Metastatic bone disease of the humerus.

Metastatic bone disease is the most common cause of destructive bone lesions in adults, and involvement of the humerus is common. Patients with destructive lesions involving <50% of the cortex are treated nonsurgically with external beam irradiation. Patients with diaphyseal lesions involving > or =50% of the cortex or those with pain after irradiation can be treated with intramedullary nailing to achieve rigid fixation. Although closed intramedullary nailing is used most often, open nailing with methylmethacrylate is appropriate for destructive lesions in which rigid fixation cannot be achiev

[The surgical treatment of fractures of the base of the fifth metacarpal bone].

Fractures of the base of the fifth metacarpal bone are relatively uncommon injuries. In contrast to similar fractures of the first metacarpal bone, various methods of treatment have been advocated. Some believe that cast immobilization or immediate unrestricted motion is a reliable method, while others recommend surgery and pinning or plate fixation. Fractures of the base of the fifth metacarpal bone are inherently unstable and inadequate reduction may result in pain, functional disability and osteoarthritic changes. We retrospectively reviewed 14 cases with intra-articular or extra-articular

Treatment of a large segmental bone defect with allograft and autogenous bone marrow graft.

Management of segmental bone defects is a challenge to orthopedic surgeons. We report a 23-year-old man who sustained segmental bone loss of 9 cm in length in his left femur due to an open fracture. Open reduction and internal fixation with a 95 degrees angle blade plate and screws, supplemented with a composite of two femoral head allografts and a graft of 100 mL of autogenous bone marrow, was performed 3 weeks after the injury, when the soft tissues had healed. The postoperative rehabilitation program included delayed weight bearing (crutches for 3 months), a limited early range of motion of

Phenolic acids prevent sex-steroid deficiency-induced bone loss and bone marrow adipogenesis in mice.

Phenolic acids, such as hippuric acid (HA) and 3-(3-hydroxyphenyl) propionic acid (3-3-PPA), can be produced from microbiome digestion of polyphenols. Previously it was found that HA and 3-3-PPA facilitate bone formation and suppress bone resorption. However, the mechanism of action by which HA and 3-3-PPA protect bone from degeneration is currently unknown. In this report, we present that HA and 3-3-PPA suppression of bone resorption is able to ameliorate bone loss in an ovariectomy (OVX) osteopenic mouse model though not to the extent of Zoledronic acid (ZA). HA and 3-3-PPA treatments were s

Metabolic regulation of skeletal cell fate and function.

Bone development and bone remodelling during adult life are highly anabolic processes requiring an adequate supply of oxygen and nutrients. Bone-forming osteoblasts and bone-resorbing osteoclasts interact closely to preserve bone mass and architecture and are often located close to blood vessels. Chondrocytes within the developing growth plate ensure that bone lengthening occurs before puberty, but these cells function in an avascular environment. With ageing, numerous bone marrow adipocytes appear, often with negative effects on bone properties. Many studies have now indicated that skeletal c

Abnormal bone remodelling in inflammatory arthritis.

Osteopenia is responsible for substantial comorbidity in patients suffering from rheumatoid arthritis and is an important factor in the surgical management of joint disease. In animal models of bone loss stimulated by inflammatory arthritis, increased bone remodelling and altered microstructure of bone have been documented. The subchondral bone plate near the joint surface is narrow and perforated by vascular inflammatory invasion, and in the shaft the thin cortices are weakened by giant resorption defects. Biomechanical tests and a mathematical model of bone strength suggest that cortical def

Biomechanical evaluation of a medial-only fixation strategy for Takeuchi type 2 lateral hinge fractures in medial open wedge high tibial osteotomy.

BACKGROUND: Medial opening wedge high tibial osteotomy (MOWHTO) is a standard treatment for knee osteoarthritis. However, lateral hinge fractures occur in 19-25% of cases, with Type 2 fractures causing significant instability. While lateral plating is recommended, it requires an additional incision. This study investigated whether medial-only fixation using a 4.5 mm cortical screw inserted into the Tomofix plate's oval hole could provide sufficient stability for Type 2 hinge fractures. METHODS: Twelve fresh pig knees underwent MOWHTO with a 6 mm opening. They were divided into two groups: Gro

Intramedullary locking plates device versus headless screw fixation for treatment of hallux valgus deformity: A retrospective observational study.

This retrospective observational study aimed to evaluate the outcomes of subcapital osteotomy using an intramedullary locking plate for the treatment of hallux valgus (HV) deformity. We hypothesized that plate fixation would provide superior angular correction and lower recurrence compared to screw fixation. Between January 2018 and July 2023, a total of 49 feet underwent corrective osteotomy for HV. Of these, 36 feet were treated with headless screws and 13 with intramedullary locking plates. All cases had a minimum follow-up duration of 12 months. Radiographic outcomes were assessed using we

The effects of fixation methods on the biomechanics of comminuted distal metaphyseal tibia fractures.

