Meso-diverticular band causing small bowel obstruction in an infant: a case report and literature review.

INTRODUCTION: Meckel's diverticulum (MD) is the embryonic remnant of the omphalomesenteric duct that occurs in approximately 2% of the population. Although usually asymptomatic, it can, in some cases, cause complications in the form of ulceration, hemorrhage, intussusception, intestinal obstruction, and perforation. CASE PRESENTATION: A 3-month-old male infant presented with a 4-day history of crampy abdominal pain, bilious vomiting, abdominal distension, constipation, and poor feeding. On admission, he appeared acutely ill with signs of shock. After nasogastric decompression, fluid resuscita…

Clinical Classification of Urethrocutaneous Fistulas Developing after Hypospadias Repair.

Background  Clinical classification of the urethrocutaneous fistulas (UCFs) was designed to help the surgeons in (1) categorizing the fistul … ocedure. Category E fistulas had associated hairy urethra, stricture distal urethra, stricture with diverticulum, perifistular scar-induced chordee, long narrow urethral plate, balanitis xerotica obliterans (BXO), and short reconstructed neourethra. Accordingly, appropriate corrective measures were taken. Miscellaneous category F was excluded from the study. R …

An Unusual Case of Meckel Diverticulitis Misdiagnosed as an Infected Urachal Cyst.

Introduction: Meckel's diverticulum (MD), a remnant of the omphaloenteric duct, is among the most frequent intestinal malformations. Another embryonic vestige is the urachus, which obliterates, becoming the median umbilical ligament; the failure of this process can lead to a urachal cyst formation. We present a case of Meckel diverticulitis misdiagnosed as an infected urachal cyst. Presentation of case: A 16-year-old girl presented with hypogastric pain, fever and vomiting. She had undergone an appendectomy 6 years prior and no digestive malformation had been documented. In the last 2 years, s…

Clear cell adenocarcinoma of the urethra: a clinicopathologic analysis of 19 cases.

Nineteen clear cell adenocarcinomas of the urethra, 18 from women and one from a man, are described. The patients ranged from 35 to 80 years of age (average, 58 yrs). The clinical presentation and gross findings were similar to those of urethral carcinomas, except that 12 tumors, all from women, arose within a urethral diverticulum. On microscopic examination, the neoplasms exhibited the classic triad of tubulocystic, papillary, and diffuse patterns that characterize this tumor. The tumors had the typical cytologic features of clear cell adenocarcinoma, including hobnail cells, flattened cells…

Pancreatic heterotopia: a reappraisal and clinicopathologic analysis of 32 cases.

A total of 32 histologically documented cases of heterotopic pancreas was found in a review of the records of the department of pathology at the Chang Gung Memorial Hospital between 1977 and 1987. This review was done to ascertain the clinical significance of this uncommon entity. In 14 patients (44%), the aberrant pancreatic tissue was symptomatic; in the other 18 (56%), it was found incidentally. In the symptomatic group, the heterotopic pancreatic tissue was found in a duplication cyst of the ileum in one patient, in the common bile duct in one, in a Meckel's diverticulum in four, in the st…

Unclassified complex urogenital anomaly in a 3-year-old girl: Diagnostic and surgical challenges.

Complex urogenital malformations are rare congenital anomalies that often escape existing classification systems due to marked anatomical variability. We describe a previously unreported urogenital malformation. A 3-year-old girl presented with ambiguous genitalia, a single perineal urogenital orifice, bilateral duplicated renal systems with complete vesicoureteral reflux, and vaginal agenesis. Endoscopy revealed a bladder dome diverticulum containing ureteral and cervical orifices. She underwent bilateral ureteral reimplantation and creation of an ileal neovagina with utero-neovaginal anastom…

Jejunal diverticulitis with misleading biochemical improvement: a diagnostic challenge.

Jejunal diverticulitis is a rare and often under-recognized cause of acute abdominal pain, frequently presenting with non-specific clinical, biochemical, and radiological findings. A 52-year-old female presented with acute abdominal pain and elevated inflammatory markers, but non-diagnostic imaging. Despite biochemical improvement with conservative management, her symptoms persisted. Diagnostic laparoscopy revealed jejunal diverticulitis with an incidental Meckel's diverticulum requiring dual segmental small bowel resection. The postoperative course was uneventful. This case highlights the lim…

Urinary bladder duplication in an adult male with associated genitourinary and spinal malformations: Multimodal imaging and surgical confirmation in a complex urogenital case.

Bladder duplication is a rare congenital anomaly, seldom reported in adults. We describe a young male presenting with recurrent urinary symptoms and lower abdominal pain. Imaging (ultrasound, CT urogram, MRI) revealed a complete sagittal septum dividing the bladder into 2 cavities with cystolithiasis, along with a hypoplastic left kidney, vesicoureteral junction (VUJ) diverticulum, partial sacral hypoplasia, and hypoplastic prostate and seminal vesicles. The patient underwent septum resection and stone removal, leading to symptom resolution. This case underscores the importance of considering …

A Rare Cause of Small Bowel Intussusception: Non-Meckel Diverticula in an Adult Patient.

