Vestibular disease unmasked by hyperventilation.

Hyperventilation-induced dizziness is often thought to be psychogenic, but its effects in the presence of known vestibular disease have not been adequately examined. In this study hyperventilation was tested in two models of vestibular disease. These were, first, patients with profound unilateral vestibular deficit (prior translabyrinthine acoustic neuroma resection [postsurgery group]) and, second, patients with variable unilateral vestibular deficit (unoperated unilateral acoustic neuroma [presurgery group]). Patients were hyperventilated for 90 seconds. Using infrared videonystagmography, 1

Teflon granuloma of the larynx: etiology, pathophysiology, and management.

Intralaryngeal Teflon injection for correction of unilateral vocal cord paralysis is known to produce a foreign body giant cell reaction. In our practice, we have seen increasing numbers of patients who had developed dysphonia related to Teflon injection. This patient series was reviewed, as were the surgical technique to correct this condition, voice results, and acoustic analysis of a subset of the patient series. We conclude that dysphonia, secondary to Teflon injection, can be either from overinjection of Teflon or inappropriate injection, or from the proliferative granulomatous response o

Ruptured distal anterior inferior cerebellar artery aneurysm--case report.

A 62-year-old female presented with a distal anterior inferior cerebellar artery (AICA) aneurysm showing severe involvement of the facial and acoustic nerves manifesting as progressive left hearing disturbance and left facial nerve paresis. She was admitted to our hospital 2 months after the onset. A saccular aneurysm arising from the meatal loop of AICA was found between the facial and acoustic nerves which were damaged directly by the aneurysm. Magnetic resonance imaging demonstrated a vascular anomaly, differentiating this disorder from cerebellopontine angle tumor, and evidence of an old s

Central neurofibromatosis: a clinical-pathological correlation.

Central neurofibromatosis is a genetic disorder of neural crest tissue derivatives that includes bilateral acoustic neuromas and other central nervous system tumors, usually meningiomas or gliomas. This is different from peripheral neurofibromatosis because of the primary central nervous system manifestations and frequent lack of accompanying peripheral neurofibromas and café au lait spots as well as different alterations of nerve growth factor. We present the temporal bone histopathology of a unique case of unsuspected central neurofibromatosis that included bilateral acoustic neuromas invadi

Neurophysiologic monitoring in posterior fossa surgery. I. Technical principles, applicability and limitations.

In 135 cases of posterior fossa surgery almost exclusively in the cerebellopontine angle (CPA) intraoperative monitoring of brain-stem acoustic evoked potentials (BAEP) and partly somatosensory evoked potentials (SEP) was performed. The series consisted of 20 microvascular decompressions, 63 acoustic neurinomas, 7 vascular lesions and 45 other space occupying lesions, mostly in the CPA. BAEP monitoring alone was employed in 76 cases, combined BAEP and SEP monitoring less frequently. The technique of anaesthesia and intraoperative monitoring is presented in detail including an analysis of techn

Auditory experience affects degeneration of the ventral cochlear nucleus in Mongolian gerbils.

Gerbils exhibit a unique encephalopathy characterized by spongioform lesions in the neuropil of the cochlear nucleus and that others have recently described. The present results suggest that the course of this degenerative disorder is affected by acoustic experience. In gerbils in which acoustic stimulation was limited postnatally, the number and the extent of these lesions was dramatically reduced. Monaural deprivation reduced lesion number and extent only in the ipsilateral cochlear nucleus; binaural deprivation affected both cochlear nuclei. The lesions were most evident in certain portions

[Determination of T2-times of tumor and muscle tissue by nuclear magnetic resonance tomography and resonance spectroscopy--results of a comparative study].

In 13 patients the T2 values of muscular and various tumor tissues was determined by MR under in-vivo conditions. The tumors were confirmed by surgery. In 5 cases there was a neurinoma of the acoustic nerve, in 3 cases a lateral neck cyst, a granuloma of the larynx and some other pathological processes. Intraoperatively the tissues are biopsied and the T2* time is determined by spectroscopy in vitro. Both the muscular and the tumor tissue show higher mean values or standard deviations in MR measurements than in measurements by spectroscopy. The relations between muscular and tumor tissue (quot

The combination of scanning and transmission electron microscopy techniques in pathology of the organ of Corti in guinea pigs.

The pathology of the organ of Corti in guinea pigs is studied by means of SEM and TEM. It was the purpose of this study to examine the surface of an experimentally altered organ of Corti by means of SEM screening method). As it was our main interest to find out the localization of acoustic stimulation regarding the tonotopicity on the surface of the organ of Corti, we always examined the whole cochlear by means of SEM first. In acoustic experiments we used long-time pure-tone stimulation beyond the directly damaging sound pressure level, as well as white noise or shots for experimental damage

Auditory brainstem evoked responses in brainstem compression due to posterior fossa tumors.

