Diverse geometric changes related to dynamic left ventricular outflow tract obstruction without overt hypertrophic cardiomyopathy.

BACKGROUND: Dynamic left ventricular (LV) outflow tract (LVOT) obstruction (DLVOTO) is not infrequently observed in older individuals without overt hypertrophic cardiomyopathy (HCM). We sought to investigate associated geometric changes and then evaluate their clinical characteristics. METHODS: A total of 168 patients with DLVOTO, which was defined as a trans-LVOT peak pressure gradient (PG) higher than 30 mmHg at rest or provoked by Valsalva maneuver (latent LVOTO) without fixed stenosis, were studied. Patients with classical HCM, acute myocardial infarction, stress induced cardiomyopathy or

Sildenafil ameliorates right ventricular early molecular derangement during left ventricular pressure overload.

Right ventricular (RV) dysfunction following left ventricular (LV) failure is associated with poor prognosis. RV remodeling is thought initiated by the increase in the afterload of RV due to secondary pulmonary hypertension (PH) to impaired LV function; however, RV molecular changes might occur in earlier stages of the disease. cGMP (cyclic guanosine monophosphate)-phosphodiesterase 5 (PDE5) inhibitors, widely used to treat PH through their pulmonary vasorelaxation properties, have shown direct cardiac benefits, but their impacts on the RV in LV diseases are not fully determined. Here we show

Left ventricular diastolic pressure-volume relations in rats with healed myocardial infarction. Effects on systolic function.

To determine the effects of healed myocardial infarction on the diastolic compliance of the left ventricle, we studied 36 rats 26 days after left coronary artery ligation. Peak cardiac output and stroke volume were measured under ether anesthesia during volume loading, and peak left ventricular developed pressure was determined during occlusion of the ascending aorta. During a slow infusion of saline into the potassium-arrested left ventricle, diastolic pressure and volume were measured continuously over the pressure range -5 to 30 mm Hg. Infarct size was determined by planimetry of serial sec

Left coronary artery arteriovenous malformation presenting as a diastolic murmur with exercise intolerance in a child with a suspected familial vascular malformation syndrome.

OBJECTIVE: Intracardiac arteriovenous malformations are rare and may be associated with sudden death in adults. This case report describes an intracardiac left coronary arteriovenous malformation in a 7-year-old boy with a suspected familial cutaneous vascular malformation syndrome. The patient presented with a diastolic murmur, exercise intolerance, chest pain, and a left ventricular mass. METHODS: The left ventricular mass was initially identified by echocardiography. Subsequently, a computed tomography scan revealed the vascular nature of the lesion. We hypothesized that the lesion represe

Clinical management of dilated cardiomyopathy: current knowledge and future perspectives.

Dilated cardiomyopathy (DCM) is a primary heart muscle disease characterized by a progressive dilation and dysfunction of either the left or both ventricles. The management of DCM is currently challenging for clinicians. The persistent lack of knowledge about the etiology and pathophysiology of this disease continues to determine important fields of uncertainty in managing this condition. Molecular cardiology and genetics currently represent the most crucial horizon of increasing knowledge. Understanding the mechanisms underlying the disease allows clinicians to treat this disease more effecti

Apical Hypertrophic Cardiomyopathy: Diagnostic Challenges in a Patient with Hypertension and Obstructive Sleep Apnea.

BACKGROUND Apical hypertrophic cardiomyopathy (ApHCM) is an uncommon phenotype of hypertrophic cardiomyopathy (HCM) characterized by myocardial thickening predominantly affecting the left ventricular apex, typically without significant obstruction of the left ventricular outflow tract. This report describes a 53-year-old man with chronic hypertension and obstructive sleep apnea (OSA) in whom characteristic electrocardiographic (ECG) and cardiac magnetic resonance imaging (MRI) findings established ApHCM, highlighting practical implications for differential diagnosis and follow-up. Modification of cardiac function in cirrhotic patients with and without ascites.

OBJECTIVES: Abnormalities in cardiac function have been reported in liver cirrhosis, suggesting a latent cardiomyopathy in these patients. In this study we investigated cardiac function in cirrhotic patients and in controls. METHODS: A total of 20 cirrhotic patients without previous or ongoing ascites, 20 cirrhotic patients with moderate-to-severe ascites, and 10 healthy controls were studied by two-dimensional Doppler echocardiography. Cardiac dimensions and left and right ventricular function were evaluated. The left ventricular geometric pattern was calculated according to Ganau's criteria

Assessment of Cardiac Dysfunction in Patients With Chronic Obstructive Pulmonary Disease (COPD): A Cross-Sectional Study.

BACKGROUND: Cardiovascular diseases (CVDs) are frequent in patients having chronic obstructive pulmonary disease (COPD). Despite that, comorbid CVDs receive less guideline-recommended screening in this population compared to others. We aimed to evaluate the cardiac function using echocardiography and to assess spirometry, arterial blood gas (ABG) as well as brain natriuretic peptide (BNP) as prognostic indicators of cardiovascular dysfunction in COPD patients. METHODS: One hundred moderate to very severe COPD patients according to GOLD guidelines with no history of cardiac diseases were recru

[Impact of antihypertensive treatment on target organs].

We review the mechanisms by which arterial hypertension induces target organs damage, particularly the heart, kidney, and vascular endothelium, which is manifested as ventricular hypertrophy, proteinuria, and renal failure and endothelial dysfunction. Furthermore, the effect of antihypertensive treatment in these situations is analyzed. Experimental and clinical studies show that angiotensin converting enzyme inhibitors (ACE inhibitors) and angiotensin II receptor antagonist drugs are more efficient than other antihypertensive treatments in the reversal of left ventricular hypertrophy and prot

Salicylate reduces ventricular dysfunction and arrhythmias during reperfusion in isolated rat hearts.

