Leber hereditary optic neuropathy triggered by the AstraZeneca coronavirus disease 2019 vaccination.

Leber hereditary optic neuropathy (LHON) is a rare mitochondrial disorder primarily affecting young males, characterized by progressive vision loss due to retinal ganglion cell degeneration. LHON is typically associated with specific mitochondrial mutations with potential triggers such as environmental factors and, more recently, postvaccination complications. We present a case of a 35-year-old male who experienced LHON onset 3 weeks following administration of the AstraZeneca Coronavirus disease 2019 vaccine. Despite lacking traditional risk factors, genetic testing confirmed the presence of

Pediatric Uveal Melanoma Masquerading as Vitreous Hemorrhage: A Case Report.

This case is among the first reported instances of pediatric uveal melanoma masquerading as vitreous hemorrhage on ophthalmologic examination as well as on computed tomography and magnetic resonance imaging, illustrating how reliance on imaging alone can delay diagnosis when fundus visualization is obscured. A 10-year-old girl presented with unilateral vision loss; initial imaging suggested serous retinal detachment with vitreous hemorrhage, but persistent clinical suspicion for melanoma versus retinoblastoma prompted referral to a tertiary ocular oncology service, where magnetic resonance ima

Severe Mpox keratitis resulting in significant visual impairment in a patient with advanced HIV: A case report.

Ocular complications of Mpox are relatively rare but can be devastating, particularly in immunocompromised individuals. We report a case of severe keratitis resulting in significant visual impairment in a 26-year-old male living with advanced HIV. The patient presented with a 2-month history of disseminated ulcerative lesions and a 1-month history of progressive vision loss in the right eye. He was newly diagnosed with HIV, and laboratory testing revealed a positive Mpox virus polymerase chain reaction (PCR) result alongside severe immunosuppression (CD4 + T-lymphocyte count: 35 cells/μL). A historical view of the development of corneal transplantation: from penetrating keratoplasty to selective transplantation of the finest corneal layers.

Replacement of damaged corneal tissue with a clear transplant represents a viable solution for restoring vision in patients with corneal blindness. Among organ transplants, corneal transplantation occupies a unique status due to the cornea's lack of vascularization. This avascular nature makes immune compatibility testing between donor and recipient unnecessary and reduces the risk of transplant rejection, making corneal transplantation the most successful organ transplant procedure worldwide. Over the past century, corneal transplantation has undergone a transformative journey, evolving from

Exploring geriatric syndromes among physically disabled older adults: A network analysis.

OBJECTIVE: To explore the network structure of common geriatric syndromes and conditions in physically disabled older adults. METHODS: We chose fourteen common geriatric syndromes and conditions from the dataset and estimated networks with the partial correlation network method. We tested the stability and accuracy of the network using the package "bootnet" in R software. RESULTS: The common geriatric syndromes and conditions were positively connected within the network, and strong connections emerged between chronic pain and dyspnea, and urinary incontinence and vision impairment. Acute Bilateral Vision Loss in a Young Male: A Case of Leber's Hereditary Optic Neuropathy.

Leber's Hereditary Optic Neuropathy (LHON) is a rare mitochondrial genetic disorder that primarily affects young adult males, leading to acute or subacute painless central vision loss. The condition results from point mutations in mitochondrial DNA, most commonly affecting the ND1, ND4, or ND6 genes, which impair the function of complex I in the mitochondrial respiratory chain. This leads to selective degeneration of retinal ganglion cells and the optic nerve, causing severe and often irreversible visual impairment. We present the case of a 24-year-old male farmer who consulted for rapidly pro

The future of CMR: All-in-one vs. real-time CMR (Part 2).

