The role of bone marrow aspirate cells in the management of atrophic mandibular fractures by mini-invasive surgical approach: Single-institution experience.

PURPOSE: The treatment of fractures involves addressing the biology of fracture repair and the mechanical stability of fracture fixation. Traditionally it has included the addition of bone graft to enhance healing. New advances in the understanding of the cellular and molecular mechanisms of fracture repair have led to the use of growth factors to accelerate bone healing. This study aimed to assess the advantages of autologous stem cell use for atrophic mandibular fracture treatment in comparison to standard technique. MATERIALS AND METHODS: A total of 35 patients (14 male and 21 female) were

An Outcome Analysis of Pediatric Diaphyseal Fractures Treated Surgically With the Titanium Elastic Nailing System.

Introduction Pediatric fractures account for one-fourth of all pediatric injuries. Stabilizing the fracture, regulating the length and alignment, encouraging bone healing, and minimizing morbidity and problems for the child and family are the objectives of treatment for diaphyseal fractures of long bones in children. Our goal is to investigate how pediatric diaphyseal long bone fractures are treated with a titanium elastic nailing system (TENS). Methods A prospective interventional study was conducted on 24 children who had displaced diaphyseal fractures of major long bones, involving 31 diaph

Ultrasound-guided supraclavicular block versus Bier block for emergency reduction of upper limb injuries: a protocol for an open-label, non-inferiority, randomised controlled trial.

BACKGROUND: Closed reduction of upper limb fractures and/or dislocations are common in the emergency department (ED), which involves physically re-aligning the injured part prior to immobilisation. As this is painful, numerous techniques are available to provide regional anaesthesia to ensure patient tolerance. A Bier block (BB) is typically performed as part of routine care. An alternative technique gaining interest in the ED is ultrasound-guided supraclavicular block (UGSCB), which involves injecting local anaesthetic around the brachial plexus at the supraclavicular fossa under ultrasound g

Distal Femoral Nail as a Cost-Effective Alternative to Tibiotalocalcaneal Nail for Ankle Fracture Non-Union in an Elderly Patient: A Case Report.

INTRODUCTION: Ankle fracture non-union in elderly patients is challenging due to osteoporotic bone, comorbidities, and risks associated with open reduction and internal fixation (ORIF). Tibiotalocalcaneal (TTC) nail fixation enables early weight-bearing but is costly and not widely available in resource-limited settings. CASE REPORT: We present an 83-year-old male with a painful, deformed left ankle and non-union of a bimalleolar fracture treated conservatively. Comorbidities included diabetes mellitus and hypertension. Due to fragile skin and high ORIF-associated risks, a distal femoral nail

Tension Band Wiring in the Treatment of Extensor Carpi Radialis Longus Avulsion Fractures at the Base of the Second Metacarpal: A Case Report and Literature Review.

An avulsion fracture of the second metacarpal is a rare injury often resulting from resisted wrist hyperflexion, and there is no consensus on the optimal treatment. A review of the literature reveals 20 articles documenting 25 cases of similar injuries. Of these, nine cases were initially managed conservatively, while 16 were treated surgically. Among the nine conservative cases, five (55.6%) required late surgical intervention due to unsuccessful initial treatment. In contrast, none of the 16 surgically treated cases reported poor clinical outcomes. This case involves a 23-year-old male with

Comparative Analysis of Surgical Approaches in the Treatment of Proximal Humerus Fractures: A Retrospective Study.

BACKGROUND: "Proximal humerus fractures [PHFs]" are common orthopedic injuries, often requiring surgical intervention for optimal outcomes. However, the choice of surgical approach remains controversial, with varying outcomes reported in the literature. METHODS: A retrospective research was overseen among subjects who had surgery for PHFs. Three surgical approaches were compared: "Open Reduction Internal Fixation (ORIF)", "Hemiarthroplasty [HA]", and "Reverse Shoulder Arthroplasty [RSA]". Demographic data, fracture characteristics, surgical details, and postoperative outcomes were analyzed.

[Traumatic Right Atrial Rupture and Right Ventricular Papillary Muscle Rupture].

A 18-year-old man was driving at 100 km/h on a motorbike and collided with a utility pole. He was taken to our hospital in a state of shock due to an unstable pelvic fracture and cardiac tamponade. Pericardial drainage was carried out, but intrapericardial hemorrhage persisted and the patient underwent emergency thoracotomy and suture of right atrial injury. After hemodynamic condition improved, the patient was transferred to a tertiary care facility. For the pelvic fracture, open reduction and fixation was performed on the 6th day after injury. During recovery, moderate mitral valve regurgita

An Unusual Terrible Triad Variant Associated with an Essex-Lopresti Injury.

Essex-Lopresti injuries and terrible triad injuries of the elbow are rare injuries that typically result from high-energy trauma such as falling from a height or a motor vehicle collision. However, the combination of an Essex-Lopresti injury and terrible triad injury is unique and poses a significant challenge for treatment as these injuries are independently associated with poor functional outcomes if they are not acutely diagnosed. We describe a case of a 19-year-old who presented with an unusual variant of a terrible triad injury associated with an Essex-Lopresti injury. The patient had a d

Traumatic bilateral testicular dislocation associated with an anterior posterior compression fracture of the pelvis: a case report.

