Factors that influence the presence of a hospice in a rural community.

PURPOSE: The purpose of this study was to identify socioeconomic, physician-related, and rural-urban factors that may influence the presence of a Medicare-certified hospice in three rural-urban areas. DESIGN: This was secondary analysis of selected socioeconomic, physician-related, and rural-urban data from 3,140 counties using the 2005 Area Resource File, a county-level database. The county was the unit of analysis. METHODS: Descriptive statistics were calculated for selected socioeconomic, physician, and rural-urban variables for the data set of 3,140 counties. Logistic regression was used

A taxometric investigation of agoraphobia in a clinical and a community sample.

The nosological status of agoraphobia is controversial. Agoraphobia may be a distinct diagnostic entity or a marker of avoidance severity. The current study examines the latent structure of agoraphobia through the use of taxometric analysis. The latent structure of agoraphobia was examined in two independent samples, one comprising outpatients presenting for treatment for panic disorder (PD) with or without agoraphobia (n=365), and the other comprising community volunteers to a national mental health survey who experienced fear or avoidance of at least one prototypic agoraphobic situation (n=6

The pancreatic beta-cell in the islet and organ community.

The endocrine pancreas consists of highly vascularized and innervated endocrine mini-organs--the islets of Langerhans. These contain multiple types of hormone-producing cells, including the insulin-secreting beta-cell. The major task of the fully differentiated beta-cell is the tight regulation of blood glucose levels by secreting insulin into the blood stream. This requires molecular features to measure glucose and produce, process, and release insulin by exocytosis. Now multiple interactions with endocrine and nonendocrine islet cells as well as with other organs have been shown to affect th

[Acute community-acquired bacterial meningitis in adults].

Bacterial meningitis in adults is fatal in 20% of patients and leads to sequels in 30%. The clinical presentation includes two of the following four symptoms and signs: fever, headache, stiff neck, altered mental status. The essential ancillary test is the analysis of the cerebrospinal fluid. Sometimes, the lumbar puncture is not feasible or deferred (brain computer tomography), requiring antibiotics and corticosteroids early. 80% of bacterial meningitis are secondary to pneumococcus or meningococcus. Empirical antibiotics must be given as soon as possible and provide coverage for these both b

Microneurosurgical management of anterior communicating artery aneurysms.

BACKGROUND: Anterior communicating artery complex is the most frequent site of intracranial aneurysms in most reported series. Anterior communicating artery aneurysms are the most complex aneurysms of the anterior circulation due to the angioarchitecture and flow dynamics of the ACoA region, frequent anatomical variations, deep interhemispheric location, and danger of severing the perforators with ensuing neurologic deficits. The authors review the practical microsurgical anatomy, importance of preoperative imaging in surgical planning, and microneurosurgical steps in dissection and clipping o

Impact of physical symptoms on perceived health in the community.

OBJECTIVE: Physical symptoms, such as musculoskeletal pain, dizziness, or headache, are common. People with more symptoms are reported to use more healthcare and have higher sickness absenteeism. We studied the impact of the number of symptoms on perceived health in a community sample. METHODS: Between June 2005 and March 2006, a random sample of 4741 adults was selected from the records of five general practices in The Netherlands. They were sent a questionnaire regarding the frequency and impact of physical symptoms, and other factors that may influence health (potential confounders or modi

The impact of mancozeb on entomofauna communities in apple orchards.

Two Large scale field studies have been conducted to the same design in commercial apple orchards in north and south France. Mancozeb (Dithane M-45) was applied at its post-annex I use rate of 1.6 kg a.i./ha on four occasions with a spray interval of 7 days. Reference treatments of a water treated control and toxic reference of dimethoate were also included. Studies were of randomized block design with 4 replicates (4 plots of 6 adjacent rows of 8 trees, on a surface of approximately 16 x 21 m) per treatment. Leaf and canopy dwelling arthropods were sampled before and at regular intervals afte

Reinvestment and falls in community-dwelling older adults.

BACKGROUND: Falls are common in older adults and have many adverse consequences. In an attempt to prevent further incidents, elder fallers may consciously monitor and control their movements. Ironically, conscious movement control may be one factor that contributes to disruption of automaticity of walking, increasing the likelihood of subsequent falls. OBJECTIVE: The Movement Specific Reinvestment Scale (MSRS), which aims to measure the propensity for movement-related self-consciousness and for conscious processing of movement, was used to try to discriminate elder fallers from non-fallers.

