A case of melioidosis with a mycotic aneurysm is reported. The blood and tissue isolates were identified as three different species of Burkholderia using the automated identification systems, VITEK 2 and matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS). The isolate was confirmed as Burkholderia pseudomallei by 16S rRNA sequencing. The typical features of the Gram staining of colonies and 16S rRNA sequencing can be useful to identify B. pseudomallei.

Group B streptococcus (GBS) is the most common pathogen in neonates and may induce the overproduction of cytokines. To further clarify temporal alterations in the levels of various cytokines/chemokines, we measured the concentrations of 16 types of these immunological responders in the serum of a neonate presenting with GBS infection. At birth, the concentrations of different cytokines/chemokines increased and that of granulocyte colony-stimulating factor remained high. Thus, these cytokines/chemokines might be associated with the pathophysiology of GBS infection.

We describe a case of empyema thoracis caused by Rhizopus oryzae diagnosed in an allogenic bone marrow transplant patient with acute lymphocytic leukemia. The isolate of R. oryzae was recovered from three pleural effusion specimens, which were black in color. It was identified on the basis of characteristic colonial appearance and microscopic findings, as well as the partial sequencing of rRNA genes. The patient died of uncontrolled R. oryzae empyema thoracis and concomitant nosocomial infection.

Quantitatively, wounds harboring bacteria that exceed 105 colony-forming units per gram are considered infected wounds. There are acute wounds and chronic wounds, and the approach to controlling the infection is similar. Although the granulating bed may be avascular, systemic anti-infectives are employed adjunctively to circumvent systemic infection. The main armamentarium of the attack is the use of topical anti-infectives, which invade the bacteria where they reside, and, consequently, reduce their numbers and promote wound healing.

Ticlopidine is an oral antiplatelet agent frequently utilized in the treatment of cerebrovascular disease. It is rarely associated with severe bone marrow suppression. A case of an elderly woman is reported who developed febrile agranulocytosis two months after commencing ticlopidine but who had a favorable outcome after cessation of that drug and treatment with granulocyte colony-stimulating factor (G-CSF). All patients should have regular monitoring of their blood counts during therapy with ticlopidine.

The deployment of the first dental field officer by the British army during the Anglo-Boer War (1899-1902) demonstrates a further South African contribution to dental history.

A 56-year-old male presented with a history of dull pain in the abdomen, accompanied by anorexia and weight loss. On examination, he had a moderate tenderness in the right kidney region. Radiological investigation could not deny renal tumor, and the right kidney was removed. Microscopically, characteristic colonies of actinomyces were seen. Preoperatively it is difficult to differentiate renal actinomycosis and renal tumor. Removal of the involved kidney followed by appropriate antibiotic therapy may be promptly curative.

Detection of Salmonella typhi in blood by culture of the mononuclear cell-platelet layer was compared with other methods currently used for the diagnosis of typhoid fever. Colonies of S. typhi were present in all mononuclear cell-platelet layer-positive cultures within 18 h of plating and were identified within an additional 10 min by a coagglutination technique. In contrast, identification of all positive cultures by conventional blood culture required 3 days.

Sixteen micro-organisms, representing clinically important respiratory microflora, were tested for their ability to stimulate the growth of Legionella pneumophila and Tatlockia micdadei in nutritionally-deficient agar media. Nutritional symbiosis, indicated by the appearance of satellite colonies of L. pneumophila or T. micdadei, was observed for H. influenzae and N. meningitidis. This interaction between normal flora and pathogens of the respiratory tract may have clinical relevance in the pathogenesis of Legionnaires' disease and Pittsburgh pneumonia.

A histidine-containing agar medium has been devised for quantitative detection of histamine-producing bacteria that are alleged to be associated with scombroid fish poisoning outbreaks. The responsible bacteria produce a marked pH change in the agar, with attendant color change of pH indicator adjacent to the colonies, thus facilitating their recognition. Proteus morganii and Klebsiella pneumoniae were the two most common histidine-decarboxylating species isolated from scombroid fish and mahi mahi.
