The feasibility and results of an arthroscopic removal of humeral locking plates and glenohumeral arthrolysis after proximal humeral fractures.

PURPOSE: This study describes the use of standard shoulder arthroscopy techniques to remove a proximal humerus locking plate following proximal humerus fracture. The goal of this study was to assess the feasibility and results of this technique. METHODS: This was a retrospective non-comparative study. Inclusion criteria were fracture union when hardware was removed, significant residual glenohumeral stiffness after 6 months of physiotherapy, arthrogenic screw(s) and/or osteonecrosis (partial or complete) of the humeral head resulting in significant pain. RESULTS: Eleven patients were include

Fewer anchors achieves successful arthroscopic shoulder stabilization surgery: 114 patients with 4 years of follow-up.

BACKGROUND: The shoulder is the most frequently dislocated joint, with an incidence of 10 to 20 per 100,000 each year. The optimum number of anchors to use in arthroscopic stabilization is a topic of growing interest; most surgeons use 3. Our stabilization technique is to commonly use only a single suture anchor to purse-string the capsulolabral tissue up and toward the glenoid. This study aimed to ascertain whether successful stabilization can be achieved with fewer than 3 anchors. METHODS: Our study comprised 114 consecutive patients with anterior instability and a Bankart lesion undergoing

An evaluation of the responsiveness and discriminant validity of shoulder questionnaires among patients receiving surgical correction of shoulder instability.

Health-related quality-of-life (HRQL) measures must detect clinically important changes over time and between different patient subgroups. Forty-three patients (32 M, 13 F; mean age = 26.00 ± 8.19 years) undergoing arthroscopic Bankart repair completed three validated shoulder questionnaires (Western Ontario Shoulder Instability index (WOSI), American Shoulder and Elbow Surgeons Standardized Shoulder Assessment form (ASES), Constant score) preoperatively, and at 6, 12, and 24 months postoperatively. Responsiveness and discriminant validity was assessed between those with a satisfactory outcome

Surgical treatment of traumatic anterior shoulder instability in american football players.

BACKGROUND: American football players have been reported to be at high risk for postoperative instability after arthroscopic stabilization of anterior shoulder instability. While some authors have recommended open methods of stabilization in athletes who play contact sports, there are few data in the literature showing more favorable results with use of an open technique. We reviewed the results of an open technique of anterior shoulder stabilization in fifty-eight American football players after a minimum of two years of follow-up. METHODS: Fifty-eight American football players underwent ope

Ultrasonographic Assessment of Posterior Shoulder Capsule Thickness in Baseball Pitchers: A Validation Study.

BACKGROUND: A thickened posterior shoulder capsule may increase injury risk in baseball pitchers due to maladaptive shoulder biomechanics such as altered arthrokinematics, decreased internal rotation range of motion (ROM) and increased scapular upward rotation. While diagnostic ultrasound offers a reliable method of measuring posterior capsule thickness (PCT), it is currently unclear whether PCT thus measured is valid. HYPOTHESIS: Diagnostic ultrasound is a valid method for measuring PCT compared with magnetic resonance imaging (MRI). STUDY DESIGN: Cross-sectional. LEVEL OF EVIDENCE: Level

A multicentre, randomized, pragmatic, parallel group, non-inferiority trial to compare the clinical and cost-effectiveness of sling immobilization versus surgery in the management of adults with a displaced fracture of the distal clavicle : protocol for the DIDACT randomized controlled trial.

AIMS: Fractures of the clavicle primarily occur in young males and constitute 2.6% to 5% of all fractures in adults. Distal clavicle fractures, where the outer end of the collarbone breaks, account for 20% to 25% of all clavicle fractures. These fractures can be called displaced if the ligaments connecting the collarbone to the shoulder blade (coracoclavicular complex) rupture. Such displaced fractures (Neer's type II and V) are currently treated with an operation involving fracture fixation or with sling immobilization. This protocol describes a randomized controlled trial that aims to evalua

Clavicle lengthening, a novel operation technique to reduce pain in brachial plexus birth injury patients.

