Results are submitted of vegetative testing as well as of studying the 24.h excretion of catecholamines, activity of cholinesterase, the levels of thyroxin, triiodothyronine and hydrocortisone in blood plasma of 120 patients with tachyarrhythmias that do not respond to antiarrhythmic drug preparations. The functional state of the vegetative nervous system in the attack-free period in the above patients was mostly determined by hyperfunction of its sympathetic subdivision in the presence of inadequate vegetative reactivity, frequently excessive, occasionally weakened, and instability of adrener

BACKGROUND: Neurolytic agents such as phenol (5% to 10%) and absolute alcohol have long been used clinically to destroy the pathogenic nerve regions that manifest pain. Both phenol and alcohol are highly destructive to nerve fibers. However, these agents exert only weak local anesthetic effects and therefore are difficult to administer to alert patients without pain. This report describes a tetracaine derivative that displays both local anesthetic and neurolytic properties. Studies with such a compound may lead to the design of neurolytic agents that are more effective and more easily administ

A case of Fisher's syndrome was described in which profound hypotension was encountered. A 70-year-old previously healthy female developed diplopia and unsteady gait following an upper respiratory infection. On admission, her blood pressure was 140/60 mmHg. Adduction of the left eye and elevation of the right eye were limited. Other cranial nerve functions and muscle strength were intact. She had gait ataxia with negative Romberg sign. Tendon reflexes were hypoactive. The cerebrospinal fluid, electrocardiography, and cranial MR imaging were unremarkable. Her serum had a high titer of IgG anti-

A 78-year-old man with a history of total gastrectomy, myocardial infarction and hypertension was admitted to our hospital because of frequent transient attacks of consciousness disturbance, aphasia and right hemiparesis after meals. His attacks occurred one to three times a day, 30 to 60 minutes after meals and lasted 120 to 180 minutes. At the time of the attacks his blood pressure decreased, but his pulse rate did not change. Cerebral angiography demonstrated atherosclerotic occlusion of the left internal carotid artery. There was no myocardial uptake of 123I-metaiodobenzylguanidine (MIBG).

Back pain--a common complaint familiar to all physicians--can be either a manifestation of musculoskeletal dysfunction or a symptom of a more serious underlying disease. Unfortunately, the diagnosis can be difficult, and back pain that is not resolved by conservative treatment requires aggressive investigation. The author presents the case history of a 17-year-old patient whose back pain had been extensively evaluated by a series of specialists to no avail until further workup revealed the problem to be Ewing's sarcoma. The steps required to diagnose elusive back pain are presented, together w

OBJECTIVE: To determine the cause of spells, present clinical features, and discuss diagnostic approaches.
DESIGN: Relevant medical literature is reviewed, and three illustrative cases are presented.
RESULTS: Spells are a sudden onset of a symptom or symptoms that are stereotypic, self-limited, and recurrent. A spell involves both subjective perceptions and objective findings. In the assessment of patients who have spells, use of a systematic approach is important in determining the cause. The causes of spells include endocrine, cardiovascular, psychologic, pharmacologic, neurologic, and oth

Gastrointestinal autonomic nerve (GAN) tumors, also known as plexosarcomas, are a rare distinct subtype of the gastrointestinal stromal tumors. These tumors are usually histologically low-grade, epithelioid or spindle-cell neoplasms that can be distinguished from the other gastrointestinal stromal tumors on the basis of their unique ultrastructural features. A 66-year-old female presented with a histologically high-grade sarcoma of the small bowel. Ultrastructural studies showed features of a GAN tumor. The light microscopic and ultrastructural features are described. The tumor cells gave stro

The above study is performed in 30 cases of mammary dysplasia (MD) and in 30 cases of mammary carcinoma (MC). "Correlation" bridge between the cell groups was an endothelial cell in the adrenergic zone and lymphocyte in the cholinergic one. A reciprocal influence of the effector part mediators and surrounding stromal cells is thought to influence the structural organization and biological behaviour of MD and MC.

