Using Evidence-Based Care Bundles to Reduce Central Line-Associated Infections in High-Risk Settings.

AIM: To improve the rates of central line-associated bloodstream infections in an intensive care and non-intensive care setting in a large Australian health service. DESIGN: Evidence-based care bundles targeting central line practices were developed and implemented using a pragmatic action research-approach. METHODS: Wards recruited change facilitators to implement the bundles. Compliance and ward engagement were measured, and pre-intervention and intervention central line-related infection rates were compared. RESULTS: Wards showed a reduction in central line-related infections by 90% duri

BACKGROUND AND AIM: Face masks are routinely worn by healthcare personnel as an infection prevention measure during surgical procedures in hospitals. Their use is more variable in general practice and emergency clinics. We assessed the scientific evidence underpinning the use of face masks during invasive surgical procedures to prevent postoperative infections. EVIDENCE BASE: We performed a systematic literature search using predefined criteria in the Medline, Embase and Cochrane Library databases. The intervention was the use or non-use of face masks during invasive surgical procedures. The

Decolonization vs Enhanced Cleaning for Multidrug-Resistant Organism Reduction in Nursing Homes.

IMPORTANCE: Universal decolonization and enhanced cleaning are strategies to prevent multidrug-resistant organism (MDRO) transmission. Whether their associations with MDRO burden are overlapping, additive, or synergistic remains unknown. OBJECTIVE: To evaluate independent and combined associations of decolonization and enhanced cleaning with MDRO carriage and environmental contamination. DESIGN, SETTING, AND PARTICIPANTS: This quality improvement study was conducted in 2 Southern California nursing homes from March 2019 to April 2021. This 4-phase study implemented (1) universal decolonizati

Electronic surveillance system shortens time-to-action and improves adherence to the isolation bundle for patients with Clostridioides difficile infection: an interrupted time series at a French university hospital.

BACKGROUND: Electronic surveillance systems may assist infection prevention and control (IPC) teams in detecting and monitoring patients infected with pathogens such as Clostridioides difficile. We aimed to estimate the impact of an electronic surveillance system on the audit rate conducted by the IPC team, response timeliness, and compliance with the C. difficile isolation bundle. METHODS: A quasi-experimental before-after study was conducted using an interrupted time series analysis from 2018 to 2025 at the University Hospital of Nancy (France). The electronic surveillance system ZINC™ was

Fostering an interdisciplinary culture of collaborative discussion to prevent central line-associated bloodstream infections.

BACKGROUND: Few studies describe quantitative thresholds for daily device review that attempt to drive a decrease in device utilization (DU) for central venous catheter devices. Increased DU in the period spanning March 2020 through December 2021 paralleled an increased incidence of central line-associated bloodstream infections (CLABSI) within a Medical Surgical Intensive Care Unit (ICU) of an acute care hospital. An intervention tool was developed to foster targeted discussions around DU to ultimately reduce the risk of CLABSI. METHODS: An evidence-based performance improvement initiative w

Etiology of Bacterial Sepsis and Isolate Resistance Patterns in Hospitalized Neonates in Zambia.

BACKGROUND: The Sepsis Prevention in Neonates in Zambia study is a prospective cohort study that evaluated an infection prevention and control (IPC) bundle in the University Teaching Hospital neonatal intensive care unit (NICU) in Lusaka, Zambia. We present here the etiologies, antimicrobial resistance profiles, and associated mortality of bloodstream infections (BSI) in this cohort. METHODS: Venous blood was collected from neonates with clinically suspected sepsis and cultured with an automated blood culture system. Organism identification and susceptibility testing were done using the Vitek

Nosocomial outbreak caused by Serratia marcescens in a neonatology intensive care unit in a regional hospital. Analysis and improvement proposals.

OBJECTIVES: Serratia marcescens (SM) may cause nosocomial outbreaks in Neonatal Intensive Care Units (NICU). We describe an outbreak of SM in a NICU and propose additional prevention and control recommendations. METHODS: Between March 2019 and January 2020, samples were taken from patients in the NICU (rectal, pharyngeal, axillary and other locations) and from 15 taps and their sinks. Control measures were implemented including thorough cleaning of incubators, health education to staff and neonates'relatives, and use of single-dose containers. PFGE was performed in 19 isolates from patients a

Outbreak of Klebsiella pneumoniae carbapenemase-producing K pneumoniae: A systematic review.

BACKGROUND: First detected in the United States in 1996, Klebsiella pneumoniae carbapenemase (KPC) has spread internationally among gram-negative bacteria, especially K pneumoniae. These microorganisms can cause serious infections in hospitalized patients, and there are few therapeutic options, culminating in increased mortality. The objective of this study was to describe the occurrence of outbreaks that were caused by KPC-producing K pneumoniae, emphasizing the interventions that were implemented to contain the outbreaks. METHODS: PubMed, Web of Knowledge, and Literatura Latino Americana em

Impact of a multimodal intervention to reduce bloodstream infections related to vascular catheters in non-ICU wards: a multicentre study.

To determine the impact of a multimodal intervention designed to reduce the incidence of catheter-related bloodstream infections (CRBSIs) outside the ICU, we conducted a prospective, quasi-experimental, before-after intervention study in 11 hospitals participating in the VINCat programme in Catalonia, Spain. The intervention consists of: (i) an evidence-based bundle of practices relating to catheter insertion and maintenance; (ii) a training programme for healthcare workers; (iii) four point-prevalence surveys to track the status of the catheters; and (iv) feedback reports to the staff involve

Characterizing the in vitro and in vivo effect of bicarbonate on azithromycin activity against Acinetobacter baumannii.

Carbapenem-resistant Acinetobacter baumannii (CRAB), a Gram-negative bacterial pathogen, has been identified by Centers for Disease Control (CDC) as the top priority pathogen for which new antibiotics are needed. We found that the addition of bicarbonate at physiologically normal levels found in the blood (23.8 mM) increased susceptibility of A. baumannii clinical isolates (n = 63) to azithromycin and resulted in MIC50 shift from 64 mg/L in CAMHB to 1-2 mg/L in CAMHB + bicarbonate (23.8 mM) or RPMI-1640 respectively. To characterize in vivo efficacy in murine blood and lung infection models, m

Protective immunity against secondary poxvirus infection is dependent on antibody but not on CD4 or CD8 T-cell function.

Renewed interest in smallpox and the need for safer vaccines have highlighted our lack of understanding of the requirements for protective immunity. Since smallpox has been eradicated, surrogate animal models of closely related orthopoxviruses, such as ectromelia virus, have been used to establish critical roles for CD8 T cells in the control of primary infection. To study the requirements for protection against secondary infection, we have used a prime-challenge regime, in which avirulent ectromelia virus was used to prime mice that were then challenged with virulent ectromelia virus. In cont
