OBJECTIVE: To describe the case of a 13-month-old girl who developed copper deficiency as a result of excessive zinc gluconate ingestion.
SETTING: Tertiary care hospital in Syracuse, NY.
INTERVENTIONS: Cessation of zinc ingestion followed by intravenous and oral copper chloride therapy.
MEASUREMENTS/MAIN RESULTS: Ingestion of zinc gluconate, 120 mg/d for 6 months, and thereafter 180 mg/d for 1 month, preceded the clinical presentation of listlessness, anemia, neutropenia, poor weight gain, abnormal sparse hair, and scorbuticlike bone changes. The effect of proteinuria on urinary zinc and copper excretion was studied in children with nephrotic syndrome (NS). Clearance, fractional excretion, and urinary excretion of zinc and copper were significantly higher in children with relapse of NS than in the same children with remission of NS or in healthy children. A linear correlation was found between proteinuria and urinary zinc and copper excretion in relapse of NS. The results of this study suggest that zinc and copper deficiency in NS may be related also to increased urinary zinc and copper losses.

Zn kinetics were studied in two surgical patients and compared with the findings in three controls. One patient had malabsorption following resection of part of the small intestine, the other was suffering from malabsorption due to radiation enteropathy incurred more than 10 years earlier. Serum zinc levels were estimated in 146 patients with different types of leprosy and 40 control subjects. Tuberculoid patients were found to have normal serum zinc levels. A systematic study of the solvent extraction of Zn, Cd and Hg salicylates is reported. Optimum conditions for extraction and separation of Zn, Cd and Hg are evaluated from a critical study of pH, sodium salicylate concentration, mesityl oxide concentration, period of equilibration, and effect of diverse ions. A separation scheme is described and results are reported for analysis of synthetic mixtures and commercial samples.

We have postulated that the sickle erythrocyte becomes membrane damaged at least partly due to excessive accumulation of calcium and then to excessive calmodulin activation (42).