Intussusception involves telescoping of a proximal bowel segment into a distal segment. Non-Meckel small bowel diverticula are rare and typically asymptomatic. We report a 37-year-old man with Crohn's disease presenting with persistent abdominal pain despite normal colonoscopies and inflammatory markers. Magnetic resonance enterography and capsule endoscopy revealed concentric small bowel thickening; double-balloon enteroscopy identified a large diverticulum in the ileum. Surgical resection was performed, with pathology revealing non-Meckel diverticula, and symptom resolution postoperatively. …

Aberrant Left Subclavian Artery-Esophageal Fistula in a Patient with a Prolonged Use of Nasogastric Tube: A Case Report and Literature Review.

Arterial-esophageal fistula is a rare but potentially fatal complication. Right aortic arch with aberrant left subclavian artery is a rare congenital vascular anomaly that can cause esophageal compression, particularly when the proximal portion of the aberrant subclavian artery forms a Kommerell's diverticulum. Prolonged use of a nasogastric tube can cause pressure necrosis of the esophagus. We report a patient with massive gastrointestinal bleeding secondary to aberrant left subclavian artery-esophageal fistula after a prolonged use of nasogastric tube. A high index of suspicion is essential …

Congenital left ventricular aneurysm with myocardial noncompaction pattern.

Congenital left ventricular aneurysm or diverticulum is very rare. Most congenital left ventricular aneurysms are asymptomatic but some cause systemic embolization, left ventricular free wall rupture, or ventricular arrhythmias causing sudden cardiac death. A 29-year-old woman with congenital left ventricular aneurysm and nonsustained ventricular tachycardia underwent surgical plication of the aneurysm with left ventricular reshaping to improve heart failure symptoms, prevent rupture of the aneurysm wall and the possibility of thrombus formation, and excise the fibrotic tissue of the left vent…

Enterobius vermicularis infestation leading to Meckel's diverticulitis in an adolescent boy: An extremely rare presentation.

Enterobius vermicularis is an intestinal nematode commonly affecting children worldwide. Its transmission is by feco-oral route. Meckel's diverticulitis due to E. vermicularis infestation is an extremely rare presentation. An 11-year-old boy presented with acute abdomen. During surgery inflamed Meckel's diverticulum (M.D) was seen. Histopathology examination of specimen revealed E. vermicularis. Till date, only one case of E. vermicularis infestation of M.D is reported around five decades ago. This histopathological confirmation is extremely important as the required treatment (Mebendazole) of…

Video-assisted thoracoscopic division of vascular rings.

This study reports our early experience with thoracoscopic division of vascular rings. Three patients were reviewed; their ages at surgery were 25 months, 4 years, and 57 years. All patients were suffering from complete vascular rings involving combinations of the right aortic arch, left ligamentum arteriosum, Kommerell's diverticulum, and retroesophageal left subclavian artery. The median surgical time was 180.5 minutes, and the patients showed immediate recovery. Three complications, namely chylothorax, transient supraventricular tachycardia, and left vocal cord palsy, were observed. Our ear…

Unusual case of emphysematous cystitis seen on FDG PET/CT.

Emphysematous cystitis is a rare condition requiring immediate aggressive medical attention. Here, we describe a very unusual case of emphysematous cystitis seen on FDG PET/CT. This finding was discovered in an 84-year-old man undergoing FDG PET/CT to monitor a known diagnosis of IgG4-related nodular sclerosis. Images revealed gas within the bladder wall and an anterior gas-filled bladder diverticulum. The patient subsequently developed sepsis with cultures revealing Escherichia coli. This case highlights the importance of recognizing this finding on varied imaging modalities, as life-threaten…

Urachal carcinoma: a report of two cases.

Urachal carcinoma is a rare neoplasm of the urinary bladder. We present two cases. Case 1, a male patient, noted urinary frequency and micturition pain for two months. The surgical specimen from a partial cystectomy represented a transitional cell carcinoma that developed from a urachal diverticulum. Case 2, a female patient, had a long history of infraumbilical mass without umbilical discharge or urinary symptom. Pathologically, the mass showed a papillary adenocarcinoma arising from a urachal cyst. Both patients received successive en bloc operation. Local recurrence and peritoneal carcinoma…

Urachal abnormalities in adults: the Ochsner experience.

Urachal abnormalities are very rarely encountered in adults. Though the urachus is normally obliterated in early infancy, it may cause a lower midline lesion in adults. Urachal abnormalities can be classified into five groups: patient urachus, urachal sinus, vesicourachal diverticulum, urachal cyst, and alternating sinus. The medical records of Ochsner Medical Institutions were reviewed for the 31-year period 1957 to 1988. Forty-one patients were identified as having anomalies of the urachus, 11 of them in the adult age group (older than 17 years). All cases were documented by pathologic exami…
