Cognitive decline preceding the onset of psychosis in patients with 22q11.2 deletion syndrome.

IMPORTANCE: Patients with 22q11.2 deletion syndrome (22q11DS) have an elevated (25%) risk of developing schizophrenia. Recent reports have suggested that a subgroup of children with 22q11DS display a substantial decline in cognitive abilities starting at a young age. OBJECTIVE: To determine whether early cognitive decline is associated with risk of psychotic disorder in 22q11DS. DESIGN, SETTING, AND PARTICIPANTS: Prospective longitudinal cohort study. As part of an international research consortium initiative, we used the largest data set of intelligence (IQ) measurements in patients with 22

Psychotherapy for Delusional Disorder: Theoretical Models and Therapeutic Techniques.

Delusional disorder is a challenging mental health condition given its resistance to conventional treatment with antipsychotic medications. Although patients with delusional disorder often retain good functioning outside of their delusional theme, they often become functionally impaired as delusional ideas become more central in their lives. Psychotherapy represents an important treatment modality for these patients, in that it may be the most effective treatment option to date. Theorists from both the psychodynamic and cognitive-behavioral schools have offered frameworks for understanding and

Neuropsychological and Neuropsychiatric Features of Idiopathic and DYT1 Dystonia and the Impact of Medical and Surgical treatment.

Dystonia is a hyperkinetic movement disorder, characterized by sustained or intermittent muscle contractions causing abnormal, often repetitive, movements, postures, or both. Executive dysfunction is a feature of cognitive function in idiopathic and DYT1 dystonia. Psychiatric morbidity is increased in dystonia, and depression, anxiety, obsessive compulsive disorders are the most common disorders. Sleep problems and pain are also frequently experienced. Evidence suggest that mood and anxiety disorders are intrinsic to the neurobiology of dystonia, but also that psychiatric co-morbidity can be s

Neurodevelopmental disorders in children with neurofibromatosis type 1.

Over the past several decades, neurofibromatosis type 1 (NF1) has become increasingly recognized as a neurodevelopmental disorder conferring increased risk for several important neurodevelopmental problems. In this review, we summarize the specific neurodevelopmental problems encountered in the context of NF1. These include impairments in general cognitive function, deficits in specific cognitive domains such as executive function and visuospatial processing and risk for specific learning disorders, impairments in attention and social skills and the overlap with attention-deficit-hyperactivity

Substance-use disorder in high-functioning autism: clinical and neurocognitive insights from two case reports.

BACKGROUND: Low prevalence of substance-use disorder has been reported in adults with autism. However, on a superficial level, adults with high-functioning autism (HFA) display a 'normal' façade when they drink alcohol, which may explain why their alcohol dependency is not better diagnosed. CASE PRESENTATION: Here, we report two cases of HFA adults who use alcohol and psychostimulants to cope with their anxiety and improve their cognitive abilities and social skills. We analyze how neurocognitive traits associated with HFA may be potential triggers for substance-use disorder. CONCLUSION: Bet

Brain metabolite alterations and cognitive dysfunction in early Huntington's disease.

Huntington's disease (HD) is a neurodegenerative disorder characterized by early cognitive decline that progresses at later stages to dementia and severe movement disorder. HD is caused by a cytosine-adenine-guanine triplet-repeat expansion mutation in the Huntingtin gene, allowing early diagnosis by genetic testing. This study aimed to identify the relationship of N-acetylaspartate and other brain metabolites to cognitive function in HD-mutation carriers by using high-field-strength magnetic resonance spectroscopy (MRS) at 7 Tesla. Twelve individuals with the HD mutation in premanifest or ear

Thought-action fusion across anxiety disorder diagnoses: specificity and treatment effects.

Thought-action fusion (TAF) is a cognitive error that has been frequently investigated within the context of obsessive-compulsive disorder (OCD). However, evidence suggests that this error may also be present in disorders other than OCD, indicating that TAF is related to higher order factors rather than a specific diagnosis. We explored TAF in a sample of patients with mixed diagnoses undergoing treatment with a transdiagnostic CBT protocol. Elevated TAF levels at baseline were not specific to patients with OCD. However, the presence of any generalized anxiety disorder (GAD) diagnosis was unex

Cognitive Vulnerabilities for Depression and Anxiety in Childhood: Specificity of Anxiety Sensitivity and Rumination.

BACKGROUND: Childhood anxiety and depression frequently co-occur. Exploring specificity in cognitive processes for anxiety and depression in childhood can provide insight into cognitive vulnerabilities contributing to the development of anxiety and depressive disorders and inform targeted psychological interventions. Anxiety sensitivity and rumination are robust cognitive vulnerabilities for anxiety and depression, respectively. However, despite conceptual similarities, they are rarely considered together within a single study. AIMS: The current study explored specific and shared associations

Dysfunctional attitudes and cognitive schemas in bipolar manic and unipolar depressed outpatients: implications for cognitively based psychotherapeutics.

Dysfunctional thought patterns are presumed to underlie cognitive biases in mood disorder patients. However, few studies have compared dysfunctional thought patterns in bipolar manic and unipolar depressed patients. Cognitive schemas and dysfunctional attitudes were evaluated using the cognitive checklist for mania and Dysfunctional Attitudes Scale (DAS) in 34 bipolar manic, 35 unipolar depressed, and 29 nonpsychiatric control subjects. Unipolar depressed subjects had significantly higher total DAS scores and subfactor scores as compared with nonpsychiatric controls, whereas bipolar patients h

Using individualized structural covariance networks to analyze the heterogeneity of cerebral small vessel disease with cognitive impairment.