Recent studies have shown the ability of salicylic acid (SA) to trap hydroxyl radicals (OH.) generated during reperfusion in ischemic myocardium. Since OH. is implicated in the pathogenesis of reperfusion injury, we examined the effect of SA on reperfusion-induced arrhythmias and postischemic ventricular dysfunction. Isolated rat hearts perfused by the Langendorff technique were preperfused with Krebs-Henseleit buffer containing SA for 10 min. Hearts were then made ischemic for 30 min, followed by 30 min of reperfusion. In a separate group, SA was administered only at the onset of reperfusion.

Differential Effects of Landiolol in Patients with Atrial Fibrillation and Atrial Tachycardia.

Landiolol, an ultra-short-acting β1-selective blocker, is more effective in controlling heart rate compared with digoxin in patients with atrial tachyarrhythmias and left ventricular dysfunction. However, the effect of atrial tachyarrhythmia type on the effectiveness of landiolol remains unknown. Hence, this study aimed to evaluate the effectiveness of landiolol in patients with atrial fibrillation (AF) and atrial tachycardia (AT), not limited to those with heart failure with a reduced ejection fraction. To this end, we evaluated the efficacy and safety of landiolol in managing tachycardiac AF

Severe foetal hypertrophic cardiomyopathy evolving to left ventricular non-compaction.

A 29-week-old male foetus was diagnosed by foetal echocardiography with severe hypertrophic cardiomyopathy with systolic dysfunction and generalized oedema, undergoing a Caesarean section at 33 weeks. Mechanical ventilation and milrinone infusion were required during the first week. Systolic function and output parameters improved progressively. Metabolic and infectious screenings were negative. At the follow-up, during the first year of life, hypertrophy regressed, posterior right auricular hypertrophy evolved to a mass with cysts, and left ventricular myocardium developed trabeculations acco

Application of stress echocardiography to the evaluation of non-coronary heart disease.

Stress echocardiography is accepted as a routine test in patients with coronary artery disease for the identification of myocardial ischaemia, the diagnosis of viable myocardium and assessment of prognosis. This review addresses the application of the test to non-coronary heart diseases. Some caution should be applied to the use of stress echocardiography for the detection of ischaemia in patients with valvular heart disease. However, examination of haemodynamics and left ventricular function during exercise is worthwhile in the assessment of symptomatic patients with apparently mild or modera

Subvalvular Aortic Stenosis.

Subvalvular aortic stenosis (SAS) is a congenital heart defect that causes fixed form of hemodynamically significant left ventricular outflow tract (LVOT) obstruction with progressive course. It has a spectrum of anatomy. It appears usually beyond infancy, causes left ventricular hypertrophy and myocardial dysfunction, and tends to involve the aortic and mitral valves in its progressive course. Although most of the patients are asymptomatic, careful monitoring is essential. Moderate to severe SAS requires surgical resection and septal myomectomy. There is a high rate of postoperative recurrenc

Normal airway responsiveness to methacholine in cardiac asthma.

Cardiac asthma has been used as a synonym for episodes of cough, dyspnea, and wheezing caused by left ventricular dysfunction. The similarity of the terms bronchial asthma and cardiac asthma, and the observed symptoms of each disease implies a common pathophysiology. Bronchial asthma is characterized pathologically by airway narrowing, inflammation, edema, and obstruction by mucus. Bronchial asthma is defined as increased responsiveness of the tracheobronchial tree, which is manifested clinically as reversible expiratory airflow obstruction. The classic symptoms of bronchial asthma are cough,

[Mechanical complications of myocardial infarction].

Despite a marked reduction in mortality after myocardial infarction during the last decades thanks to heart monitoring and early reperfusion, there remains a significant rate of in-hospital mortality. This is a consequence of refractory ventricular dysfunction in most cases, or mechanical complications of myocardial infarction in the remaining cases. Mechanical complications include septal rupture with ventricular septal defect, tamponnade following rupture of the left ventricular free wall, and acute mitral regurgitation due to papillary muscle infarction and rupture. Although these complicat

Renin-Angiotensin System Blockade in Aortic Stenosis: Implications Before and After Aortic Valve Replacement.

Aortic stenosis (AS) is a common valvular heart disease in the aging population that is characterized by a variable period of asymptomatic phase before development of symptoms and severe AS. Mortality and morbidity is substantial even after aortic valve replacement, in part related to persistent left ventricular hypertrophy, diastolic dysfunction, and heart failure. Renin-angiotensin system (RAS) blockade therapy is associated with modulation of adverse left ventricular remodeling, reduction in myocardial hypertrophy, and fibrosis, resulting in clinical improvements in patients with congestive

Interventricular septal defects from penetrating injuries of the heart: a report of 12 cases and review of the literature.

In the past 7 years, 1967 through 1973, 12 patients who had interventricular septal defects from penetrating stab wounds of the thorax were treated at Detroit General Hospital. In 10, the defects were confirmed by cardiac catheterization. Four of the 12 patients were symptomatic and required surgery. Two of these patients had pulmonary-to-system flow ratios less than 2:1. Both these patients had left ventricular dysfunction as a result of coronary artery and myocardial damage from the penetrating trauma to the left ventricle. One of the 4 patients had an injury to the tricuspid valve; this is
