Intracompartmental Pressure and Burn Depth Dynamics in Circumferential Deep Dermal Burns Treated with Bromelain-Based Enzymatic Debridement: A Retrospective, Hypothesis-Generating Single-Center Study.

Background/Objectives: Circumferential deep dermal burns generate progressive sub-eschar pressure with risk of burn-induced compartment syndrome (BICS). Bromelain-based enzymatic debridement is a proposed non-surgical decompressive strategy, but the temporal kinetics of intracompartmental pressure (ICP) normalization and their relationship to burn depth are poorly characterized. We explored these kinetics and introduce, as an exploratory descriptive concept, the Enzymatic Decompression Half-Time (EDT50), mapping the ICP normalization curve stratified by Laser Doppler Imaging (LDI) burn depth.

Primary epidural sporadic Burkitt lymphoma in a 3-year-old: Case report and literature review.

BACKGROUND: Burkitt lymphoma (BL) is a common tumor of childhood that usually arises in the abdomen or pelvis in its sporadic form. In a minority of cases, BL can present with CNS involvement, usually as a secondary site. Rarely, BL can arise primarily in the epidural space and present with back pain, or less commonly, acute myelopathy. This presentation is a surgical emergency and requires vigilant management. CASE DESCRIPTION: We describe a case of pediatric BL arising primarily within the epidural space and presenting with progressive difficulty walking in a 3-year-old boy. Progression to

Craniocerebral missile injuries in a combat zone: spectrum of injuries and lessons learnt.

BACKGROUND: High-velocity missile injuries are commonly encountered in war or war-like situations. Aggressive resuscitation, early evacuation to neurosurgical center, and application of neurosurgical principles remain tenets of success. METHODS: The spectrum of injuries and clinical profile of 14 such cases with craniocerebral missile injuries managed at our center in the northern sector were included. Site of injury, GCS at presentation, associated injuries, surgical intervention, duration of hospitalization, and recovery of the patient were analyzed. RESULTS: Five patients had sustained gu

Novel case of percutaneous access of afferent limb of Roux-en-Y hepaticojejunostomy for biliary decompression and jejunoplasty in a patient with primary sclerosing cholangitis.

Primary sclerosing cholangitis is a rare condition characterized by multifocal fibrotic bile duct strictures and progressive liver disease. Due to its recurrence even after liver transplantation, an alternative surgical procedure, the Roux-en-Y hepaticojejunostomy, is practiced with positive outcomes. We present a case of a 55-year-old female with history of primary sclerosing cholangitis and Roux-en-Y hepaticojejunostomy approximately 25 years ago who presented to the emergency department with acute cholangitis. Computed tomography of the abdomen revealed a dilated loop of small bowel in the

Clinical analysis of microvascular decompression in patients with hemifacial spasm: a retrospective study.

BACKGROUND: Unilateral contractions of the facial muscles characterize hemifacial spasm (HFS). Microvascular decompression (MVD) was widely accepted for the treatment of HFS. To investigate the operative effects and surgical complications in patients with HFS after MVD. METHODS: A retrospective analysis of 540 patients with HFS after MVD was conducted from January 2017 to May 2018. All patients were followed up for 2 years on average. Surgical effects were evaluated on the patients' manifestations according to the Cohen score classification. RESULTS: During the follow-up period, 455 patients

Cervical myelopathy due to subaxial calcium pyrophosphate dihydrate (CPPD) deposition with simultaneous asymptomatic crowned dens syndrome: two case reports.

BACKGROUND: There are few reports of cervical myelopathy caused by an attack of subaxial calcium pyrophosphate dihydrate (CPPD) deposition. Moreover, there has been no report on cervical myelopathy by subaxial CPPD deposition with simultaneous asymptomatic crowned dens syndrome (CDS) at the same time. CASE PRESENTATION: The first case was a 68-year-old male complaining of cervical myelopathic symptoms. Plain radiographs, computed tomography (CT) and magnetic resonance imaging (MRI) findings revealed spinal cord compression by calcified round lesions at C4 as well as a calcified lesion behind

Acute atraumatic carpal tunnel syndrome due to flexor tendon rupture following palmar plate osteosynthesis in a patient taking rivaroxaban.

