Enterovesical fistula in a patient with B-cell lymphoma.

Enterovesical fistula is a known complication of several conditions involving the bowel or the genitourinary system. Its occurrence with primary small-bowel lymphoma is rare. The authors report a case of enterovesical fistula associated with secondary small-bowel non-Hodgkin's lymphoma. They review the criteria for classification of small-bowel lymphoma as primary or secondary and discuss the radiologic findings consistent with enterovesical fistula, with reference to the role of computed tomography as the initial radiologic study.

Cyst of the choroid plexus in the lateral ventricle: case report and review of the literature.

We report an intraventricular cyst in a 2-year-old boy who had myoclonic jerks of the extremities. The cyst had no communication with the ventricular system or subarachnoid space. Total removal of the cyst adhering to the choroid plexus was accomplished. The glial cyst wall was lined in part by flattened or cuboidal epithelium. The preoperative and postoperative computed tomographic scans and magnetic resonance images are presented, and congenital benign cysts in the lateral ventricle are reviewed and discussed.

Causes and circumstances of maternal death: a secondary analysis of the Community-Level Interventions for Pre-eclampsia (CLIP) trials cohort.

BACKGROUND: Incomplete vital registration systems mean that causes of death during pregnancy and childbirth are poorly understood in low-income and middle-income countries. To inform global efforts to reduce maternal mortality, we compared physician review and computerised analysis of verbal autopsies (interpreting verbal autopsies [InterVA] software), to understand their agreement on maternal cause of death and circumstances of mortality categories (COMCATs) in the Community-Level Interventions for Pre-eclampsia (CLIP) cluster randomised trials. METHODS: The CLIP trials took place in India,

A clinical decision support system for ECG diagnosis: Surpassing commercial standards through dual-stream feature fusion.

BACKGROUND: Automated electrocardiogram (ECG) interpretation is essential for clinical efficiency, yet many existing deep learning (DL) models are perceived as "black boxes" and lack direct benchmarking against established commercial standards. To foster clinician trust and support reimbursement justification, AI systems must demonstrate not only superior accuracy but also alignment with established clinical criteria. METHODS: We proposed a Dual-Stream Signal Fusion Architecture that integrates latent spatio-temporal features from 21,799 raw 12-lead ECG waveforms (PTB-XL) with 148 expert-engi

EEG-AI: An agentic system for AI-assisted semi-automated EEG preprocessing and artifact removal.

BACKGROUND: EEG is widely used to identify neural markers, personalize treatments, and evaluate interventions. However, low signal-to-noise ratio and mixing of artifactual distorted interpretation. Traditional preprocessing approaches, including Independent Component Analysis (ICA), require extensive manual inspection and are laborious. This study aims to develop a human-in-the-loop preprocessing framework, in which AI agents assist experts in identifying, labeling, and iteratively refining artifact removal decisions. NEW METHOD: The proposed framework integrates a large language model (LLM)-

Acute Traumatic Aortic Injury: What the Radiologist Needs to Know.

Acute traumatic aortic injury (ATAI) is a rare but life-threatening consequence of blunt trauma that requires prompt diagnosis and accurate imaging assessment. This review presents an imaging-based approach to ATAI, with emphasis on computed tomography angiography (CTA) as the first-line modality for diagnosis, grading, treatment planning, and follow-up. CTA enables the detection of both direct and indirect signs while also allowing for the assessment of lesion severity, extent, and associated findings that may influence management. Familiarity with common mimics and anatomic variants improves

Advances in the Management of Sternoclavicular Joint Injuries.

➢ The sternoclavicular joint (SCJ) serves as the only osseous connection between the axial skeleton and the upper limb and is a synovial, saddle-like joint with robust posterior ligamentous stabilizers and a fibrocartilaginous disc. ➢ The brachiocephalic veins and other mediastinal structures are at risk from injury or surgery about the SCJ. ➢ SCJ injuries are best imaged with computed tomography (CT). CT angiography is warranted when a vascular injury is suspected, and magnetic resonance imaging (MRI) is useful to define soft-tissue injuries. ➢ Acute posterior SCJ dislocations in active, heal

LungGPT: A unified multimodal system for interpretable diagnosis and clinical decision support of respiratory diseases.

Respiratory diseases cause significant morbidity, yet diagnosis remains labor intensive and dependent on physician expertise. Here, we present LungGPT, a unified multimodal system trained on 147 million tokens of domain-specific electronic health records from 125,917 participants. LungGPT comprises two modules: LungGPT-Dx for respiratory disease diagnosis and early warning of critical illness, and LungGPT-Ex for interpretable diagnostic reasoning and treatment recommendations. In large-scale evaluations, LungGPT-Dx achieves a macro-average area under the curve (AUC) of 0.852 (95% confidence in

Diffuse splenic calcifications in systemic lupus erythematosus: a case-based review.

Systemic lupus erythematosus (SLE) is a multi-organ autoimmune disease with a wide spectrum of splenic manifestations. In recent literature, splenic calcifications have increasingly been recognized as a rare manifestation of SLE. We describe a patient presenting with fever in whom abdominal imaging revealed splenomegaly with diffuse splenic microcalcifications, prompting further diagnostic evaluation that ultimately led to the diagnosis of SLE. To contextualize this finding, we performed a systematic case-based literature review and identified 29 previously reported cases, resulting in a total

Head Tracking for Confirming User Selection for Human-Machine Interaction.

