A health-economic analysis of diagnosis and treatment of obstructive sleep apnea with continuous positive airway pressure in relation to cardiovascular disease. The Greek experience.

PURPOSE: Obstructive sleep apnea syndrome (OSAS) is common in adult population and it is associated with increased morbidity and mortality, especially due to cardiovascular disease (CVD). Both diagnosis, based on polysomnography, and treatment with continuous positive airway pressure (CPAP), carry a potentially high cost. The present study aims to analyze the cost-effectiveness of CPAP treatment versus no treatment, in the long-term, as it examines the effect of this treatment on the incidence of CVD. METHODS: A Markov model was constructed to observe the disease evolution in patients with OS

Autonomic dysregulation as a basis of cardiovascular, endocrine, and inflammatory disturbances associated with obstructive sleep apnea and other conditions of chronic hypoxia, hypercapnia, and acidosis.

Obstructive sleep apnea has traditionally been viewed as a structural disease. A multitude of systemic endocrine and cardiovascular abnormalities have been previously attributed to the prevalence of obesity in these patients. A growing body of clinical evidence, however, points to a relationship between sleep apnea and its systemic abnormalities independent of obesity. We hypothesize that this association is based on a maladaptive autonomic response of chemoreceptors, reacting to the hypoxia, hypercapnia, and acidosis of sleep apnea. The elevated sympathetic response triggers an inflammatory c

Sarcopenia, frailty syndrome and obstructive sleep apnea as elements of the aging phenotype in head and neck cancer and upper airway disorders: a scoping review.

BACKGROUND: Sarcopenia, frailty syndrome, and obstructive sleep apnea (OSA) are increasingly recognized as age-related conditions with overlapping biological pathways. Their coexistence may be relevant in head and neck cancer and upper airway disorders, but direct evidence integrating OSA with head and neck cancer (HNC) and geriatric vulnerability remains limited. OBJECTIVE: This scoping review aimed to map the evidence on the relationships among sarcopenia, frailty, and OSA in the context of head and neck cancer and upper airway disorders, and to determine whether the available literature su

The correlation between triglyceride-glucose index and the risk and severity of obstructive sleep apnea in the general population: a single-center cross-sectional study.

BACKGROUND: Obstructive sleep apnea (OSA) is a common disorder characterized by airway obstruction. Recent evidence suggests that insulin resistance (IR) may significantly impact sleep health. The triglyceride-glucose (TyG) index has emerged as a non-invasive marker of IR, potentially linked to OSA risk. However, existing studies often featured small sample sizes and limited analyses. This study aimed to evaluate the association between the TyG index and the risk and severity of OSA in the general population. METHODS: We conducted a cross-sectional, single-center, retrospective study involvin

Impact of CPAP treatment for obstructive sleep apnea on stomatognathic system function: a study on bite force, thickness, and temperature of the masseter and temporal muscles.

Obstructive sleep apnea (OSA) affects the body due to repeated obstruction of the upper airways. Continuous positive airway pressure (CPAP) is a treatment that can influence muscle function. This nine-month longitudinal study evaluated the effects of CPAP on molar bite force and on the thickness and surface temperature of the masseter and temporal muscles in adults. The study included 12 adults aged 20-70 with moderate or severe OSA, confirmed by polysomnography, all using CPAP. Molar bite force was measured with dynamometer; muscle thickness was evaluated by ultrasound at rest and during maxi

Catheter Ablation Is Associated With a Decrease in Major Adverse Cardiovascular Events and All-Cause Mortality in Patients With Atrial Fibrillation and Obstructive Sleep Apnea.

INTRODUCTION: Obstructive sleep apnea (OSA) is a common risk factor for the development and progression of atrial fibrillation (AF). Whether catheter ablation for AF management in patients with coexisting OSA yields survival benefit remains unclear. This study aimed to assess the association of catheter ablation with the incidence of major cardiovascular events (MACE) and all-cause mortality in patients with AF and OSA. METHODS: This was a retrospective cohort study using the TriNetX electronic health record (EHR) database, a global, federated clinical research network aggregating de-identifi

Effect of a Mediterranean Diet Adapted to the Mexican Population on Indicators of Metabolic Risk in Patients With Obstructive Sleep Apnea: Protocol for a Randomized Controlled Trial.

BACKGROUND: Obstructive sleep apnea (OSA) is characterized by episodes of intermittent airway obstruction during deep sleep or REM (rapid eye movement). It is associated with cardiometabolic risk diseases. The base treatment is continuous positive airway pressure (CPAP), to which not all patients are able to adapt. The Mediterranean diet (MD) has proven to be effective in reducing cardiovascular risk markers; however, it must be adapted to different populations. Among patients with OSA, it can effectively reduce clinical entity and cardiometabolic risk, while improving quality of life and slee

Three-year effects of bariatric surgery on obstructive sleep apnea in patients with obesity grade 1 and 2: a sub-analysis of the GATEWAY trial.

BACKGROUND: Most of the evidence on bariatric surgery on obstructive sleep apnea (OSA) is based on observational studies and/or short-term follow-up in patients with obesity grade 3. SUBJECTS/METHODS: This randomized study compared the effects of roux-en-Y gastric bypass (RYGB) or usual care (UC) on OSA severity in patients with obesity grade 1-2. Mild, moderate, and severe OSA was defined by the apnea-hypopnoea index (AHI): 5-14.9; 15-29.9, and ≥30 events/h, respectively. OSA remission was defined by converting any form of OSA into normal AHI (<5 events/h). RESULTS: After 3-year of follow-u

Remotely controlled mandibular positioning of oral appliance therapy during polysomnography and drug-induced sleep endoscopy compared with conventional subjective titration in patients with obstructive sleep apnea: protocol for a randomized crossover trial.

BACKGROUND: The amount of mandibular protrusion is a key factor in optimizing the efficacy of mandibular advancement device (MAD) therapy in an individual patient diagnosed with obstructive sleep apnea. This process is called titration and is generally based on resolution of subjective symptoms like snoring and/or daytime sleepiness as a function of protrusion. An objective approach uses a remotely controlled mandibular positioner (RCMP) during a full-night polysomnography (PSG), in analogy with continuous positive airway pressure (CPAP) titration. More recently, the feasibility of RCMP use du

Sleep Apnea and Fetal Growth Restriction (SAFER) study: protocol for a pragmatic randomised clinical trial of positive airway pressure as an antenatal therapy for fetal growth restriction in maternal obstructive sleep apnoea.

INTRODUCTION: Fetal growth restriction (FGR) is a major contributor to fetal and neonatal morbidity and mortality with intrauterine, neonatal and lifelong complications. This study explores maternal obstructive sleep apnoea (OSA) as a potentially modifiable risk factor for FGR. We hypothesise that, in pregnancies complicated by FGR, treating mothers who have OSA using positive airway pressure (PAP) will improve birth weight and neonatal outcomes. METHODS AND ANALYSIS: The Sleep Apnea and Fetal Growth Restriction study is a prospective, block-randomised, single-blinded, multicentre, pragmatic