Bone fractures require the proper integration of fixation devices to ensure long-term biomechanical performance and accelerated recovery, while allowing for early load-bearing and equitable load-sharing. The primary objective of this research is to elucidate the effect of four different generations of fixation devices on synthetic tibiae with induced comminuted fractures. The fixation devices considered herein comprised an intramedullary nail, as well as various distal interlocking screws and plates. The overall biomechanical response was studied under quasi-static compressive loading, followi

Elastic stable intramedullary nails compared to locking compression plates for treating unstable distal ulnar fractures in adults: a prospective comparative study.

BACKGROUND AND PURPOSE: Distal ulna fractures often occur in conjunction with distal radius fractures and other associated injuries. Currently, there are no satisfactory internal fixation systems available for addressing unstable distal ulna fractures, and a definitive consensus on the most effective treatment approach is still lacking. The objective of this research was to evaluate the clinical outcomes of using elastic stable intramedullary nails (ESIN) compared to locking compression plates (LCP) for treating unstable distal ulnar fractures in adults. METHODS: In a prospective clinical stu

Biomechanics of humeral locking plate augmented with fibular strut allograft and intramedullary strut plate using finite element analysis.

A humeral locking plate augmented with fibular strut allograft treated for proximal humeral fracture without internal structural support is satisfactory. While it is better clinically and biomechanically than the locking plate alone, it has disadvantages, including difficulty to obtain and the possibility of infection. Other alternative augmentation approaches are in demand. Therefore, the hypothesis of this study is whether intramedullary strut plate can replace fibular strut allograft as a surgical method while providing similar biomechanical performance. The finite element analysis (FEA) mo

Medial buttress plate use in neck of femur fracture fixations: A systematic review.

BACKGROUND: Femoral neck fractures, particularly Pauwels type II and III, pose significant challenges due to their vertical instability and susceptibility to complications such as non-union and avascular necrosis (AVN). Medial buttress plates (MBPs) have emerged as a promising adjunct in fixation, offering biomechanical advantages by neutralizing shearing forces and enhancing stability. However, the clinical efficacy of MBPs across different fixation techniques, plate configurations, and positioning remains unclear. PURPOSE: This study aimed to (1) analyse outcomes of femoral neck fracture fi

Behavior of Locked Intramedullary Implants Spanning the Distal Femoral Growth Plate: Results From an Ovine Model.

INTRODUCTION: Safe retrograde femoral nail placement across the distal femoral growth plate could broaden fracture and reconstructive management options for skeletally immature patients, especially in resource-poor settings. Previous data suggested up to 7% of the distal femoral physis can be violated using a retrograde implant without subsequent growth arrest or inhibition. When the intramedullary implant is locked in the metaphysis, the bone will grow with the nail passing from the epiphysis to the metaphysis. However, the consequences of this growth across the physis are not well understood

Is there a negative correlation between screw length and correction rate via guided growth? A retrospective study of 138 limbs with genu varum.

PURPOSE: To identify factors that contribute to the speed of angular correction in skeletally immature patients with genu varum treated with paraphyseal tension band plates, hypothesizing that screw length and divergence, severity of deformity, and underlying pathology influence the rate and speed of genu varum correction. METHODS: Fifty-three patients (38 males, 15 females) undergoing genu varum correction were included; a total of 138 physes (64 distal femur, 74 proximal tibia) were assessed. The median age at surgery was 37.83 ± 34.27 months (24.37-188.60), and the median follow-up was 11.

Minimally Invasive Plate Osteosynthesis for Scapular Fractures.

This procedure demonstrates a 65-year-old man who sustained an injury approximately 2 weeks ago with a displaced multifragmented intra- and extra-articular scapular fracture. It describes the technique of a minimally invasive approach and fixation with the use of mini fragment plating. Indications are a displaced extra-articular scapula body, glenoid neck, and intra-articular fracture of the glenoid. Intraoperative imaging confirms anatomic reduction and stable fixation. Follow-up shows a good functional outcome and return to normal daily life.

Achieving correction of moderate to severe hallux valgus deformity of the 1st metatarsal by Proximal opening wedge osteotomy using a locking plate (A prospective study).

BACKGROUND: The hallux valgus deformity is among the most frequent foot deformities. Treatment of symptomatic moderate-to-severe hallux valgus deformities offers huge challenges in foot and ankle. There are many surgical procedures have been described for the correction of moderate-to-severe hallux valgus deformity. Proximal osteotomies has become popular with the recent development of low-profile plates for the opening wedge. We believe that the proximal opening wedge osteotomy technique provides excellent correction, technically easy to perform and reproducible. PATIENTS AND METHODS: This is

Removal of the Metaphyseal Screw From Tension Band Constructs After Hemiepiphysiodesis: High Rates of Physeal Tethering and Subsequent Implant Removal.

BACKGROUND: Removal of the metaphyseal screw from tension band plate constructs after correction of angular deformity in patients treated with hemiepiphysiodesis has been suggested as an alternative to removing the plate and both screws. While this has the potential benefit of easier implant removal and reinsertion in the event of rebound, there is debate in the literature regarding the benefits and risks of leaving the epiphyseal screw and plate in place. METHODS: Patients treated with hemiepiphysiodesis at the distal femur and/or proximal tibia with tension band plates and screws who underw
