Maintenance of wakefulness test and multiple sleep latency test. Measurement of different abilities in patients with sleep disorders.

The multiple sleep latency test and the maintenance of wakefulness test were administered on the same day to 258 consecutive patients whose clinical presentation required evaluation for excessive sleepiness. While the MSLT is the standard test for assessing excessive daytime sleepiness, the MWT may have some clinical advantage over the MSLT when the assessment of daytime alertness is the primary goal. To explore further the relationship between alertness and sleepiness, we have conducted a thorough analysis of the similarities, differences, and correlations between MWT and MSLT. The results of…

Memorialization practices in the context of the COVID-19 pandemic in Brazil: the online wake ritual.

This study aimed to understand the online wake ritual as a virtual Memorialization practice in the context of the COVID-19 pandemic in Brazil. This is an exploratory and descriptive qualitative study, with document analysis as the main study design. A search was conducted on the Google platform, with links published between March 11, 2020 and July 27, 2021, in Portuguese, with "Brazil" as the geographic definition. Among the 4924 links identified, 162 links referred to online news articles published in the context of the pandemic and available for full and free access. Among these, 65 were sel

Where Are We Now? An Update on Colectomies for Diverticulitis in the Wake of the COVID-19 Pandemic.

BackgroundThe COVID-19 pandemic strained hospital systems and post-pandemic data on colectomy trends for diverticulitis are limited. We aimed to describe trends in colonic resections in patients with diverticulitis before, during, and after the pandemic.MethodsThis retrospective cohort study queried the American College of Surgeons NSQIP database, analyzing patients who underwent a colectomy for diverticulitis during quarters 2-4 (April 1-December 31) in 2018 ("pre-COVID"), 2020 ("during COVID"), and 2022 ("post-COVID"). The primary study outcome was emergency case rates during the 3 time peri

Distinct EEG Features and Sleep/Wake Behaviour Predict Anxiety Phenotypes in Mice.

Preoperative anxiety is a prevalent clinical problem linked to adverse surgical outcomes, including elevated pain, prolonged hospital stays and increased risk of postoperative delirium. Current assessments rely heavily on subjective measures, necessitating objective biomarkers. We investigated whether baseline electroencephalography (EEG) and sleep-wake profiles can predict anxiety levels in a mouse model. Fifty male C57BL/6N mice were implanted with EEG and electromyography (EMG) electrodes, recorded for 24 h post-recovery before undergoing a cued fear-conditioning protocol. Six behavioural i

Sleep-Wake Dysregulation in Human African Trypanosomiasis: From Neuroinvasion to Neuronal Dysfunction.

Human African trypanosomiasis (HAT) or sleeping sickness is a neglected tropical disease with a progressive central nervous system (CNS) involvement and marked sleep and circadian rhythm abnormalities. Even though this is a prominent feature of HAT, the connection between parasite neuroinvasion, neuroinflammation, circadian dysfunction, and neurological impairment is not fully understood. This narrative review aims to summarize the most up-to-date knowledge about sleep and circadian disturbance in HAT and proposes an integrated approach for the Trypanosome-Associated Sleep Disorder (TASD). The

Chronotype-specific associations of sleep-wake schedule with behavioral, physiological, and psychological parameters.

Diurnal variations in human performance are known to be associated with chronotype, which reflects individual differences in preferred sleep … s, irrespective of sleep-wake schedule, whereas cognitive performance peaked at different stages of wakefulness-earlier for ECs and later for LCs-independent of the time of day. These findings suggest that chronotype-aligned sleep-wake schedules could help optimize fatigue management, cognitive performance, and motor control. Diurnal variations in performance …

Obstructive sleep apnea syndrome, orexin, and sleep-wake cycle: The link with the neurodegeneration.

Obstructive sleep apnea syndrome (OSAS) significantly affects the sleep-wake circadian rhythm through intermittent hypoxia and chronic sleep fragmentation. OSAS patients often experience excessive daytime sleepiness, frequent awakenings, and sleep fragmentation, leading to a disrupted circadian rhythm and altered sleep-wake cycle. These disruptions may exacerbate OSAS symptoms and contribute to neurodegenerative processes, particularly through the modulation of clock gene expression such as CLOCK, BMAL1, and PER. Emerging evidence connects OSAS to cognitive impairment and suggests that these c

Early neurological wake-up test in intubated patients with traumatic brain injury.

BACKGROUND: Daily wake-up has been implemented widely in intensive care units (ICU) and could improve the patients' prognosis. However, little is known about the benefit of early neurological wake-up test (ENWT) in patients with acute traumatic brain injury (TBI). We aimed to investigate the role of ENWT as a clinical monitoring tool for TBI and its association with prognosis. METHODS: This is an observational retrospective study included intubated and continuously sedated TBI in ICU, and all data were extracted from three tertiary hospitals from China. The main exposure of interest was ENWT,

Towards Effective Helicobacter pylori Eradication: Emerging Therapies in the Wake of Antibiotic Resistance.