INTRODUCTION: Few case reports describe the development of a hematoma under oral anticoagulation as the cause of an atraumatic carpal tunnel syndrome. CASE REPORT: A 76 years old woman presented an acute atraumatic carpal tunnel syndrome of her left hand under oral anticoagulation with rivaroxaban due to atrial fibrillation. 12 years ago, palmar plate osteosynthesis of a distal radius fracture had been performed on the affected wrist. Open decompression of the carpal canal was performed due to persistent severe pain under intense pain therapy and progressive neurological symptoms. The cause o

Lumbar Disc Herniation Presented with Contralateral Symptoms.

OBJECTIVE: This study aimed to unravel the putative mechanism underlying the neurologic deficits contralateral to the side with lumbar disc herniation (LDH) and to elucidate the treatment for this condition. METHODS: From January 2009 to June 2015, 8 patients with LDH with predominantly contralateral neurologic deficits underwent surgical treatment on the side with LDH with or without decompressing the symptomatic side. A retrospective review of charts and radiological records of these 8 patients was performed. The putative mechanisms underlying the associated contralateral neurological defic

CREST Calcinosis Affecting the Lumbar and Cervical Spine and the Use of Minimally-Invasive Surgery.

Calcinosis in CREST (calcinosis, Raynaud's phenomenon, esophageal dysmotility, sclerodactyly, and telangiectasia) syndrome can affect the spinal and paraspinal areas. We present the first case to our knowledge where a CREST syndrome patient required surgery for spinal calcinosis in both the cervical and lumbar areas. A 66-year-old female with a history of CREST syndrome presented with right-sided lower extremity radicular pain. A computed tomography (CT) scan showed bilateral lumbar masses (5.8 cm on the right, 3.8 cm on the left) that projected into the foramina and into the spinal canal. The

Neurosurgical aspects of the treatment of neuroblastoma patients.

Neuroblastoma (NB) is the most common extracranial solid tumor in children. The neoplasm grows from progenitor cells of the sympathetic nervous system and can be detected anywhere along the sympathetic neurological circuit: retroperitoneally, mediastinally, cervically, and pelvically. Examination of children with suspected neuroblastoma is comprehensive and performed in strict compliance with a therapeutic protocol. A decision on the treatment regimen is made based on the tumor staging and the risk group of the patient. The diagnosis and treatment of NB patients are comprehensive and can be fu

The extreme lateral approach for treatment of thoracic and lumbar vertebral body metastases.

In addition to radiotherapy, the utility of surgical decompression and stabilization in patients with metastatic vertebral body tumors causing cord compression, progressive deformity and/or intractable pain has been well demonstrated. Minimally invasive approaches are an attractive alternative to traditional procedures as they may reduce the degree of disruption of normal anatomy, decrease blood loss, shorten hospital stays and reduce the risk of infection or wound dehiscence. The extreme lateral approach is a procedure that provides access to the anterior spine through a small incision along

Creation of a functional Potts shunt by stenting the persistent arterial duct in newborns and infants with suprasystemic pulmonary hypertension of various etiologies.

BACKGROUND: The surgical creation of a Potts shunt has been reported in children with suprasystemic idiopathic pulmonary artery (PA) hypertension (IPAH) refractory to any medical therapy. This surgical approach allows acute decompression of the right ventricle (RV) and thereby avoids ventricular failure. We present 4 newborns and infants with different causes of pulmonary hypertension (PH) in whom interventional stenting of the patent ductus arteriosus (PDA) was used to create a "functional" Potts shunt. METHODS: Suprasystemic PH was diagnosed by cardiac catheterization in 2 newborns with com

Management of lumbosacropelvic fracture-dislocation using lumbo-iliac internal fixation.

