Alveolar Distraction.

Alveolar distraction osteogenesis (ADO) has been used for the reconstruction of atrophic alveolus for decades. The advantage of this technique is that it augments the bone and soft tissues together, creating a better alveolar platform for subsequent surgeries and dental rehabilitation. It is especially useful in patients with large and/or complex alveolar clefts for which approximating the alveolar segments reduces the size of the bony cleft and associated fistula. Displacement of the transported segment is the most frequently encountered complication of ADO but can be managed by constructing …

Treatment of midfacial hypoplasia in syndromic and cleft lip and palate patients by means of a rigid external distractor (RED).

INTRODUCTION: Distraction Osteogenesis (DO) became an alternative for the treatment of severe craniofacial skeletal dysplasias. The rigid external distraction device (RED) is successfully used to advance the maxilla and all the maxillary-orbital-frontal complex (monobloc) in children, adolescents and adults. This approach provides predictable and stable results, and it can be applied alone or with craniofacial orthognathic surgical procedures. OBJECTIVE: In the present article, the technical aspects relevant to an adequate application of the RED will be described, including the planning, surg…

Five years experience with a new intraoral maxillary distraction device (RID).

Maxillary distraction osteogenesis is well established for the treatment of severe retromaxilla. We report our experience since 2004 of the treatment of 19 patients using a new intraoral maxillary distraction device. Maxillary advancement was successful in all patients with mean advancement of 9.6mm (range 4-17) measured at a point in lateral cephalograms. The new device limited surgical exposure and the amount of materials implanted, and improved control in every phase of the distraction. It was psychologically accepted by patients and was more comfortable than existing devices.

Metabolic bone disease in adolescents: recognition, evaluation, treatment, and prevention.

Bone is a metabolically active organ that undergoes constant remodeling. Bone is very important for maintaining homeostasis of calcium and phosphorous in the body. Disorders of bone metabolism may encompass disease processes involving underactive or overactive bone turnover. The spectrum of abnormal bone metabolism includes rickets and osteomalacia; osteopenia and osteoporosis; osteogenesis imperfecta, renal osteodystrophy; osteopetrosis and osteosclerosis; and effect of pharmacologic therapies on bone metabolism. The recognition, evaluation, treatment, and prevention of these conditions will …

Low doses of pamidronate for the treatment of osteopenia in non-ambulatory children.

In recent years, bisphosphonates, primarily intravenous (iv) pamidronate, have become very widely used in children with severe osteogenesis imperfecta (OI). This has occurred without the well-controlled clinical trials usually required before a medication becomes 'standard care'. Without placebo-controlled trials it is difficult to define precisely the risks and the benefits, but in children with very severe OI and numerous fractures the risk/benefit ratio is so clearly favorable that widespread use of pamidronate in this situation appears appropriate.

[Etiological and pathogenetic mechanisms in development of craniosynostoses in children].

Craniosynostoses belong to a heterogenous group of diseases characterized by a preterm closing of cranial sutures. The symptoms may be nonsyndromal, incomplete syndromal and syndromal. Most popular are now hereditary and genetic theories of its development. The attention is focused on the factors of fibroblast growth and receptors to these factors located on osteoblasts. Gene mutation results in alteration of protein receptor structure, increased osteoblast activity and production of great amount of osteoinducing factors, collagen, acceleration of mineralization, enhancing osteogenesis in the …

Histopathological and biochemical changes in the sutural region in craniosynostosis.

OBJECT: Histopathological observations and biochemical analysis of sutural bones in nine patients with craniosynostosis were compared with control subjects of the same age range. METHODS: Microscopic examination in craniosynostosis showed formation of an active osseous front, with higher osteoblastic activity than in controls. Biochemical analysis revealed higher calcium, phosphorus, alkaline phosphatase, phospholipids and chondroitin sulphate-A contents in sutural bones of the same patients. CONCLUSIONS: The present study systematically establishes a premature increase in osteogenesis in th…

Joint hypermobility and genetic collagen disorders: are they related?

The HDCTs constitute a heterogeneous group of rare genetically determined diseases, the best known of which are Ehlers-Danlos and Marfan syndromes and osteogenesis imperfecta. Hypermobility is a feature common to them all, but it is also a feature that is highly prevalent in the population at large. Symptomatic hypermobile subjects (whose symptoms are attributable to their hypermobility) are said to be suffering from the benign joint hypermobility syndrome, which has many features that overlap with the HDCTs. It is not yet known whether there is a variety of hypermobility (symptomatic or other…

[The human genome--chromosome 7].