Traumatic dislocation of a single testicle is an exceedingly rare event and bilateral dislocation is even less common. Traumatic testicular dislocation occurs by the mechanism of direct pressure exerted on the scrotum. Motorcycle collisions are the most frequent causative mechanism, usually related to deceleration straddle injuries. Since traumatic testicular dislocation is associated with other severe injuries, it is usually a late finding, leading to delayed diagnosis and treatment. This report describes a rare case of a patient involved in a motorcycle crash who sustained bilateral testicul

Management of nasal bone fractures.

This review discusses treatment options for nasal fracture, a common condition due mainly to road accidents, sport injuries, and physical confrontations. Being frequently associated with multiple trauma, many broken noses are not promptly diagnosed and treated, leading to secondary nasal deformities and chronic obstructions. A description of nasal anatomy is followed by considerations on the pathogenesis of nasal fracture and its clinical assessment. Each patient's history must be recorded (cause of trauma, previous facial injuries, prior nasal deformity, or obstruction) and careful physical e

Does Insurance Status Affect Access to Care Among Ankle Fracture Patients? An Institutional Retrospective Study.

Delayed access to care for patients with ankle fractures may increase risk of complications, particularly if surgical management is warranted. Medicaid is a state and federal insurance program in place for those with low income, which has previously been associated with delayed access to care among patients with ACL tears and total hip arthroplasties. The purpose of this study is to assess whether patient insurance status affects access to care for ankle fracture patients, using data from a single institution. A retrospective cohort study (N = 311 patients)was performed on individuals that und

Comparing the Outcomes of Intramedullary Nail Fixation for Subtrochanteric Femur Fractures Performed by Trauma Fellowship-Trained and Non-Trauma-Trained Surgeons.

INTRODUCTION: Reduction of subtrochanteric femur fractures can be challenging due to the strong deforming forces at the hip. Increased experience with trauma fellowship training may potentially result in better outcomes, but subspecialty-trained surgeons may not be available for each case. PURPOSE: The goal of this study was to compare postoperative complication rates, radiographic outcomes, and union rates among trauma fellowship-trained and non-trauma-trained orthopaedists following intramedullary nail fixation (IMN) of subtrochanteric femur fractures. METHODS: All patients with a subtroch

A six months old neglected anterior shoulder dislocation managed by closed reduction and Latarjet procedure.

A neglected shoulder dislocation is a rarer entity and only few cases are reported in the literature. An anterior dislocation of the shoulder is rarely missed as patients present with limb in abduction and external rotation, an attitude very familiar to orthopaedic surgeon. Occasionally such cases are missed when they present with fracture of proximal humerus or when they receive treatment from unqualified practitioners who commonly practise in rural areas. Owing to very few reports there is paucity of literature and no standard treatment protocol exists for neglected anterior dislocation of t

Late treatment of a dorsal transscaphoid, transtriquetral perilunate wrist dislocation with avascular changes of the lunate.

The outcome of delayed treatment of an unreduced transscaphoid, transtriquetral, perilunate fracture dislocation of the carpus is unpredictable. Long-term follow-up observations in a 22-year-old man treated three months postinjury showed changes in the lunate consistent with avascular necrosis at the time of open reduction and internal fixation. Early resolution of this was evident by nine months, and complete resolution was seen at the follow-up examination (four years and two months). Despite delay in treatment, this patient had full, pain-free wrist motion. Consequently, avascular changes o

Surgical Reduction and Fixation of Tibial Spine Fractures in Children: Arthroscopic Suture Fixation.

The pediatric tibial spine fracture can be considered the equivalent of the pediatric anterior cruciate ligament (ACL) rupture, especially considering that failure to manage these injuries appropriately could result in substantial knee pathology and long-term functional failure. Arthroscopy-assisted internal fixation of the tibial spine fracture minimizes the patient's hospital stay compared with that associated with an open arthrotomy approach, but the postoperative rehabilitation may be paramount to a successful outcome. Compared with conservative management, surgical reduction and fixation

[Medial Hoffa fracture: a case report].

INTRODUCTION: knee femoral condyle coronal fractures or Hoffa fractures are among the rarest fractures of the pelvic limb, being even more rare those of the medial condyle. The success in the management of these patients lies in the timely diagnosis and the consequent anatomical reduction of the femoral articular surface. CLINICAL CASE: the objective of this article is to present a patient with right medial Hoffa fracture, registered as a single case in the archives of our hospital. The patient was referred to us on November 2011, with a 10-day course after suffering a fall from horse ridding

Kyphoplasty in osteoporotic vertebral compression fractures--guidelines and technical considerations.

Osteoporotic vertebral compression fractures are a menace to the elderly generation causing diminished quality of life due to pain and deformity. At first, conservative treatment still is the method of choice. In case of resulting deformity, sintering and persistent pain vertebral cement augmentation techniques today are widely used. Open correction of resulting deformity by different types of osteotomies addresses sagittal balance, but has comparably high morbidity. Besides conventional vertebral cement augmentation techniques balloon kyphoplasty has become a popular tool to address painful t

Sternal fractures associated with spinal injury.

Twenty-eight cases of sternal fractures and/or dislocations were reviewed for the presence of associated spinal injury. Patient records and radiographs were studied. Sixteen cases had inadequate radiographs to rule out spine fracture. Three cases had known direct injuries to the sternum. Of the remaining nine patients, eight had spine fractures. All of the spine fractures were consistent with a flexion injury mechanism. Three patients had spine fractures at the thoracic level and four at the lumbar level. One patient had lower cervical and thoracic injuries. In the patients with spine injury,