Taxol is one of the more significant advances in cancer chemotherapy in the last 10-15 years with a 30-50% response rate in both breast and ovarian cancers. The toxicity profile of taxol is still evolving as changes in the dosing and scheduling of the drug are instituted. We herein present the first evidence of taxol-induced quantifiable, objective autonomic neuropathy in two patients, one with and one without diabetes. The patients did not have symptoms of autonomic dysfunction prior to taxol therapy. Both developed severe orthostatic hypotension following completion of 24-hr infusional taxol

A physiological monitoring capability was developed for the Army's Crew Station Research and Development Facility (CSRDF), a research simulator for advanced rotorcraft. Preliminary physiological data are reported from studies of simulator-induced sickness. Our objective was to demonstrate sensitivity of physiological measures relative to self-reports of simulator sickness severity. The data suggested that heart period, tachygastria, and skin conductance level were more sensitive to simulator sickness than were vagal tone and normal myoelectrical gastric activity.

The histopathologic, immunohistochemical and electronmicroscopic findings of four dysautonomic cats are presented. The main histopathologic changes were seen in the nervous system, e.g. in sympathetic and parasympathetic ganglia that were similarly affected. Acute cases showed a degeneration of most of the neurons, in one subacute case less neurons and degeneration of some of the remaining neurons were present. Degenerating neurons were swollen or shrunken and had pycnotic, eccentric nuclei. The Nissl substance was lost, the cytoplasma stained eosinophilic and was often vacuolated. Degeneratin

We report a case of multiple system atrophy-autonomic failure in which the most interesting features were the uncommon onset of a laryngeal stridor in the absence of orthostatic hypotension and the clinical-nosographic possibilities for the autonomic changes found. The differentiation between autonomic failure and pure autonomic failure and their relationship to multiple system atrophy are still under discussion. We agree with the authors who consider that the absence/presence of orthostatic hypotension may be taken as discriminant for this purpose.

We used CT to make a quantitative evaluation of the pontine volumes of 55 patients with adult-onset cerebellar degeneration and compared the results with their clinical records. Patients with pontine atrophy showed significantly more autonomic dysfunction and extrapyramidal signs; their other clinical features did not differ from patients without pontine atrophy. After dividing the patients into 2 groups, those with and without autonomic dysfunction or extrapyramidal signs, we found that those with these manifestations also had pontine atrophy, a later onset and shorter duration of the disease

The effects of naloxone and a met-enkephalin analogue on head pain, vascular responses, and autonomic-associated symptoms were studied in 24 migraine patients, 12 patients suffering from tension-type headache, and 24 normal subjects in whom headache was induced by intravenous injections of increasing doses of histamine (histamine test). A hypersensitivity to histamine was found in migraine patients. Naloxone slightly increased the intensity of pain in migraine and tension-type headache sufferers. The met-enkephalin analogue did not affect the intensity of pain in migraine patients, tension-typ

Familial amyloidotic polyneuropathy (FAP) is characterized by both sensimotor and autonomic dysfunction. Autonomic disturbance involving the gastrointestinal tract, the urinary bladder, the cardiac conduction system, and the peripheral circulation has been described. In this study simple, non-invasive tests of autonomic function based on heart rate variability were applied to 12 patients with FAP and 12 healthy volunteers. The heart rate variation during normal breathing, deep breathing and during tilt from recumbent to standing position was measured. All tests showed significantly less heart

The site of origins, peripheral courses and intramucosal distribution of the sympathetic post-ganglionic nerve fibers supplying to the larynx were investigated by means of wheat germ agglutinin-horseradish peroxidase (WGA-HRP) and Falck-Hillarp method in 51 cats. The cervical sympathetic ganglia which send the postganglionic fibers to the larynx were the superior cervical ganglion (SCG), middle cervical ganglion (MCG), and stellate ganglion (SG). Location of MCG was at the bifurcation of the cervical sympathetic trunk, rostrally to the point of its crossing with the subclavian artery. After WG
