Role of the Chest Wall in Newborn Respiratory Function at Birth.

The chest wall significantly impacts respiratory function after birth, but its role in the newborn remains poorly understood as it is structurally and functionally different from adults. In neonates, the chest wall is highly compliant, which allows it to expand to accommodate the incoming air and the lung liquid cleared into the pulmonary interstitium during lung aeration. However, the high neonatal chest wall compliance predisposes it to distortion, which reduces breathing efficiency and necessitates respiratory muscle activation to stabilize it. This increases the work of breathing and, when…

Assessment of respiratory flow cycle morphology in patients with chronic heart failure.

Breathing pattern as periodic breathing (PB) in chronic heart failure (CHF) is associated with poor prognosis and high mortality risk. This work investigates the significance of a number of time domain parameters for characterizing respiratory flow cycle morphology in patients with CHF. Thus, our primary goal is to detect PB pattern and identify patients at higher risk. In addition, differences in respiratory flow cycle morphology between CHF patients (with and without PB) and healthy subjects are studied. Differences between these parameters are assessed by investigating the following three c

Analysis of contrast-enhanced dynamic MR images of the lung.

Recent studies have demonstrated the potential of dynamic contrast-enhanced magnetic resonance imaging (MRI) describing pulmonary perfusion. However, breathing motion, susceptibility artifacts, and a low signal-to-noise ratio (SNR) make automatic pixel-by-pixel analysis difficult. In the present work, we propose a novel method to compensate for breathing motion. In order to test the feasibility of this method, we enrolled 53 patients with pulmonary embolism (N = 24), chronic obstructive pulmonary disease (COPD) (N = 14), and acute pneumonia (N = 15). A crucial part of the method, an automatic

[Pulmonary or circulatory causes of dyspnea - value of spiroergometry].

Patients often report of dyspnea during exertion which should be further analysed as a hint of heart or lung disease. When case history, chest X-ray, ECG, spirometry, and methacholine test do not explain the complaints, a cardiopulmonary exercise test is recommended. Parameters of spiroergometry can often elucidate cardiocirculatory or pulmonary causes. In cardiocirculatory triggered dyspnea the following pattern can be found: reduced maximal oxygen consumption and work rate (WR), an early plateau of oxygen consumption/WR, reduced increase of oxygen consumption/WR, reduced heart rate reserve <

Functional morphology and physiology of slowly adapting pulmonary stretch receptors.

Since the original work by Hering and Breuer (1868) on slowly adapting pulmonary stretch receptors (SARs), numerous studies have demonstrated that these receptors are the lung vagal afferents responsible for eliciting the reflexes evoked by moderate lung inflation. SARs play a role in controlling breathing pattern, airway smooth muscle tone, systemic vascular resistance, and heart rate. Both anatomical and physiological studies support the contention that SARs, by their close association with airway smooth muscle, continuously sense the tension within the myoelastic components of the airways c

Technical validation of a multimodal emotion-adaptive biofeedback system for autonomic regulation using guided breathing.

Breathing interventions delivered through biofeedback have been shown to support physiological modulation consistent with resonance breathing responses. However, most biofeedback systems on the market are based on fixed procedures, provide limited physiological feedback, and do not adapt to user comfort or emotional state. The system is designed to work under a typical 20-min biofeedback timeframe and uses a two-stage adaptive respiration rate (RR) algorithm that gradually converges toward resonance frequency while dynamically responding to signs of emotional discomfort. System-level testing w

Noninvasive respiratory support and patient self-inflicted lung injury in COVID-19: a narrative review.

COVID-19 pneumonia is associated with hypoxaemic respiratory failure, ranging from mild to severe. Because of the worldwide shortage of ICU … pneumonia. Respiratory mechanics, vascular compromise, viscoelastic properties, lung inhomogeneity, work of breathing, and oesophageal pressure swings are discussed. The concept of P-SILI has been widely investigated in recent years, but controversies persist regarding its mechanisms. To minimise the risk of P-SILI, intensivists should better understand its underly …

Molecular/Ionic Basis of Vagal Bronchopulmonary C-Fiber Activation by Inflammatory Mediators.

Stimulation of bronchopulmonary vagal afferent C fibers by inflammatory mediators can lead to coughing, chest tightness, and changes in breathing pattern, as well as reflex bronchoconstriction and secretions. These responses serve a defensive function in healthy lungs but likely contribute to many of the signs and symptoms of inflammatory airway diseases. A better understanding of the mechanisms underlying the activation of bronchopulmonary C-fiber terminals may lead to novel therapeutics that would work in an additive or synergic manner with existing anti-inflammatory strategies.

Domiciliary High-Flow Nasal Therapy in Primary Ciliary Dyskinesia.