Cardiac Magnetic Resonance (CMR) protocols can be lengthy and complex, which has driven the research community to develop new technologies to make these protocols more efficient and patient-friendly. Two different approaches to improving CMR have been proposed, specifically "all-in-one" CMR, where several contrasts and/or motion states are acquired simultaneously, and "real-time" CMR, in which the examination is accelerated to avoid the need for breathholding and/or cardiac gating. The goal of this two-part manuscript is to describe these two different types of emerging rapid CMR protocols. To

Biparietal Osteodystrophy Visualized on a Dermatologist's Scalp Examination.

Biparietal osteodystrophy, also known as biparietal thinning, is an uncommon illness that causes symmetrical thinning of the parietal bones. It primarily affects women in their sixth and seventh decades of life. We present a case of a 73-year-old female with osteoporosis and osteoarthritis who visited a dermatology clinic with a bulging mass around her vertex scalp, accompanied by episodes of dizziness and blurred vision. The patient had a history of head trauma, but the skull abnormality was observed prior to these episodes. Laboratory testing was unremarkable except for high vitamin D levels

Bardet-Biedl syndrome: A clinical overview focusing on diagnosis, outcomes and best-practice management.

Bardet-Biedl syndrome (BBS) is a genetic disorder characterized by early-onset obesity, polydactyly, genital and kidney anomalies, developmental delay and vision loss due to rod-cone dystrophy. BBS is an autosomal recessive disorder with >20 implicated genes. The genotype-phenotype relationship in BBS is not clear, and there may be additional modifying factors. The underlying mechanism is dysfunction of primary cilia. In BBS, receptor trafficking in and out of the cilia is compromised, affecting multiple organ systems. Along with early-onset obesity, hyperphagia is a prominent symptom and cont

Retinal Organoids: A Human Model System for Development, Diseases, and Therapies.

Inherited retinal degenerations (IRD) encompasses a group of heterogeneous disorders causing debilitating visual diseases and blindness, affecting more than two million people worldwide, in all age groups. The inheritance patterns vary from autosomal dominant, autosomal recessive, X-linked, and sporadic with mutations in over 260 genes identified to date. Despite the significant advances in clinical diagnosis, there is no effective treatment available. Human-induced pluripotent stem cells (hiPSC) derived in vitro 3D retinal organoids offer a powerful preclinical tool to investigate the molecul

Experimental and computational analyses of calcium dynamics in 22q11.2 deletion model astrocytes.

Methods for deriving mechanistic information from intracellular calcium dynamics have largely been applied to neuronal data despite the knowledge of roles of glial cells in behavior, cognition, and psychiatric disorders. Using calcium imaging, computer vision, and Bayesian kinetic inference (BKI), we analyzed calcium dynamics in primary astrocytes derived from control or Df1/+ mice, a model of 22q11.2 deletion (DiGeorge syndrome). Inference of the highest-likelihood molecular kinetic characteristics of intracellular calcium dynamics identified changes in the activity of the sarcoendoplasmic re

Early disruption of photoreceptor cell architecture and loss of vision in a humanized pig model of usher syndromes.

Usher syndrome (USH) is the most common form of monogenic deaf-blindness. Loss of vision is untreatable and there are no suitable animal models for testing therapeutic strategies of the ocular constituent of USH, so far. By introducing a human mutation into the harmonin-encoding USH1C gene in pigs, we generated the first translational animal model for USH type 1 with characteristic hearing defect, vestibular dysfunction, and visual impairment. Changes in photoreceptor architecture, quantitative motion analysis, and electroretinography were characteristics of the reduced retinal virtue in USH1C

Cerebral venous sinus thrombosis: a complication of nitrous oxide abuse.

Nitrous oxide (NO) is an inhalant that has become increasingly popular as a recreational drug. While it is presumed to be harmless, a number of adverse effects of NO have been described. We discuss the case of a 24-year-old man with no medical history, who initially presented to the emergency department with progressive polyneuropathy caused by vitamin B12 deficiency after NO abuse. Two days after being discharged with hydroxocobalamin supplementation, the patient returned with a severe headache, blurry vision and slurred speech. Imaging revealed cerebral venous sinus thrombosis. Hypercoagulab