BACKGROUND: Cerebral small vessel disease (CSVD) includes vascular disorders characterized by heterogeneous pathomechanisms and different neuropathological clinical manifestations. Cognitive dysfunction in CSVD is associated with reductions in structural covariance networks (SCNs). A majority of research conducted on SCNs focused on group-level analysis. However, it is crucial to investigate the individualized variations in order to gain a better understanding of heterogeneous disorders such as CSVD. Therefore, this study aimed to utilize individualized differential structural covariance netwo

Circuit based anti-correlation, attention orienting, and major depression.

Major depression is a multidimensional disorder producing emotional dysregulation, cognitive impairment, and neuro-vegetative symptoms. A pathophysiological model of depression needs to explain how these dimensions interact to produce specific clinical phenotypes and how these interactions may predict remission to specific treatments. It is unlikely that major depression results from discrete brain lesions. Here we propose to define major depression as a disorder of neural networks. We review evidence suggesting that the dynamics of neural networks involved in allocation of attention resources

Tsc1-mTORC1 signaling controls striatal dopamine release and cognitive flexibility.

Tuberous Sclerosis Complex (TSC) is a neurodevelopmental disorder caused by mutations in TSC1 or TSC2, which encode proteins that negatively regulate mTOR complex 1 (mTORC1). TSC is associated with significant cognitive, psychiatric, and behavioral problems, collectively termed TSC-Associated Neuropsychiatric Disorders (TAND), and the cell types responsible for these manifestations are largely unknown. Here we use cell type-specific Tsc1 deletion to test whether dopamine neurons, which modulate cognitive, motivational, and affective behaviors, are involved in TAND. We show that loss of Tsc1 an

Relationships among maladaptive cognitive content, dysfunctional cognitive processes, and borderline personality features.

Previous research has demonstrated that maladaptive cognitive content, including dysfunctional attitudes and negative automatic thoughts, is associated with emotional distress. Similarly, dysfunctional cognitive processes, including thought suppression and rumination, have been shown to intensify psychological difficulties. Although maladaptive cognitive content and dysfunctional processes have been linked to borderline personality disorder (BPD), most research has been conducted with Axis I disorders. This study examined the incremental validity of dysfunctional cognitive content and processe

Influence of appearance-related TV commercials on body image state.

This study investigates the influence of media exposure on body image state in eating-disordered (ED) patients. The attitudinal and perceptual components of body image are assessed, as well as any associations with dysfunctional cognitions and behavioral consequences. Twenty-five ED patients and 25 non-ED controls (ND) viewed commercials either featuring appearance (AC; 5 min) or not featuring appearance (NC; 5 min). Both perceptual and attitudinal body image components changed markedly after the AC condition for ED patients, compared with the ND group and NC condition. Cognitions referring to

Thought deficit in psychotic depression and chronic paranoid schizophrenia: methodological and conceptual issues. Commentary on Marjorie L. Carter's "The assessment of thought deficit in psychotic depression and chronic paranoid schizophrenia".

Based on test comparisons involving psychotic depressives and chronic paranoid schizophrenics, Carter (J. Nerv. Ment. Dis. 174: 336-341, 1986) inferred that cognitive disorder may reflect a general psychosis factor rather than a differential diagnostic marker. Review of the design and analysis raises methodological and conceptual issues that obviate such interpretation. Problems include absence of control for IQ and chronicity of illness, sample bias incurred by selectively matching groups for education, use of parametric statistics on ordinal data, measures that do not differentially assess c

[Forgetfulness].

In old age, cognitive functions decline in comparison to performance earlier in life. Subjective memory complaints often cause anxiety. Patient and informant history taking, along with simple additional cognitive tests are usually sufficient to reassure patients or to initiate further diagnostic work-up. Differential diagnoses include depression, delirium, other psychiatric or neurological disorders, metabolic causes, intoxications or side effects of drugs. Physical examination and laboratory tests generally have limited added value. Main reasons for referral are a suspected treatable cause fo

Characteristics of cognitive functions in children with epilepsy.

The aim of the present work was to study the causes and structure of interictal cognitive and emotionalpersonality impairments in children with different types of epilepsy. A total of 40 children were studied, aged from eight to 18 years. Symptomatic epilepsy was diagnosed in 31 patients (77.5%) and idiopathic epilepsy in nine (22.5%). Cognitive functions were compared in relation to the type of epilepsy, type of seizures, and treatment with antiepileptic and its efficacy. The results showed that intellectual-mnestic disorders were seen in 62% of children with epilepsy, more profound impairmen

[Sleep-disordered breathing in early childhood: their neurocognitive repercussions].

Sleep-disordered breathing in early childhood includes a wide array that includes conditions ranging from primary snoring to the most severe forms of obstructive sleep apnoea syndrome. It may be associated to behavioural and cognitive alterations such as learning and memory disorders or problems with attention, even in the milder forms of the disorder. Our aim is to review the clinical manifestations of sleep-disordered breathing in early childhood, focusing on the neurocognitive and behavioural deficits, as well as the pathophysiological mechanisms involved. Sleep-disordered breathing produce