Helicobacter pylori (H. pylori) infection is a leading cause of gastritis, peptic ulcers, and gastric cancer, affecting more than half of the global population. Its persistence in the acidic gastric environment and its ability to evade host immunity present major treatment challenges. Although antibiotics remain the standard therapy, rising antimicrobial resistance has reduced treatment efficacy, prompting the search for alternative and adjunct approaches. Emerging therapies include probiotics, antimicrobial peptides (AMPs), and plant-derived compounds, which target H. pylori through membrane

Adverse Health Consequences of Poor Air Quality in Nepal: A Wake-Up Call.

Literatures shows that poor air quality index (AQI) is associated with several adverse health impacts, such as asthma, chronic obstructive pulmonary disease, diabetes mellitus, tuberculosis, cancer, pneumonia, cataracts, heart diseases, and mental health disorders. Nepal, one of the lower-middle-income countries, has become vulnerable to adverse health impacts due to poor AQI over time. The capital city of Nepal, Kathmandu, is frequently placed as one of the most polluted cities by IQAir. As such, this statement marks a caution for policymakers and the public to be aware of the future effects

Absence seizures and sleep-wake abnormalities in a rat model of GRIN2B neurodevelopmental disorder.

OBJECTIVE: Pathogenic mutations in GRIN2B are an important cause of severe neurodevelopmental disorders resulting in epilepsy, autism, and intellectual disability. GRIN2B encodes the GluN2B subunit of N-methyl-d-aspartate receptors (NMDARs), which are ionotropic glutamate receptors critical for normal development of the nervous system and synaptic plasticity. Here, we characterized a novel Grin2b heterozygous knockout rat model with electroencephalography (EEG) and pharmacological interventions to block spontaneous seizures. METHODS: Through western blot analysis we assessed the extent of Glu

An integrate-and-fire mathematical model of sleep-wake neuronal networks in the developing mammal.

Sleep behavior is present in nearly all animals, and is a vital part of growth, development, and overall health. Infant mammals cycle randomly between short bouts of sleep and wake, and the lengths of these bouts both follow an exponential distribution. As mammals mature into adulthood, the mean sleep and wake bout lengths increase, and we also observe a change in the distribution of wake bout lengths from exponential to power law. Focusing on three regions of the brainstem that are involved in sleep-wake regulation, we develop a novel integrate-and-fire neuronal network model to expand upon p

Characteristics and Neural Mechanisms of Sleep-Wake Disturbances After Traumatic Brain Injury.

Sleep-wake disturbances (SWDs) are one of the most common complaints following traumatic brain injury (TBI). The high prevalence and socioeconomic burden of SWDs post-TBI have only been recognized in the past decade. Common SWDs induced by TBI include excessive daytime sleepiness (EDS), hypersomnia, insomnia, obstructive sleep apnea (OSA), and circadian rhythm sleep disorders. Sleep disturbances can significantly compromise quality of life, strain interpersonal relationships, diminish work productivity, exacerbate other clinical conditions, and impede the rehabilitation process of TBI patients

Wake slow waves in focal human epilepsy impact network activity and cognition.

Slow waves of neuronal activity are a fundamental component of sleep that are proposed to have homeostatic and restorative functions. Despite this, their interaction with pathology is unclear and there is only indirect evidence of their presence during wakefulness. Using intracortical recordings from the temporal lobe of 25 patients with epilepsy, we demonstrate the existence of local wake slow waves (LoWS) with key features of sleep slow waves, including a down-state of neuronal firing. Consistent with a reduction in neuronal activity, LoWS were associated with slowed cognitive processing. Ho…

[The significance of glymphatic pathway in the relationship between the sleep-wake cycle and neurodegenerative diseases].

Selective and progressive death of neurons is a characteristic feature of the process of neurodegeneration and leads to corresponding neuronal dysfunctions. Neurodegenerative diseases represent a heterogeneous group of clinically distinct disorders with similar molecular mechanisms of pathogenesis. They are based on the processes of abnormal aggregation of proteins, the formation of fibrillary insoluble structures and their deposition in the form of histopathological inclusions in the tissues of the nervous system. Disturbance of homeostatic functions that regulate neuronal ion and energy meta

State Funding For Substance Use Disorder Treatment Declined In The Wake Of Medicaid Expansion.

The US continues to grapple with an escalating epidemic of opioid-related overdose and mortality. State funds, which are the second-largest source of public funding for substance use disorder (SUD) treatment and prevention, play a critically important role in responding to this crisis. Despite their importance, little is known about how these funds are allocated and how they have changed over time, particularly within the context of Medicaid expansion. In this study we assessed trends in state funds during the period 2010-19, using difference-in-differences regression and event history models.