We report the case of an adolescent with severe primary ciliary dyskinesia (PCD) phenotype associated with a rare genotype. His clinical condition deteriorated, with daily cough and breathlessness, hypoxemia, and lung function decline. Despite being started on home noninvasive ventilation (NIV), the symptoms progressed to dyspnea at rest and thoracic pain. High-flow nasal cannula (HFNC) was started during the daytime as an adjuvant to NIV, and he was started on regular oral opioids for pain and dyspnea control. There was a clear improvement in comfort and dyspnea and breathing work relief. Add

Decompensated Differential Carbon Dioxide Removal During Venoarterial Extracorporeal Membrane Oxygenation: A Physiologic Framework.

Decompensated differential carbon dioxide (CO 2 ) removal can occur during spontaneous breathing on peripheral venoarterial extracorporeal membrane oxygenation (V-A ECMO) when a low right radial arterial partial pressure of carbon dioxide (PaCO 2 ) prompts sweep gas flow (SGF) reduction, decreasing extracorporeal CO 2 removal and shifting the compensatory burden to the native lung. When cardiac output (CO) remains low, this shift may require disproportionately higher alveolar ventilation, manifesting as tachypnea and increased work of breathing. We therefore frame this phenotype as a CO-depend…

Vocal Cord Dysfunction: Rapid Evidence Review.

Vocal cord dysfunction (i.e., vocal cords closing when they should be opening, particularly during inspiration) should be suspected in patients presenting with inspiratory stridor or wheezing; sudden, severe dyspnea (without hypoxia, tachypnea, or increased work of breathing); throat or chest tightness; and anxiety, particularly in females. Common triggers include exercise, asthma, gastroesophageal reflux disease, postnasal drip, upper or lower respiratory tract infection, and irritants. Nasolaryngoscopy and pulmonary function testing, with provocative exercise and methacholine, can help diagn…

Analysis of respiratory flow signals in chronic heart failure patients with periodic breathing.

In patients with chronic heart failure (CHF), oscillatory breathing pattern predicts poor prognosis. This work proposes a method to identify the respiratory pattern to determine periodic breathing (PB), Cheyne-Stokes respiration (CSR) and non-periodic respiratory patterns (nPB) through the respiratory flow signal. 26 patients are studied, classified in G(1) (PB), G(2) (CSR) and G(3) (nPB). The flow signal is filtered and normalized, to obtain the positive envelope that describes the respiratory pattern. With this new signal some features are extracted through its power spectral density (PSD).

[Methodology and evaluation of exposure to fumes formed during welding of chromium-nickel steel].

Using various methods for determination of welding fumes components and taking into account a simultaneous determination of soluble compounds of chromium III and IV, hygienic evaluation in two working places employing different systems of MMA/SS welding, TIG/SS welding and padding was carried out. As it is indicated by the results, proper hygienic evaluation of working conditions is possible owing to the measurements of individual exposure of workers in the breathing zone, allowing for determination of chromium manganese, nickel, and, while applying low-hydrogen electrodes, also fluorides, in

Wearable face mask-attached disposable printed sensor arrays for point-of-need monitoring of alkaline gases in breath.

Blood sampling, despite its historical significance in clinical diagnostics, poses challenges, such as invasiveness, infection risks, and limited temporal fidelity for continuous monitoring. In contrast, exhaled breath offers a noninvasive, pain-free, and continuous sampling method, carrying biochemical information through volatile compounds like ammonia (NH3). NH3 in exhaled breath, influenced by kidney function, emerges as a promising biomarker for renal health assessment, particularly in resource-limited settings lacking extensive healthcare infrastructure. Current analytical methods for br

Is loaded breathing an inflammatory stimulus?

PURPOSE OF REVIEW: To summarize recent data indicating that loaded breathing generates an inflammatory response. RECENT FINDINGS: Loaded breathing initiates an inflammatory response consisting of elevation of plasma cytokines and recruitment and activation of lymphocyte subpopulations. These cytokines do not originate from monocytes but are instead produced within the diaphragm secondary to the increased muscle activation. Oxidative stress is a major stimulus for the cytokine induction secondary to loaded breathing. The production of cytokines within the diaphragm may mediate the diaphragm mu

Breath Biomarkers of Pediatric Malaria: Reproducibility and Response to Antimalarial Therapy.

BACKGROUND: Many insect-borne pathogens appear to manipulate the odors of their hosts in ways that influence vector behaviors. In our prior work, we identified characteristic changes in volatile emissions of cultured Plasmodium falciparum parasites in vitro and during natural human falciparum malaria. In the current study, we prospectively evaluate the reproducibility of these findings in an independent cohort of children in Blantyre, Malawi. METHODS: We enrolled febrile children under evaluation for malaria and collected breath from children with and without malaria, as well as healthy contr