Could patients' coughing have communicative significance?

Medical discourse positions patients with coughs and colds negatively, so consulting health services with 'minor' respiratory illness is therefore more accountable than for other medical problems. Patients face dilemmas since they must persuade doctors of the doctorability of their illness without being seen as hypochondriacal, and they risk losing face if doctors decide that there is nothing much wrong. It is known that the placement of non-lexical features of talk such as laughing or crying can have interactional meaning. Using a data set of video-recorded doctor-patient cough and cold consu

Estimating community drug abuse by wastewater analysis.

BACKGROUND: The social and medical problems of drug abuse are a matter of increasing global concern. To tackle drug abuse in changing scenarios, international drug agencies need fresh methods to monitor trends and patterns of illicit drug consumption. OBJECTIVE: We tested a sewage epidemiology approach, using levels of excreted drug residues in wastewater, to monitor collective use of the major drugs of abuse in near real time. METHODS: Selected drug target residues derived from use of cocaine, opiates, cannabis, and amphetamines were measured by mass spectrometry in wastewater collected at

Incidence and clinical characteristics of interstitial cystitis in the community.

We utilized physician-coded diagnoses and chart reviews to estimate the incidence of interstitial cystitis (IC) in women. A computer search of the Kaiser Permanente database was performed to identify newly coded diagnoses of IC (ICD-9 code 595.1) between May 2002 and May 2005. Chart reviews were performed and patient demographics, diagnosing physicians, and symptom characteristics were recorded. The IC incidence rate was 15 per 100,000 women per year. The mean age of the patients was 51 years (range 31-81 years). The most common presenting symptoms were frequency (70%), dysuria (52%), urgency

[Community-acquired methicillin-resistant Staphylococcus aureus].

Recently, methicillin-resistant Staphylococcus aureus (MRSA) has emerged as a cause of community-acquired (CA) infections among patients without established risk factors for MRSA. CA-MRSA strains mainly cause mild skin and soft tissue infections in otherwise healthy children and young adults, but can also cause severe necrotizing fasciitis and pneumonia. In contrast to nosocomial MRSA, CA-MRSA are, in general, susceptible to multiple antimicrobials and present a different genotype. Most CA-MRSA strains share the staphylococcal chromosomal cassette (SCCmec) type IV and produce Panton-Valentine

Transmission of Streptococcus pneumoniae in an urban slum community.

BACKGROUND: Inhabitants of slum settlements represent a significant proportion of the population at risk for pneumococcal disease in developing countries. METHODS: We conducted a household survey of pneumococcal carriage among residents of a slum community in the city of Salvador, Brazil. RESULTS: Among 262 subjects, 95 (36%) were colonized with Streptococcus pneumoniae. Children <5 years of age (OR, 8.0; 95% CI, 3.5-18.6) and those who attended schools (OR, 2.7, 95% CI, 1.2-6.0) had significantly higher risk of being colonized. Of 94 isolates obtained from colonized individuals, 51% had ser

Interspecies interactions within oral microbial communities.

While reductionism has greatly advanced microbiology in the past 400 years, assembly of smaller pieces just could not explain the whole! Modern microbiologists are learning "system thinking" and "holism." Such an approach is changing our understanding of microbial physiology and our ability to diagnose/treat microbial infections. This review uses oral microbial communities as a focal point to describe this new trend. With the common name "dental plaque," oral microbial communities are some of the most complex microbial floras in the human body, consisting of more than 700 different bacterial s

The changing face of Klebsiella pneumoniae infections in the community.

Klebsiella pneumoniae (Kp) is an important pathogen both in the community and the hospital setting. In the community the emergence of virulent organisms with predominantly K1/K2 capsular serotypes has been observed over the last 20 years, these pathogens cause a distinct clinical syndrome consisting of pyogenic liver abscesses, sometimes accompanied by meningitis and abscesses elsewhere. In the hospital environment, under heavy antibiotic usage, multiple drug resistance has been increasingly observed in Kp. Kp strains harbouring extended spectrum beta-lactamases (ESBL) and more recently metall