BACKGROUND: Brachial plexus birth injury (BPBI) can lead to an imbalance of shoulder musculature that can lead to glenohumeral contractures, and joint and osseous deformities. Glenoid hypoplasia, lengthening of coracoid and acromion, protraction, lateral rotation and elevation of the scapula, and shortening of the clavicle can be observed. As a consequence, the trapezius, levator scapulae, rhomboid, and supraspinatus muscles are overloaded in daily activities causing pain, which can be difficult to treat conservatively. It is hypothesized that operative lengthening of the clavicle may lead to

Effects of Motor Imagery on Movement-Based Fear in Musculoskeletal Conditions: A Critically Appraised Topic.

CLINICAL SCENARIO: The presence of movement-based fear during rehabilitation negatively impacts injury recovery, quality of performance, and perceived disability. While current evidence supports using graded motor imagery (GMI) to reduce movement-based fear in patients with complex regional pain syndrome and chronic neck pain, the effects of GMI on reducing movement-based fear in patients undergoing physical therapy (PT) for an upper- or lower-extremity musculoskeletal injury are unclear. CLINICAL QUESTION: Does incorporating GMI with traditional PT rehabilitation result in decreased movement

Diagnosis of Bilateral Calcifications of Temporomandibular Joint Disc by Image Fusion.

The calcification of the articular disc is an uncommon lesion, usually discovered in hips, elbows, and shoulders, but rarely in temporomandibular joints (TMJ). The TMJ disc calcification may be related to pain and limitation of the mandibular mobility, however, most of the patients were asymptomatic. A 61-year-old female was referred to our hospital after a maxillofacial fist injury, bilateral TMJ disc calcifications were found accidentally by radiological examination. Here the significance of image fusion of cone-beam computed tomography and magnetic resonance imaging (MRI) in the diagnosis o

[Upper extremity deep vein thrombosis following soft tissue trauma].

A 27-year-old man sustained soft tissue injury to the left shoulder following a fall. Three days later, a diagnosis of left proximal subclavian vein thrombosis was made. The patient was given intravenous urokinase for 24 hours, followed by intravenous unfractionated heparin and oral warfarin. Oral anticoagulant treatment was continued for 12 months. His symptoms completely disappeared after three months of treatment. It is important to be aware of this unusual but potentially serious complication, as early diagnosis and treatment may limit morbidity and mortality.

The value of axial view in the radiography of shoulder girdle--experiences with a new modification of positioning.

A series of 117 patients underwent radiography of the shoulder girdle, performed using a modification which allowed the examination of also severely injured patients in the axial view. The findings in the frontal and axial views were compared. A total number of 161 pathological findings were discovered; out of them 30 were visible only in the frontal view and 49 only in the axial view. The series indicates that the axial view can be taken painless in patients with recent injury, and that the frontal and axial views considerably supplement each other.

Incidence and predictors of roller derby injuries among female roller derby athletes.

Little is known about roller derby injuries. To describe the types of injuries that female roller derby athletes experience, quantify the incidence of injuries and examine the relationship between selected demographic variables and injuries. Women's roller derby leagues were contacted explaining the study and requesting participation. A second message was sent with the link to the electronic questionnaire to collect data from March to June 2015. Of the 1 395 respondents, 48.7% (679) reported having at least one injury in the previous 12 months. The skaters' average age was 32 and have played r

[Management of isolated subscapularis tendon tears].

Isolated subscapularis tendon tears are rare and often of traumatic origin. Despite specific clinical tests and exact radiological tools (ultrasound, magnetic resonance imaging), these tendon ruptures are often overlooked. For restoration of normal biomechanical joint function, immediate operative reconstruction is recommended. Tears of the upper subscapularis and partial articular-side tendon can be safely managed arthroscopically. The biceps tendon is often involved, and adequate treatment with tenotomy or tenodesis must be given to prevent postoperative pain. Complete subscapularis tendon a

Arthroscopic repair of a posterior humeral avulsion of the inferior glenohumeral ligament (HAGL) lesion.

Recently, the humeral avulsion of the inferior glenohumeral ligament (HAGL) has been described as a cause of shoulder instability. All documented cases in the literature describe the avulsion as an anterior and lateral disruption leading to anterior instability. We detail a previously unreported case of a HAGL lesion involving the posterior band of the inferior glenohumeral ligament. Arthroscopic reattachment using an additional posteroinferior portal resulted in a successful repair.
