vestibular

The vestibulo-ocular reflex (VOR) stabilizes gaze to permit clear vision during head movements. It has been supposed that VOR function might be inferred from dynamic visual acuity (DVA), the acuity during imposed head motion. We sought to determine effectiveness of DVA for detection and lateralization of unilateral vestibulopathy, using rigorous psychophysical methods. Seventeen normal and 11 unilaterally vestibulopathic subjects underwent measurement of optically best corrected DVA during head motion. A variable size letter "E" 6 m distant was displayed in oblique random orientations to deter

UNLABELLED: In 1973 Lücke described nystagmus induced by application of a vibrator on the mastoid process. Since that time, several Authors have applied the vibrator test in normal subjects or patients suffering from various well-characterized conditions.
OBJECTIVE: Recognizing that results obtained with the vibrator test are sometimes in contradiction with those of other vestibular tests, our aim was to formulate an assumption regarding the genesis of induced nystagmus in an attempt to explain such contradictions.
MATERIAL AND METHODS: We considered seven very different clinical situations.

Degenerative changes of the cervical spine often cause the pains and vertigo as well as pathological results of otoneurological tests. The aim of the study was to estimate the frequency of the symptoms occurrence in the group of 146 patients, 63 male and 83 female, with degenerative changes of the cervical spine. Permanent or periodical headaches of various intensity occurred in 60% of the patients. Balance disturbances was observed in 74% of the patients, specially in women. In the cervical spine X-ray there were observed most frequently marginal degenerations with osteophytosis (16%), interv

BACKGROUND: Assessment of nystagmus fixation suppression can be used as an additional diagnostic tool for patients with an acute vestibular syndrome to distinguish between a central or peripheral cause. We investigated the ability of physicians to detect fixation suppression using a nystagmus simulation model.
METHODS: We used a nystagmus simulator to measure the accuracy of the nystagmus fixation suppression test. Fixation suppression was assessed randomly in 6170 trials by 20 otorhinolaryngologists and neurologists, segregated into 2 groups based on their neurootological experience, a begin

A review of more than 20 years of experience with vibratory stimulation in vestibular diseases is given. Vibrations in a frequency frame of 30-60 Hz are applied on the mastoid during 20 s. Eye movements are recorded by video-oculography to detect a vibration-induced nystagmus (VIN). A VIN occurs in cases of a side difference between the peripheral vestibular receptors beating to the side of better excitability. In patients with central lesions, a VIN does not appear generally, only exceptionally if the lesion is located laterally in the brain stem. In healthy subjects, a VIN does not occur. Th

Though benign paroxysmal positional vertigo (BPPV) is the most common vestibular disorder causing positional vertigo, patients with typical positional vertigo in which the findings of positional nystagmus do not meet the diagnostic criteria for BPPV are often encountered in the clinic. Recently a concept of the light cupula was introduced, which accounts for some of positional vertigo. Under a normal condition in which the specific gravity of the cupula is same as that of the surrounding endolymph, semicircular canals (SCCs) are not influenced by the gravity. The light cupula, which indicates

Benign paroxysmal positional vertigo, vestibular neuronitis and Menière's disease cause most cases of acute vertigo. However, doctors must consider central neurological reasons to vertigo. If it is determined that a patient has oto-neurological vertigo, the next task is to determine whether the patient has a peripheral or a central cause of vertigo, if the condition is potentially lethal and if there is a need for acute radiological imaging and/or medical intervention. This review highlights the oto-neurological approach to the dizzy patient with particular focus on the patient's history, clin

In conclusion, the assessment of the dizzy patient is indeed complex and far-reaching. The expertise of family practitioners, internist, neurologists, otolaryngologists, and neurosurgeons at times is essential and one should not feel inhibited about referring the patient for further evaluation to our colleagues so that the patient may best be served and resolution of the patient's complaints may result.

After stapedectomy otoliths were displaced from the endolymphatic space to the cupula of the posterior semicircular canal and caused a paroxysmal benign positional nystagmus which lasted 18 months. The positional nystagmus demonstrated a rotatory component during the Hallpike manoeuvre. The paroxysmal benign nystagmus disappeared after transtympanic nerve neurectomy of the inferior branch of the vestibular nerve (Gacek 1974). This procedure is only recommended for vertigo lasting longer than 1 year. The current findings support the theory that cupulolithiasis is the cause of paroxysmal benign
