Predicting the psychosocial outcome of epilepsy surgery: A longitudinal perspective on the 'burden of normality'.

Good seizure outcomes and good psychosocial outcomes following epilepsy surgery do not necessarily follow one from the other. This study explored the relationship between several presurgical psychosocial characteristics and postsurgical quality-of-life outcomes. The study aimed to develop the concept of 'the burden of normality' and identify risk factors for a poor psychosocial outcome that could be targeted with ameliorative presurgery cognitive behavioral techniques. Data were collected from 77 epilepsy surgery patients from three UK epilepsy centers and presurgery and postsurgery follow-up

Osteoarthritis year in review 2014: clinical.

A systematic search was conducted for the time period April 1 2013 to March 30 2014 using PubMed to identify major osteoarthritis (OA) clinical research themes of the past year. Articles within each theme were selected for inclusion in this review based on study quality and relevance. Two major themes emerged, which relate to the current understanding of OA as a heterogeneous condition with multiple pathogenic mechanisms and clinical manifestations. Theme 1 stems from the role of systemic inflammation in OA pathogenesis, and the concept of 'metabolic OA'. Over the past year, research has exami

The heart of the art: emotional intelligence in nurse education.

The concept of emotional intelligence has grown in popularity over the last two decades, generating interest both at a social and a professional level. Concurrent developments in nursing relate to the recognition of the impact of self-awareness and reflexive practice on the quality of the patient experience and the drive toward evidence-based patient centred models of care. The move of nurse training into higher education heralded many changes and indeed challenges for the profession as a whole. Traditionally, nurse education has been viewed as an essentialist education, the main emphasis bein

The role of biofilms in otolaryngologic infections.

PURPOSE OF REVIEW: Bacterial biofilms have recently been shown to be important in diseases of the head and neck. Because the concept of biofilms is novel to most practitioners, it is important to gain a basic understanding of biofilms and to recognize that strategies developed to treat planktonic bacteria are ineffective against bacteria in a biofilm. RECENT FINDINGS: Bacteria preferentially exist in complex, surface-attached organizations known as biofilms. Bacteria in biofilms express a different set of genes than their planktonic counterparts and have markedly different phenotypes. Biofilm

Position of the American Dietetic Association: optimal weight as a health promotion strategy.

The American Dietetic Association supports the concept of optimal weight that considers a variety of factors to determine the most favorable weight for the individual. Adequate research exists to support the following: The prevention of obesity in children and adults through nutrition and health education should be a primary objective for registered dietitians and other health care professionals. Optimal weight for individuals should be determined by considering health risks, heredity, age, sex, percentage body fat, and realistic goal-setting. Children should not be placed on restricted-calori

Difficult-To-Treat Depression. Scoping Review.

ObjectiveRecently, several academics have recommended that the concept of difficult-to-treat depression (DTD) should be considered in some of the cases where achieving or maintaining remission of depressive symptoms is not possible. In 2020, a consensus statement, not based on a formal process and systematic review defined difficult-to-treat depression as "depression that continues to cause significant burden despite normal treatment efforts". In addition to addressing symptom control, interventions for DTD should also target other factors, including the management of psychiatric and medical c

Self-reporting quality of life in mild-to-moderate Alzheimer's disease and Lewy body dementia: Comparing capability and health-focused measures using response process validation.

BACKGROUND: Capability measures offer theoretical advantages for assessing wellbeing in health economics and outcomes research, yet their feasibility for self-reporting by people living with dementia remains to be established. This exploratory study compares the feasibility of self-reporting on capability versus health-focused quality of life measures among people living with mild-tomoderate dementia, using response process validation. METHODS: Twenty-three community-dwelling participants living with Alzheimer's disease (n = 11) or Lewy body dementia (n = 12) completed four measures using con

The therapeutic mechanism-trajectories principle (TMTP): A framework for informational architecture in chronic disease self-management.

OBJECTIVES: Self-management interventions in chronic disease are frequently communicated as general wellness advice. Horne's Necessity-Concerns Framework showed adherence tracks perceived necessity (OR = 1.742) and ameliorated concerns (OR = 0.504); Miller found health literacy correlates more strongly with adherence for non-medication regimens (r = 0.20) than medication regimens (r = 0.09). Neither addresses the authority signals prescriptions carry and self-management advice lacks. METHODS: Discussion paper grounded in conceptual analysis, integrating Horne's NCF, Miller's health literacy w

Internalized Disability Stigma Among People Living With Chronic Traumatic Brain Injury.