Five patients with unilateral posterior fossa tumors were investigated with Auditory Brainstem Evoked Responses (B.E.R.). The B.E.R., recorded from the ear ipsilateral to the tumor, were consistent with a lesion of the acoustic nerve or the cochlear nuclei in all patients. The contralaterally evoked responses in four patients with large tumors and radiological signs of brainstem compression showed normal latencies and amplitudes of the first three waves, while waves IV and V were definitely abnormal with respect to amplitudes and latencies. (The 4th patient had a normal wave IV and a delayed,

Relief of the occluded ear sensation to improve earmold comfort.

Earmold comfort is an important factor in the overall satisfaction a hearing-impaired person has with a hearing aid system. Recent research into the subjective effects of earmold venting indicates that, for the naive hearing-aid user, the solid mold produces a "closed" and "blocked" sensation that is relieved to a degree, depending on the vent diameter. To improve earmold comfort, all earmolds should be vented. However, there are certain types of hearing loss where conventional venting techniques would be undesirable, in particular, cases of severe to profound hearing loss where venting would

Echocardiogram and phonocardiogram related to the movement of the pulmonary valve.

Pulmonary valve movement and the related acoustic phenomena were investigated using high speed strip-chart echo- and phonocardiographic recording. The opening of the pulmonary valve had no definite relationship to the acoustic phenomena, whereas the pulmonary ejection sound was closely related in time to the early systolic maximal opening of the valve. The concomitant pulmonary ejection systolic murmur faded away by the time of the mid-systolic semi-closure of the valve, where the tiny extrasound occurred in a half of cases. The pulmonary component of the second heart sound occurred after the

[The use of long pure-tone stimuli with relatively long-rise-times in clinical electro-cochleography (author's transl)].

The clinical use of the action potentials (SAP) of the acoustic nerve stimulated by sinustone bursts of relatively fast rise times has been described extensively elsewhere. The use of sinus tone bursts with relatively long rise times has the advantage of high frequency specifity. Thresholds for frequences of 500 Hz upwards may be found within a +/-12.5 dB SL confidence intervall. Judging the relation between the N1 amplitude in muV and the stimulus level in dBHL - the socalled Input/Output (I/O) curve - a differentiation between recruiting and non recruiting receptor behaviour can be made. The Effects of Electromagnetic Articulography Sensors on Speech in Individuals With and Without Parkinson's Disease.

PURPOSE: This study examined how wearing electromagnetic articulography (EMA) sensors affects acoustic and perceptual speech outcomes in people with Parkinson's disease (PwPD) with dysarthria and neurologically healthy control speakers. Additionally, the study explored potential after-effects on acoustic and perceptual measures following approximately 45 min of wearing EMA sensors in both groups. Finally, we investigated whether wearing EMA sensors or after-sensor effects differentially impacted the two groups. METHOD: Thirty-four speakers (21 controls and 13 PwPD) read "The Caterpillar" pass

Impact of Simulated Artifacts on the Classification Performance of Apical Views in Transthoracic Echocardiography Using Convolutional Neural Networks.

Background: In recent years, artificial intelligence (AI) methods, including deep convolutional neural networks (CNNs), have gained increasing importance in supporting the automated analysis of echocardiograms. The aim of this study was to evaluate the impact of selected image artifacts-motion blur, acoustic shadowing, and speckle noise-on the performance of automatic classification of standard transthoracic echocardiographic (TTE) views using deep learning models. Methods: The analysis included 217 TTE video clips (2170 frames) covering apical views: two-chamber (A2C), three-chamber (A3C), fo

Development and Evaluation of a Low-Cost Open-Source Nasometer.

Hypernasality is a common characteristic of several speech disorders and can significantly affect perceived speech intelligibility and quality. Nasometry quantifies nasalance by calculating the proportion of acoustic energy emitted from the nasal cavity relative to the combined nasal and oral acoustic output during speech production and is commonly used in clinical assessment and research. However, commercially available nasometers are costly and limited in portability, restricting their use in resource-limited or remote settings. The primary purpose of this study was to design and build a low

A simulation study of transcranial magnetoacoustic stimulation of the basal ganglia thalamic neural network to improve pathological beta oscillations in Parkinson's disease.

BACKGROUND: Parkinson's disease (PD) is a common neurodegenerative disease. Transcranial magnetoacoustic stimulation (TMAS) is a new therapy that combines a transcranial focused acoustic pressure field with a magnetic field to excite or inhibit neurons in targeted area, which suppresses the abnormally elevated beta band amplitude in PD states, with high spatial resolution and non-invasively. OBJECTIVE: To study the effective stimulation parameters of TMAS mononuclear and multinuclear stimulation for the treatment of PD with reduced beta band energy, improved abnormal synchronization, and no t

A comparative analysis of low intensity ultrasound effects on living cells: from simulation to experiments.

Ultrasounds are already broadly exploited in clinical diagnostics and are now becoming a powerful and not harmful tool in antitumoral therapies, as they are able to produce damages towards cancer cells, thank to inertial cavitation and temperature increase. The use of US alone or combined to molecular compounds, microbubbles or solid-state nanoparticles is the focus of current research and clinical trials, like thermoablation, drug sonoporation or sonodynamic therapies. In the present work, we discuss on the non-thermal effects of ultrasound and the conditions which enable oxygen radical produ
