BACKGROUND: Truncating titin (TTN) mutations, especially in A-band region, represent the most common cause of dilated cardiomyopathy (DCM). Clinical interpretation of these variants can be challenging, as these variants are also present in reference populations. We carried out systematic analyses of TTN truncating variants (TTNtv) in publicly available reference populations, including, for the first time, data from Exome Aggregation Consortium (ExAC). The goal was to establish more accurate estimate of prevalence of different TTNtv to allow better clinical interpretation of these findings.

The development of large genetic biobanks with deep phenotyping and whole-exome or whole-genome sequencing is enabling an ever-deeper understanding of human evolution. Here, we review recent developments in two main research directions: (1) using the increasing sample sizes to infer extremely strong evolutionary constraints and (2) leveraging biobanks' extensive phenotyping to relate the effects of natural selection on variants to their impacts on traits. In particular, there has been recent progress in gene-specific estimates of the strength of selection acting against loss-of-function mutati…

BACKGROUND: Keratitis-ichthyosis-deafness (KID) syndrome is a rare disorder characterized by progressive vascularizing keratitis, ichthyosiform erythrokeratoderma, and neurosensory hearing loss. It is caused by missense mutations in the GJB2 gene. Its known association with Dandy-Walker malformation (DWM), a developmental anomaly of the posterior cranial fossa, has only been described once antenatally.
CASE PRESENTATION: We report a case of a fetus with a DWM seen in neurosonography and fetal MRI. A whole exome sequencing revealed a heterozygous D50N mutation of the GJB2 gene, diagnosing KID …

GM1 gangliosidosis is an autosomal recessive lysosomal storage disorder caused by pathogenic GLB1 variants that impair β-galactosidase activity, resulting in GM1 ganglioside accumulation. The infantile (type I) form is the most severe. We describe the case of a one-year-old girl born to consanguineous parents who presented with developmental regression, hypotonia, coarse facial features, hepatosplenomegaly, macular cherry-red spots, Mongolian spots, and sensorineural hearing loss. Whole-exome sequencing revealed a novel homozygous GLB1 variant, NM_000404.4:c.1525T>A (p.Trp509Arg), absent from …

OBJECTIVE: This is a case report of a COL4A1 gene mutation which was confirmed by further genetic testing following anomalies observed in prenatal ultrasound and fetal brain magnetic resonance imaging (MRI).
CASE REPORTS: The ultrasound examination of the patient revealed a mass in fetal left intracranial cavity. Repeated subsequent MRI detected an evolving mass in the left frontal parietal lobe. All tests such as karyotyping, chromosomal microarray analysis (CMA), and PCR-cytomegalovirus detection of amniotic fluid, returned negative results. However, the whole exome sequencing (WES) identif…

X-linked chondrodysplasia punctata 2 (CDPX2) is a genetic disorder characterized by skeletal, cutaneous, and ophthalmic abnormalities. In this study, we present with a fetal case of CDPX2 which was identified in the first trimester by ultrasound and was confirmed by molecular diagnosis. The 12-week ultrasound revealed that nearly the entire thorax exhibited irregular rib-like stippling in the four-chamber view with postaxial polydactyly in the right hand. A follow-up ultrasound at 16+1 weeks demonstrated persistent findings along with evidence of a narrow thorax and shortened limbs. Trio exome…

KAT6B mutations are responsible for Say-Barber-Biesecker-Young-Simpson syndrome or Genitopatellar syndrome, with most mutations occurring in its exon 18. A pregnancy with normal early antenatal examination revealed the presence of hypospadias in the fetus but with no abnormal amniotic fluid volume in ultrasonography at 29th+ 4d weeks' gestation. After amniocentesis, the trios' whole exome sequencing was performed and a novel frameshift mutation (KAT6B: exon10: c.2153_2159del, p. R718Lfs*3) was identified for her fetus, then verified by the parents as a de novo mutation. Following this couple's…

A case of a 3-year-old asymptomatic male patient, who previously lost two brothers and was found to have a homozygote ryanodine receptor 2 gene mutation, is presented. The patient's initial electrocardiogram showed right bundle branch block, first-degree AV block, and left anterior hemiblock (trifascicular block), and echocardiographic examination revealed increased trabeculation in the left ventricle. Since the patient's mother and father were consanguineous, a homozygous missense mutation was detected in the ryanodine receptor type 2 gene within the scope of clinical exome sequencing in the …

Hereditary antithrombin (AT) deficiency due to mutations in SERPINC1 is known to be the most severe form of thrombophilia. We report three members in a family with hereditary AT deficiency with a novel mutation in exon 2 of SERPINC1, that is c.119 G>A (p.Cys40Tyr). Two brothers presented with acute pulmonary thromboembolism (PTE) at 18 and 21 years of age, whereas their 58-year-old father did not have any thrombotic episode till date. The in-silico prediction of the variant was found to be highly damaging by PolyPhen-2, SIFT and MutationTaster. Clinical exome sequencing did not show any strong…

Nablus mask-like facial syndrome (NMLFS) is a rare condition characterized by unique facial features, initially described in a 4-year-old boy from Nablus, Palestine. These features include expressionless facial appearance, tight facial skin, blepharophimosis, sparse eyebrows, and a flat nose. Genetic studies have identified a deletion of 8q22.1 as the cause of the syndrome, however while 26 patients have been reported with the deletion, only 13 displayed the characteristic facial features. Here we report on a 35-year-old male with 8q21.3-q22.1 deletion identified by whole exome sequencing and …

Optical Genome Mapping (OGM) technology has garnered growing interest for the identification of chromosomal structural variations (SVs), particularly complex ones that are implicated in genetic diseases in humans. In this study, we performed genetic diagnostics on a neonatal patient who presented with feeding difficulties, hypotonia, and an atrial septal defect. We utilized a combination of trio-whole exome sequencing and OGM for our analysis. The results revealed an unbalanced translocation between maternal chromosomes 4 and 6 in the proband, ogm[GRch38]t(4:6)(q35.2;q25.3), resulting in a 2.8…

A toddler presented with failure to thrive and dysmorphic features since birth. On examination, she was found to have a cleft lip, syndactyly, hypopigmented patchy skin lesions and patchy alopecia. The baseline haematological evaluation was normal. Given the syndromic features, whole exome sequencing was performed and revealed a heterozygous pathogenic variant in exon 8 of the PORCN gene, associated with focal dermal hypoplasia. Despite adequate nutrition, no significant improvement was observed in height and weight. Further evaluation revealed growth hormone deficiency and the patient was ini…

Limb abnormalities are the second most frequent birth defects seen in infants, after congenital heart disease. Over the past 150 years, more than 50 classifications for limb malformations based on morphology and osseous anatomy have been presented. The goal of the current study is to investigate the genetic basis of congenital limb abnormalities in the Pakistani population. Eight families, presenting different forms of limb anomalies, including syndactyly, polydactyly, synpolydactyly, and ectrodactyly in an autosomal dominant manner, were genetically and clinically investigated. Whole exome se…

Early neonatal seizures have myriad causes and variable prognoses. While acute symptomatic seizures are the most common events, a significant number of cases have a genetic background for such seizures, and a timely diagnosis can help in appropriate management and prognostication. We present a case of a neonate referred to our center with multi-focal clonic seizure starting from the first day of life. Routine metabolic, radiological, and electrographic studies failed to unravel the cause, necessitating whole exome sequencing (WES), which revealed a homozygous deletion of the SLC13A5 gene on ch…
