Intraductal papillary mucinous tumor of the pancreas: a pictorial essay.

Intraductal papillary mucinous tumor (IPMT) of the pancreas was identified and classified only recently. IPMT has a primarily intraductal, papillomatous growth pattern, which is associated with excessive mucin secretion and results in progressive ductal dilatation or cyst formation. The tumor occurs in four forms: segmental or diffuse involvement of the main pancreatic duct and macrocystic or microcystic involvement of a branch duct. In the past, many IPMTs may have been misdiagnosed as chronic pancreatitis because of their generally benign behavior. The correct diagnosis, once achieved only w

Cutaneous vascular anomalies. Part I. Hamartomas, malformations, and dilation of preexisting vessels.

Classification of cutaneous vascular anomalies is difficult because conceptual confusion persists between vascular neoplasms and malformations. However, hemangiomas of the infancy fulfill criteria both for hyperplasia and neoplasm because they result from proliferation of endothelial cells, but often undergo complete regression. Despite these pitfalls we have classified cutaneous vascular anomalies into the following categories: hamartomas, malformations, dilatations of preexisting vessels, hyperplasias, benign neoplasms, and malignant neoplasms. In this first part of our clinicopathologic rev

Comparison of transthoracic and transesophageal echocardiography with surgical findings in mitral regurgitation. The ESMIR Research Group.

This prospective study was conducted to ascertain whether echocardiographic evaluation could provide more insight into the genesis of mitral regurgitation (MR) before surgery. All patients underwent preoperative transthoracic and transesophageal echocardiography. Nine centers participated in the ESMIR (Echocardiographic Selection of patients for MItral valve Reconstruction) study and 350 patients were included. Compared with surgical findings, the percentage of functional abnormalities correctly predicted by both echo modalities was highest in patients with increased leaflet mobility (83% for

Late complications after surgical exclusion of the thoracic aorta.

Two patients who underwent surgical exclusion of the thoracic aorta for chronic dissecting aneurysms of the descending aorta died 4 and 8 years after surgery due to complications in part related to the operative technique. One patient developed pseudoaneurysm of the proximal aortic stump with fatal exsanguination through an aortapulmonary fistula. The other patient developed progressive dilatation of the remaining abdominal aorta with subsequent rupture. The surgical approach in this case was extremely difficult because of the previous exclusion procedure which determined a highly demanding re

Rapidly progressive enlargement of the fourth ventricle in the preterm infant with post-haemorrhagic ventricular dilatation.

Six preterm infants who developed disproportionate enlargement of the 4th ventricle during the neonatal period, associated with post-haemorrhagic ventricular dilatation (PHVD), before shunt placement are reported. Five of the six preterm infants developed cyanotic spells and/or bradycardias at the time of rapid enlargement of the 4th ventricle, suggestive of raised posterior fossa pressure, which resolved following insertion of and drainage from a subcutaneous reservoir. In one of the three survivors an isolated 4th ventricle was subsequently diagnosed later in infancy, requiring drainage. The

The natural history of aortic dilatation in Marfan syndrome.

OBJECTIVES: To determine the relationship between age and aortic dilatation in patients with Marfan syndrome and to define the rate of progression of aortic dilatation in these patients. DESIGN: All patients were evaluated in a multidisciplinary clinic to establish a firm diagnosis of Marfan syndrome. Aortic dimensions were measured by echocardiography and patients with Marfan syndrome were followed up with annual physical and echocardiographic examinations to detect any change in aortic diameter over the subsequent four years. PATIENTS: One hundred and fifty-seven patients were referred to

Congenital choledochal cyst with intrahepatic involvement.

We reviewed congenital choledochal cyst with intrahepatic involvement in 38 patients aged 52 days to 29 years. Dilatations were classified into the following three forms, based on the shape of the extrahepatic and intrahepatic bile duct: cystic-cystic, cystic-cylindrical, and cylindrical-cylindrical. Stenosis of the hepatic duct near the hilum probably does not cause dilatation of the intrahepatic duct. The anomalous arrangement of the pancreatobiliary union seems to determine only the form of extrahepatic dilatation, and cystic dilatation of the intrahepatic duct may be caused by some other f

Research progress on pathogenic genes of dilated cardiomyopathy.

Dilated cardiomyopathy (DCM) is a primary myocardial disorder characterized by progressive ventricular dilatation and impaired myocardial systolic contractility, and it represents the most common form of cardiomyopathy globally. DCM drives a substantial worldwide disease burden, thus presenting a formidable and persistent challenge to global public health systems. The pathogenesis of DCM is marked by extreme etiological heterogeneity: 30%-50% of cases have a familial origin, with genetic determinants serving as the core driver of disease onset and progression. With the rapid advancement and wi

Patient respiratory-triggered quantitative T2 mapping in the pancreas.

BACKGROUND: Long acquisition times and motion sensitivity limit T2 mapping in the abdomen. Accelerated mapping at 3 T may allow for quantitative assessment of diffuse pancreatic disease in patients during free-breathing. PURPOSE: To test the feasibility of respiratory-triggered quantitative T2 analysis in the pancreas and correlate T2 -values with age, body mass index, pancreatic location, main pancreatic duct dilatation, and underlying pathology. STUDY TYPE: Retrospective single-center pilot study. POPULATION: Eighty-eight adults. FIELD STRENGTH/SEQUENCE: Ten-fold accelerated multiecho-sp

ATAD3A Limits Aortic Dissection via Mito-Lysosome Contacts and Lipoylation.