The seventh chromosome achieved maximum popularity due to locus CFTR the pathological alleles of which cause one of the frequent metabolic hereditary disorders--cystic fibrosis. It is also the site which causes the development of some types of retinitis pigmentosa (RP9, RP10), osteogenesis imperfecta and dominant forms of myotonia congenita. On the long arms there is a complicated multi-segmental gene system of receptors of T-lymphocytes, TCRB, important in cell immunity and it is assumed that it is also the locus of taste perception of phenylthiocarbamide (PTC) is located there.

Children and adolescents with brittle bones--psycho-social aspects.

Osteogenesis imperfecta (OI) or brittle bones is a most unusual disease and there is a limited number of people in the whole world suffering from it. Most studies reported from this area are in the medical field and few illuminate the psycho-social effects of the disease. Three studies of children and young people with brittle bones have been carried out within the psycho-social field in Sweden. They are all based on inquiries and interviews with children and parents engaged in the Swedish National Organization for Physically Disabled Children and Adolescents.

Craniosynostosis: a new operative technique.

In the treatment of craniosynostosis several methods have been proposed to prevent craniectomies from early reclosure. New bone is formed at the edge of the craniectomy, but particularly at the outer surface of the dura. The author has developed a method to separate the edges from each other and to stop osteogenesis inside the craniectomy. The outer layer of the dura is dissected free from the inner layer folded over the edge of the groove and sutured to the outer periosteum. The operative technique and the results in 40 patients with a follow up of 17 years are described.

In vitro validation of a CRISPR/Cas9 guide RNA targeting COL1A1 gene by cleavage assay.

The CRISPR/Cas9 system has become a widely used tool for genome editing; however, editing efficiency is largely determined by the design and functional activity of the guide RNA (gRNA). Experimental validation prior to cellular applications is particularly important when targeting genes associated with genetic disorders, such as COL1A1, which is frequently mutated in Osteogenesis Imperfecta. This study performed an in vitro functional validation of a CRISPR/Cas9 gRNA designed to target exon 2 of the COL1A1 gene, using a cell-free cleavage assay as a preliminary step before cellular genome edit…

Apoptotic-mimetic nanovesicles orchestrate immune-vascular-osteogenic crosstalk for critical-sized craniofacial bone regeneration.

The management of critical-sized craniomaxillofacial bone defects remains a formidable clinical challenge due to the concomitant issues of prolonged inflammation, inadequate vascularization, and insufficient endogenous osteogenesis. Herein, a novel cell-free bone regeneration strategy is reported based on biomimetic apoptotic nanovesicles (Apo-NVs) derived from apoptotic T-lymphocytes membrane. The Apo-NVs, which mimic the "find-me" and "eat-me" signals of natural apoptotic cells, are engineered into a gelatin methacryloyl (GelMA) hydrogel for sustained localized delivery. In vitro, the Apo-NV…

Nitric oxide synthases: A delicate dance between bone regeneration and neuronal birth.

Spinal cord injury (SCI) is a devastating condition resulting from traumatic or nontraumatic injury/chronic disorder. The pathogenesis of SCI necessitates a comprehensive approach, as it involves therapeutic strategies addressing both bone (spine) and neural (spinal cord) damage. This review centers on the pivotal role of nitric oxide (NO) and its synthesizing enzymes, nitric oxide synthases (NOS), in mediating the crosstalk between osteogenesis and neurogenesis. NO's effects are context-dependent, exhibiting a delicate balance between beneficial and detrimental actions. Reduced levels of nitr…

Research Progress of Bone Grafting: A Comprehensive Review.

Bone tissue, the second most transplanted tissue after blood, is utilized in over 2.2 million bone grafts annually to address various bone-related conditions including fractures, tumors, bone infections, scoliosis, congenital defects, osteoporosis, osteoarthritis, and osteogenesis imperfecta. According to incomplete statistics, $4.3 billion was spent on bone graft materials in 2015 alone, with projections suggesting this figure may reach $66 billion by 2026. The limited availability of autogenous bone graft considered the gold standard due to their three critical biological properties: osteoco…

MiR-30d-5p Regulates Bone Remodeling and Vessel Remodeling in Osteoporosis by Targeting GRP78.

Osteoporosis is one of the major bone disorders that has a close relationship with osteogenesis and angiogenesis. MiRNA coordinates a cascade of anabolic and catabolic processes in bone homeostasis and dynamic vascularization. Here, the role of miR-30d-5p in osteoporosis and its mechanism were investigated. To explore the effect of miR-30d-5p on osteogenesis and angiogenesis, an ovariectomized (OVX) mouse model, bioinformatics analysis, and multiple in vitro assays were used. Compared to the sham group, OVX mice reduced BV/TV, Tb.Th, Tb.N, and BMD, while it increased Tb.Sp. The number of osteo…
