Psychopharmacology in child and adolescent psychiatry: a review of the past seven years. Part I.

OBJECTIVE: To present a critical overview of the literature published in the past 7 years on the efficacy and safety of psychoactive agents in mental retardation with associated psychiatric disorders, autistic disorder, Tourett's disorder, and attention-deficit/hyperactivity disorder. METHOD: Double-blind and placebo-controlled clinical trials and open studies were reviewed and selected reports presented. RESULTS: The literature review reveals that progress has been made in the psychopharmacological treatment of the above conditions. This is partly because more studies use larger sample size

Editorial Perspective: Evolutionary psychiatry as a scientific framework for neurodiversity: implications for ADHD and ASD in children and young people.

The neurodiversity movement has gained considerable momentum in recent years, asserting that conditions such as attention-deficit/hyperactivity disorder (ADHD) and autism spectrum disorder (ASD) represent natural variations in human neurodevelopment rather than pathological disorders (Baron-Cohen, 2017). This perspective has found increasing resonance among people with lived experience, advocacy groups and a growing number of clinicians. Yet, it has also attracted criticism for lacking a rigorous scientific foundation, leaving it vulnerable to the charge that it is more ideological than eviden

The neuropsychology of basal ganglia.

Basal ganglia are subcortical structures specialized at very early age, functionally different according to the right or left side. They are part of complex distributed network composed by parallel segregated loops where specific information are processed and open loops where different information are integrated. These loops are connected to specialized cortical areas thus entering into distributed processing of higher order cognitive functions and behaviours. Lesion or malfunction of basal ganglia nuclei cause deficits in different neuropsychological functions and neurobehavioural diseases, s

Kohlschütter-Tönz Syndrome - Report of an additional case.

Kohlschütter-Tönz Syndrome is a rare disorder clinically characterized by amelogenesis imperfecta, epilepsy and progressive mental deterioration. We present an additional case of this syndrome of a nine year-old boy who was referred by pigmented teeth. The mental deterioration was associated with speech delay, impulsive behavior, attention-deficit/hyperactivity disorder, and learning problems. The physical examination revealed a reduction of lower third, slightly palpebral fissures, low ear and hair implantation, coarse hair and hypertrichosis. The intraoral examination showed alteration in te

Auditory and visual processing in Williams syndrome.

Williams syndrome is a neurodevelopmental disorder caused by a deletion on chromosome 7. It is characterized by a range of medical problems in addition to severe impairments in visuospatial processing and oversensitivity to sounds, including hypersensitivity to sounds (hyperacusis) and extreme fear from sounds (phonophobia). In spite of impairments in visuospatial processing, object and face processing abilities are relatively preserved in WS.The present review discusses the growing research in the field linking the unique sensory phenotype in WS with underlying structural and functional brain

Sensitivity and specificity of TRH stimulation test in depressed and nondepressed adolescents.

The thyrotropin-releasing hormone (TRH) stimulation test was carried out in 100 acutely hospitalized adolescents (ages 13-17 years) with a variety of diagnoses. The test was positive (delta max thyrotropin less than or equal to 7 microIU/ml) in 36.6% of adolescents with major depression, 20.8% with conduct disorders, 17% with adjustment disorders, and 43% with substance abuse disorders. A X2 analysis of the data showed no significant difference among these four diagnostic groups. The test was negative in adolescents with attention deficit disorder and dysthymic disorder. Comparison of the six

Comparison of various WISC-R summary scores for a psychiatric sample.

Compared two sets of summary scores for the Wechsler Intelligence Scale for Children-Revised (WISC-R). The traditional scores used to summarize information about WISC-R performance (i.e., Verbal IQ (VIQ), Performance IQ (PIQ) and discrepancy between VIQ and PIQ) were compared to scores based on the three factors identified by Kaufman (1975) (i.e., Verbal Comprehension (VC), Perceptual Organization (PO) and Freedom from Distractibility (FD). Ss were 260 psychiatric outpatients who were administered the WISC-R during a 4-year period. The independent variables were sex and DSM II diagnosis. There

The impact of ADHD on attentional bias modification outcomes: Evidence from gaze-contingent music reward therapy for social anxiety disorder.

BACKGROUND: Attention Bias Modification (ABM) for social anxiety disorder (SAD) aims to reduce anxiety by training attention away from social threats. ABM training involves various attentional processes, including on-task sustained attention, a faculty that is typically impaired in individuals with attention deficit/hyperactivity disorder (ADHD). Thus, ABM's efficacy for individuals with comorbid ADHD remains uncertain. This study examined whether comorbid ADHD moderates outcomes of Gaze-Contingent Music Reward Therapy (GC-MRT), an emerging eye-tracking-based ABM intervention for SAD. METHODS

Executive dysfunction in medication-naïve children with ADHD: A multi-modal fNIRS and EEG study.

