A 26- year-old woman who had a left head tilt since childhood was undergoing left inferior rectus resection to correct her left inferior rectus paresis. During the surgery, when the inferior rectus was isolated and engaged with an eye muscle hook, the muscle tore (snapped) completely into two pieces, 8 mm posterior to insertion. Fortunately, we were able to find the proximal portion of the muscle and, after a 3 mm resection, of the distal yet attached 8 mm portion, the proximal and distal portions were sutured together with a non-absorbable suture. After nine months followup there was signific

In this chapter, the authors use the computation, anatomy, and physiology (CAP) principles to consider the impact of common clinical problems on action. They focus on 3 major syndromes: paresis, apraxia, and ataxia. They also review mechanisms that could account for spontaneous recovery, using what is known about the best-studied clinical dysfunction--paresis--and also ataxia. Together, this and the previous chapter lay the groundwork for the third chapter in this series, which reviews the relevant rehabilitative interventions.

Dystonia is a syndrome of involuntary, repetitive (or sustained) muscle contractions of opposing muscles, which may result in torsions and abnormal postures. Tardive dystonia is a form of the disorder that starts after longer term use of dopamine antagonists. It occurs in approximately 3% of patients receiving ongoing antipsychotic treatment and is often difficult to reverse. Dystonia can also be induced by compounds other than antipsychotics, such as antidepressants, levodopa, carbamazepine, dextroamphetamine, and diphenylhydantoin. In these cases, it is transient, generally disappearing afte

As long as motor disorders are controlled by DOPAtherapy, exercise programs and rehabilitation would not appear to be essential for patients suffering from Parkinson's disease. Such measures do become necessary however when secondary occurrence of motor decline develops. Physical medicine and rehabilitation have not been really involved in Parkinson's disease and few articles have assessed the value of these programs. In fact, controlled randomized studies have faced two kinds of methodological difficulties, those due to rehabilitation practices, and those due to Parkinson's disease specificit

Children with excessive femoral anteversion frequently walk with abnormal internal rotation of the hip. The authors hypothesized that excessive anteversion decreases the abduction moment arm of the gluteus medius and that this moment arm is restored with internal rotation; hence internal rotation may be a compensatory mechanism to preserve abduction capacity. To test this hypothesis a three-dimensional computer model of an adult lower limb was developed to determine how changes in femoral anteversion angle, neck-shaft angle, and hip internal rotation angle affect the abduction moment arm of th

An eleven-months-old boy with motor developmental delay, muscle weakness, hypotonia and normal tendon reflexes was diagnosed as having infantile polymyositis, histologically with a vasculitis component. Corticosteroid treatment was successful. At the age of 5 years he showed a normal motor pattern.

As we have earlier shown, sudden extreme changes in disease severity are a characteristic feature of Parkinson's disease. These variations take two main forms, those natural to the disease and those occurring as a result of treatment. Treatment-related response variations can sometimes be attributed to the known pharmacokinetic properties of anti-parkinsonian drugs and, in particular, levodopa. These responses are time-dependent. In contrast, other types of response variation may be unpredictable in time, and are at present rarely amenable to treatment. Identification of the cause of variation

If the heterophoria is the cause of a fixation disparity, then certain conditions can be experimentally observed. An experiment was performed and the conditions previously set forth as indicative of a cause-effect relationship between the heterophoria and fixation disparity were observed.