BACKGROUND: Aortic aneurysm and dissection (AAD) is a fatal vascular emergency with limited mechanism-based therapies. ATAD3A (ATPase family AAA [ATPases associated with diverse cellular activities] domain-containing protein 3A), a mitochondrial AAA+ ATPase enriched at organelle contact sites, has been implicated in mitochondrial signaling, but its role in AAD remains unclear. METHODS: AAD was induced in wild-type, ATAD3A knock-in, and vascular smooth muscle cell (VSMC)-specific knockdown mice generated using short hairpin RNA against ATAD3A (sh-ATAD3A) by 3-week β-aminopropionitrile monofuma

Cohort-level clinical trajectory and molecular landscape of idiopathic subglottic stenosis for precision laryngology-a study of the Canadian Airways Research (CARE) group.

BACKGROUND: First described in 1972, idiopathic subglottic stenosis (iSGS) is a serious chronic orphan disease characterised by recurrent scarring of the subglottis. Although the cause is unknown, iSGS is almost exclusively restricted to Caucasian females typically in their fourth to sixth decade. However, given its rare incidence (1:400,000), understanding the clinical trajectory and molecular factors associated with iSGS disease development and prognosis has been difficult. In the current study we sought to unravel the pathogenesis of iSGS at the clinical, transcriptional, and genetic level

Histology and Glutamine Synthetase Immunoreactivity in Liver Biopsies From Patients With Congestive Heart Failure.

BACKGROUND: Long-standing congestive heart failure can induce a constellation of histopathology changes in the liver that can range from mild sinusoidal dilation to advanced fibrosis and loss of normal perivenular expression of glutamine synthetase (GS). Liver biopsies might be performed to assess the perioperative risk of these patients or to determine the need of synchronous liver transplant. We aimed to assess interobserver agreement in recognizing these liver histologic features in patients undergoing evaluation for heart transplantation and to examine whether immunohistochemistry of GS wi

Distal Cholangiocarcinoma Diagnosed via Percutaneous SpyGlass Cholangioscopy After Failed Endoscopic Retrograde Cholangiopancreatography (ERCP) in a Patient With Suspected Gallstone Pancreatitis.

Cholangiocarcinoma is a rare and aggressive malignancy of the biliary epithelium that frequently presents with nonspecific symptoms and is often diagnosed at an advanced stage. Diagnosis may be delayed when coexisting hepatobiliary conditions obscure the underlying pathology, and conventional imaging or endoscopic retrograde cholangiopancreatography (ERCP) may fail to identify distal bile duct lesions, particularly when biliary access cannot be achieved. We report the case of an 81-year-old man who presented with findings initially suggestive of gallstone pancreatitis but with evolving feature

Bladder stone as the first clue to a hidden vesicovaginal fistula: diagnostic and surgical insights from a multidisciplinary approach-a case report.

BACKGROUND: Women presenting with urinary tract symptoms such as bladder stones are often presumed to have primary urological disease. However, urogynecologic conditions-including vesicovaginal fistula (VVF) and retained vaginal foreign bodies-can rarely manifest with predominant urological symptoms, leading to diagnostic delay. CASE DESCRIPTION: A 20-year-old female presented with persistent lower urinary tract discomfort and urinary leakage for 2 years. She denied abdominal pain, distension, or menstrual irregularities. No psychiatric or surgical history was reported, and renal function was

Clinical and imaging phenotypes.

Functional/Secondary tricuspid regurgitation (STR) accounts for over 85% of clinically significant tricuspid regurgitation (TR) and is associated with adverse prognosis and impaired quality of life. Advances in percutaneous tricuspid valve (TV) interventions underscore the need to differentiate TR aetiologies, mechanisms, and phenotypes. STR is subdivided into atrial (A-STR), caused by right atrial dilation and tricuspid annular enlargement without significant leaflet tethering, and ventricular (V-STR), resulting from right ventricular dilation/dysfunction with leaflet tethering. Ascending aortic aneurysm and dissection risk: focus on aortic height index.

OBJECTIVES: Acute aortic dissection (AAD) commonly occurs with a dilation of the ascending aorta at diameters under the threshold of surgical indication. Aortic diameter/height index (AHI) has been proposed for risk stratification and more accurate prophylactic surgery. METHODS: From January 2001 to November 2023, all patients operated on for AAD at our Institute were prospectively collected and retrospectively analysed, calculating the AHI. A Control Group without aortic pathology was stratified for the same risk index, and a modelling risk analysis for aortic dissection was also performed.

ASNA1 is essential for cardiac development and function by regulating tail-anchored protein stability and vesicular transport in cardiomyocytes.

Recent studies have linked compound heterozygous mutations in ASNA1 to progressive dilated cardiomyopathy and early infantile mortality in humans. However, the specific role of ASNA1 in cardiomyocytes and the molecular mechanisms underlying ASNA1-related cardiomyopathy remain poorly understood. Tail-anchored (TA) proteins, characterized by a single C-terminal transmembrane domain (TMD), require post-translational targeting to intracellular membranes, a process primarily mediated by the evolutionarily conserved Guided Entry of Tail-anchored proteins (GET) pathway in yeast and the Transmembrane