OBJECTIVE: Children with attention deficit hyperactivity disorder (ADHD) exhibit deficits in executive function. Since there are no clear biomarkers for the disorder, this study aimed to investigate the neurophysiological biomarkers for deficits in executive function in children with ADHD using functional near-infrared spectroscopy (fNIRS) and electroencephalography. METHODS: Twenty patients diagnosed with ADHD and 19 typically developing children (TDC; 8-11 years old) were included. Event related potentials (ERPs) were recorded using an electroencephalogram (EEG) and oxygenated hemoglobin co

The efficacy of a training that combines activities on working memory and metacognition: Transfer and maintenance effects in children with ADHD and typical development.

Introduction: It has been demonstrated that children with attention deficit and hyperactivity disorder (ADHD) have impairments in working memory (WM), and particularly its visuospatial component, responsible for academic underachievement. Furthermore, children with ADHD have difficulty in metacognition, and consequently use inappropriate strategies to control attention and impulsive behavior. The aim of the present study was to devise a training that combined individual exercises on visuospatial WM and group metacognitive activities capable of helping children with ADHD to ameliorate their per

Are fetal growth impairment and preterm birth causally related to child attention problems and ADHD? Evidence from a comparison between high-income and middle-income cohorts.

BACKGROUND: Cross-cohort comparison is an established method for improving causal inference. This study compared 2 cohorts, 1 from a high-income country and another from a middle-income country, to (1) establish whether birth exposures may play a causal role in the development of childhood attention problems; and (2) identify whether confounding structures play a different role in parent-reported attention difficulties compared with attention deficit hyperactivity disorder (ADHD) diagnoses. METHODS: Birth exposures included low birth weight (LBW), small-for-gestational age (SGA), small head c

The effects of memory load and stimulus relevance on the EEG during a visual selective memory search task: an ERP and ERD/ERS study.

OBJECTIVE: Psychophysiological correlates of selective attention and working memory were investigated in a group of 18 healthy children using a visually presented selective memory search task. METHODS: Subjects had to memorize one (load1) or 3 (load3) letters (memory set) and search for these among a recognition set consisting of 4 letters only if the letters appeared in the correct (relevant) color. Event-related potentials (ERPs) as well as alpha and theta event-related synchronization and desynchronization (ERD/ERS) were derived from the EEG that was recorded during the task. RESULTS: In

Auditory and visual automatic attention deficits in developmental dyslexia.

Several studies have provided evidence for a phonological deficit in developmental dyslexia. However, recent studies provide evidence for a multimodal temporal processing deficit in dyslexia. In fact, dyslexics show both auditory and visual abnormalities, which could result from a more general problem in the perceptual selection of stimuli. Here we report the results of a behavioral study showing that children with dyslexia have both auditory and visual deficits in the automatic orienting of spatial attention. These findings suggest that a deficit of selective spatial attention may distort the

The effects of increasing resource demand on vigilance performance in adults with schizophrenia or developmental attentional/learning disorders: a preliminary study.

The goal of this study was to assess whether degree of information processing load differentially affects vigilance performance in patients with schizophrenia as compared to normal controls or patients with other attentional disorders. We contrasted the performance of clinically stable outpatients with schizophrenia (n = 18), with that of normal controls (n = 17), and adults with developmental attentional/learning disorders (n = 13) on simple and demanding versions of a visual continuous performance test (CPT). Patients with schizophrenia were significantly impaired on both versions of the CPT

Potential scalp stimulation targets for mental disorders: evidence from neuroimaging studies.

Mental disorders widely contribute to the modern global disease burden, creating a significant need for improvement of treatments. Scalp stimulation methods (such as scalp acupuncture and transcranial electrical stimulation) have shown promising results in relieving psychiatric symptoms. However, neuroimaging findings haven't been well-integrated into scalp stimulation treatments. Identifying surface brain regions associated with mental disorders would expand target selection and the potential for these interventions as treatments for mental disorders. In this study, we performed large-scale m

Diffusion imaging, white matter, and psychopathology.

The functional significance of the brain's white matter was not fully appreciated until new imaging methods were developed to visualize fiber pathways and connections in the living brain. Rapid advances in diffusion tensor imaging (DTI) have led to substantial insights into human brain development and disease processes and have thrust white matter into the focus of researchers and clinicians alike. The full clinical potential of this relatively new technique remains to be determined, but early indicators suggest that DTI will be a significant new technology in mapping mechanisms of human healt

"Dude, you don't have Tourette's:" Tourette's syndrome, beyond the tics.

While coprolalia is the most well-known symptom of Tourette's syndrome (TS), it affects only a minority of persons with the condition. TS is a chronic neurobiological condition consisting of vocal and motor tics. Many children with TS have associated obsessive-compulsive disorder (OCD) and/or attention deficit hyperactivity disorder (ADHD), both of which can interfere with school, peer, and family functioning more than the tics themselves. The article distinguishes TS from other tic disorders and reviews literature on epidemiology, etiology, clinical course, and diagnostic and treatment issues

Medication in the treatment of the behavioural sequelae of traumatic brain injury.

A significant proportion of children who sustain traumatic brain injury will go on to experience disturbance in their academic, emotional, and social functioning. There is a role for medication in the treatment of these late onset changes. This review will focus on the recognition and pharmacological treatment of the most common and most problematic presentations that may follow head injury, and supports the use of stimulant medication for secondary attention-deficit-hyperactivity disorder despite the small evidence base for use in children.