OBJECTIVE: Qualitative evidence suggests that disability stigma is an important concept in the lived experiences of individuals with traumatic brain injury (TBI). However, there have been no studies in this population specifically evaluating internalized disability stigma. We sought to characterize variability in levels of internalized stigma and examine the applicability of the "why try" effect (reduced self-efficacy and life goal attainment as consequences of internalized stigma) to people living with chronic TBI. SETTING: Outpatient research laboratory. PARTICIPANTS: Eighty-four adults wi

Crossing horizons: unraveling perspectives on enhancing medical students' success through at-risk factor exploration.

BACKGROUND: Medical universities often face the ongoing challenge of identifying and supporting at-risk students to enhance retention rates and academic success. This study explores a comprehensive analysis of perceived at-risk factors impeding academic and career aspirations and compares the perspectives of students and faculty in a medical school. METHODS: We focused on first and second-year medical (MBBS) students and teaching faculty in an international medical college offering a twinning program in India and Malaysia. Our investigation involved a comprehensive assessment of 25 at-risk fa

Online group-based internal family systems treatment for posttraumatic stress disorder: Feasibility and acceptability of the program for alleviating and resolving trauma and stress.

OBJECTIVE: Demand for trauma-focused therapy continues to increase, especially in community mental health care settings where group treatment models can be cost-effective and increase access to care. The Internal Family Systems (IFS) model for posttraumatic stress disorder (PTSD) may offer an effective therapeutic approach. The purpose of this proof-of-concept study was to evaluate the feasibility and acceptability of a novel, trauma-focused, group-based treatment approach and investigate potential mechanisms of action. METHOD: Study participants completed the Program for Alleviating and Reso

"It's not just the hit itself": the social practice of injecting drug use among gay and bisexual men in Australia.

BACKGROUND: Injecting drug use is purportedly more common among gay and bisexual men (GBM) than the general Australian population. Approaches designed to support the wellbeing of people who inject drugs may not be effective for GBM who inject, due to divergent settings, substances, and/or symbolism. We sought to identify the critical elements shaping injecting among GBM as a social practice and the implications for health and psychosocial wellbeing. METHODS: We conducted 19 in-depth interviews with GBM in Australia with lifetime experience of injecting drug use, adopting the Frameworks Method

Implementation of self-directed learning within clinical clerkships.

Background: The main aim of medical curricula is to prepare students for the first day at the work place. While teaching clinical competence is pivotal, clinical clerkships are often the last chance to close knowledge gaps with the help of clinical teachers. Self-directed learning is a dynamic field for research within medical education, though its curricular implementation is rare. This study focuses on the needs assessment of clinical clerkships using the concept of self-directed learning. Methods: The study comprised an educational experience at the Ludwig-Maximilians Universität (LMU) Muni

Testing an equifinality model of nonsuicidal self-injury among early adolescent girls.

Nonsuicidal self-injury (NSSI) is a common behavior among females that has been shown to confer risk for continued self-injury and suicidal attempts. NSSI can be viewed conceptually as behavior that is pathognomonic with aggression and/or depression. Empirical research on concurrent correlates supports this concept: numerous and diverse factors are shown to be significantly associated with self-harm, including depression, emotion dysregulation, impulsivity, and aggression and other conduct problems, as well as environmental stressors such as bullying, harsh parenting, and negative life events.

Self-reported chemical sensitivity in Germany: a population-based survey.

OBJECTIVES: Environmental clinics are frequented by patients with fears and complaints related to environmental triggers. A dose-independent overreaction to small doses of widely used and generally non-toxic chemicals is referred to as multiple chemical sensitivity (MCS), but no clearly defined clinical syndrome with objective physical findings has been delineated so far. We aimed to obtain information about symptoms, supposed environmental triggers, the frequency of self-reported chemical sensitivity, and of the diagnosis MCS in Germany. METHODS: We conducted a representative survey among 20

Development of a health behavior scale for older adults living alone receiving public assistance.

BACKGROUND: To reduce health disparities, prevention of non-communicable diseases (NCD) by performing desirable health behavior in older adults living alone with low socioeconomic status is an essential strategy in public health. Self-perception of personal power and practical skills for daily health are key elements of desirable health behavior. However, methods for measuring these concepts have not been established. This study aimed to develop a health behavior scale for older adults living alone receiving public assistance (HBSO). METHODS: The self-administered mail survey covered 2818 old

Gel filtration chromatographic study on the self-association of surfactants and related compounds.

After a brief survey of gel filtration chromatography (GFC) and the self-association equilibrium, it is shown that GFC is a unique tool for investigating the self-association of many substances, such as surfactants, chlorpromazine hydrochloride (CPZ), Methylene Blue (MB) and a sulfobetaine derivative (CHAPS) of cholic acid. The hydrodynamic radius of an aggregate can be estimated with the gel of large pore size, such as Sephadex G-200. This method is utilized for the study of the sphere-rod transition of surfactant micelles. A precise value of monomer concentration can be determined from the c