OBJECTIVE: Traumatic lumbosacropelvic fracture-dislocation is a rare but potentially serious injury. Conventional methods like lumbosacral fixation used to treat such injuries often result in suboptimal outcome secondary to complications like pseudoarthroses, sagittal imbalance and hardware failure. In this study, we retrospectively analysed the clinical features and management for this trauma using lumbo-iliac fixation. METHODS: Eight patients (6 male, 2 female; 21-52 years old, mean: 38.4) with traumatic lumbosacropelvic fracture-dislocation were surgically managed by lumbo-iliac internal f

Surgical outcomes of elderly patients with cervical spondylotic myelopathy.

BACKGROUND: Cervical spondylotic myelopathy is a potentially serious neurologic disorder that commonly presents with gait difficulty and hand dysfunction. Because the development of CSM is in large part related to advanced spondylosis and degenerative disk disease, elderly patients appear to be at an increased risk to develop this condition. The surgical outcomes of this patient population have been understudied; the authors seek to report their clinical results in a series of patients with CSM older than 75 years who underwent surgical treatment. METHODS: This report is composed of a cohort

[Direct and late outcome in trigeminal neuralgia treated by means of microvascular decompression in cerebellopontine angle].

BACKGROUND: Microvascular decompression (MVD) is a method of choice in the surgical treatment of trigeminal neuralgia (TN). OBJECTIVE: To evaluate direct and long-term results of MVD in TN. MATERIAL AND METHODS: The authors analyzed 20 patients (9 women and 11 men, mean age 62.2 years) with TN who had been treated surgically at the Department of Neurosurgery of the University Hospital in Cracow between October 1998 and September 2002. MVD was performed using Janetta technique via suboccipital retro-sigmoid craniectomy. All patients underwent additional neurolysis. Indentations in portio majo

Treatment of congenital spondyloptosis in an 18-month-old patient with a 10-year follow-up.

STUDY DESIGN: Case report. OBJECTIVES: To present the case of a patient with congenital spondyloptosis treated and followed over 10 years. SUMMARY OF BACKGROUND DATA: The surgical management of spondyloptosis in children is variably reported in the literature. Some authors propose that posterior fusion in situ is a safe and reliable procedure, whereas others suggest that reduction of the slipped vertebra may prevent some of the adverse sequelae of in situ fusion, which include nonunion, bending of the fusion mass, and persistent lumbosacral deformity. Many investigators advocate a combined a

Simultaneous somatosensory evoked potential and electromyographic recordings during lumbosacral decompression and instrumentation.

OBJECTIVE: Both motor and sensory neurological deficits have been reported after decompression and instrumentation of the lumbosacral spine. In this report, we describe a simple and effective method by which sensory and motor functions can be monitored simultaneously, using somatosensory evoked potentials (SSEP), spontaneous electromyographic (EMG) activity, and compound muscle action potential monitoring. The concomitant use of these monitoring techniques allows ongoing functional evaluation of the cauda equina and spinal cord during patient positioning, surgery, wound closure, and anesthetic

Thoracolumbar burst fractures treated with combined anterior and posterior surgery.

Between 1985 and 1992, 84 burst fractures were surgically managed with anterior decompression, autologous iliac crest strut graft, and posterior instrumentation and fusion. Fifteen were lost to follow-up, leaving 69 patients for detailed review. Of the 22 patients with complete or partial neurologic injury, 12 patients either totally or partially recovered function following surgery. Of these 12 patients, 6 improved 1 Frankel grade; 5 improved 2 Frankel grades, and 1 improved 3 Frankel grades. Follow-up was 12 to 91 months (average 41 months). For all of the fractures in this series, the mean

From Protrusion to Extrusion: Reassessing the Selective Role of Intradiscal Interventions.

CONTEXT: Extruded lumbar disc herniations are traditionally managed surgically, whereas contained protrusions are more commonly considered suitable for percutaneous intradiscal procedures. However, extrusion itself may not be a universal contraindication to all needle-based intradiscal interventions, particularly when the fragment is non-sequestered and remains continuous with the parent disc. This narrative review evaluates the evidence, safety considerations, and clinical positioning of intradiscal procedures in extruded, non-sequestered lumbar disc herniations. EVIDENCE ACQUISITION: A stru