This paper presents a human-machine interface designed to help individuals with upper-limb impairments select items on a screen using only head movements. The system allows users to make selections and confirm choices without requiring hand input. A study with 22 participants (11 male, 11 female, average age 23 years) evaluated its performance across three configurations. Results showed a clear learning curve, with participants achieving greater accuracy and consistency over time. Users adapted well to the system, maintaining stable mouth-opening and head-transition movements close to target v

Dynamic gene regulatory network inference from single-cell data using optimal transport.

MOTIVATION: Modelling gene expression is a central problem in systems biology. Single-cell technologies have revolutionized the field by enabling sequencing at the resolution of individual cells. This results in a much richer data compared to what is obtained by bulk technologies, offering new possibilities and challenges for gene regulatory network inference. RESULTS: In this work, we introduce GRIT (gene regulation inference by transport)-a method to fit a differential equation model and to infer gene regulatory networks from single-cell data using the theory of optimal transport. The idea

An in-silico analysis of the influence of left bundle branch block and myocardial fibrosis on electrocardiographic QRS duration and morphology and vectorcardiographic QRS area.

BACKGROUND AND AIMS: Vectorcardiographic (VCG) QRS area has shown great promise as a predictor of cardiac resynchronization therapy outcomes. However, underlying conduction factors influencing the genesis of QRS area and its relationship with myocardial fibrosis remain unexplored. This study aimed to investigate the impact of left bundle branch block (LBBB) and fibrosis on QRS duration and morphology in comparison to QRS area through an in-silico analysis. METHODS: ECGsim was used to simulate four scenarios: normal ventricular activation without (1) and with fibrosis (2), and LBBB without (3)

Computational prediction of deleterious nonsynonymous SNPs in the CTNS gene: implications for cystinosis.

BACKGROUND: Cystinosis is a rare autosomal recessive lysosomal storage disorder caused by mutations in the CTNS gene, which encodes cystinosin, a lysosomal cystine transporter. These mutations disrupt cystine efflux, leading to its accumulation in lysosomes and subsequent cellular damage. While more than 140 mutations have been identified, the functional and structural impacts of many nonsynonymous single nucleotide polymorphisms (nsSNPs) remain poorly understood. Nonsynonymous SNPs are of particular interest because they can directly alter protein structure and function, potentially leading t

The ADME profile of threo-4-methylmethylphenidate (4-Mmph, 4-MeTMP, CAS: 467468-40-2) as new psychoactive substance (NPS): Prediction of absorption, distribution, metabolism and excretion parameters using integrated in silico approach for clinical and forensic purposes.

Threo-4-methylmethylphenidate (4-Mmph, 4-MeTMP, CAS: 467468-40-2) is a stimulant that could be recognized as a new psychoactive substance. It is a derivative of methylphenidate, which is a medicament for treating attention deficit hyperactivity disorder. This study aims to use integrated in silico methods to predict the absorption, distribution, metabolism, and excretion (ADME) of 4-Mmph for the first time. The absorption potential of 4-Mmph indicates gastrointestinal absorption and potential brain penetration, however that it is neither a substrate nor an inhibitor of P-glycoprotein. The pred

Diffuse Gastric Uptake on 18 F-FDG PET/CT Caused by Involvement of Mastocytosis in a Patient With Systemic Mastocytosis.

A 65-year-old man with systemic mastocytosis and associated hematological neoplasm (plasma cell myeloma, SM-AHN) received an 18 F-FDG PET/CT for staging. In addition to the typical bone marrow and splenic uptake, PET/CT also showed diffuse 18 F-FDG uptake in the gastric wall. It suggested the involvement of myeloma or mastocytosis. Subsequent histopathologic analysis of gastric biopsy specimens through endoscopy confirmed the diagnosis of mastocytosis involvement.

Obstructive Hydrocephalus Caused by Tumefactive Perivascular Spaces: A Case Report.

BACKGROUND Perivascular spaces (PS), often called Virchow-Robin spaces, are fluid-filled regions surrounding the brain's blood vessels, typically benign and detected incidentally on MRI scans. However, their enlargement, termed tumefactive PS, is rare and poses significant diagnostic challenges, potentially mimicking severe intracranial conditions. Accurate identification is critical, as these expanded spaces can lead to serious clinical outcomes like hydrocephalus or brain herniation. Misinterpretation of imaging can result in incorrect diagnoses and unwarranted procedures. This case report h

Left-Sided Perforated Appendicitis in an Elderly Man with Intestinal Malrotation: A Case Report.

BACKGROUND Left-sided acute appendicitis is rare and usually reflects congenital anomalies that misplace the cecum and appendix. Although type I intestinal malrotation (non-rotation) seldom remains undiagnosed into late adulthood, it can complicate diagnosis in older patients, whose comorbidities suggest more common causes of left lower-quadrant pain. CASE REPORT A 67-year-old man with hypertension, mixed dyslipidemia, hyperuricemia, and chronic kidney disease had 48 hours of migrating colicky abdominal pain, localizing to the left flank and iliac fossa, with fever and leukocytosis.
