Pain Medicine and the Opioid Crisis: Time to Own Our Responsibility!

Pain remains a major unmet health need. Rising opioid prescribing has not reduced population pain and has directly contributed to increased morbidity and mortality. Opioid use disorder (OUD) and pain frequently coexist. Many people with OUD experience acute and chronic pain and a substantial proportion of patients prescribed opioids develop OUD. We have contributed to this problem. In this Position Paper, we defend the idea that we must urgently redefine pain medicine as a frontline discipline in opioid use disorder prevention and recovery. Pain management must be reframed as part of the solut

Dynamic CD177/CD10 ratio for infection diagnosis and mortality risk stratification in critically ill patients: a prospective cohort study.

BACKGROUND: Two crucial needs persist in delivering effective critical care: reliable differentiation of infectious vs non-infectious systemic inflammation and detection of impending sepsis before the occurrence of organ dysfunction. This prospective cohort study evaluates the clinical utility of the polymorphonuclear neutrophil (PMN) biomarker CD177/CD10 ratio to differentiate and monitor infections. METHODS: A total of 219 healthy volunteers and 145 patients were assigned to four clinically defined groups: healthy controls (HC), non-infectious inflammation (NI), non-septic infection (NS-I),

Circulating Calprotectin (cCLP) in autoimmune diseases.

BACKGROUND AND AIM: Calprotectin (CLP) is a heterodimeric complex formed by two S100 proteins (S100A8/A9), which plays a pivotal role in innate immunity. Due to its intrinsic cytotoxic and proinflammatory properties, CLP controls cell differentiation, proliferation and NETosis and has been associated with a wide range of rheumatic diseases. Our review summarizes the widespread interest in circulating CLP (cCLP) as a biomarker of neutrophil-related inflammation, in autoimmune rheumatic disease (ARD) and non-ARD. METHODS: A thorough literature review was performed using PubMed and EMBASE databa

Pressure sores. Epidemiology and current management concepts.

Pressure sores remain common, with a prevalence of 5 to 9% and more than 70% occurring in patients over 70 years of age. They are often falsely ascribed to poor nursing care, but can more usefully be regarded as a potentially preventable complication of an acute immobility illness. Prevention involves identification of patients at risk, appropriate nursing care measures and the use of special equipment. Much of the special equipment is excessively complex and not validated by clinical trial work. The airwave system, polystyrene bead bed system and Vaperm mattress have been best studied and are

Code-Based Versus AutoML Methods for Pill Recognition in Clinical Settings: Comparative Performance Study.

BACKGROUND: Visual identification and verification of medications during dispensing and administration are prone to human error, particularly in high-pressure and high-volume clinical settings. Misidentification can lead to medication errors, posing risks to patient safety and placing a burden on health care systems. Recent advances in computer vision and object detection offer promising solutions for automated solid oral dosage form (pill) recognition. However, comprehensive studies comparing code-based and no-code (automated machine learning [AutoML]) approaches for pill recognition are lack

KL-6 assisted subtyping of ARDS: from subtype-specific metabolomics to LPCAT1 as a pathogenic target.

BACKGROUND: Acute lung injury (ALI) and the resulting acute respiratory distress syndrome (ARDS) exhibit marked clinical and biological heterogeneity, hindering subtype identification, biomarker discovery, and targeted therapy development. We used untargeted metabolomics to identify serum and bronchoalveolar lavage fluid (BALF) biomarkers across ARDS subtypes and potential therapeutic targets. We introduce a KL-6–assisted refinement of the Berlin Definition to separate patients with similar PaO₂/FiO₂ ratios but differing degrees of alveolar epithelial damage. METHODS: We enrolled 166 participa

Embedding social determinants in mHealth for pediatric oncology: co-designing a patient-centred tool for febrile neutropenia in resource-limited settings.

BACKGROUND: Febrile neutropenia (FN) is a potentially life-threatening complication of chemotherapy in pediatric oncology. In resource-limited settings, timely care is often hampered by structural and social barriers. Broader social determinants of health (SDoH)—including food insecurity, housing instability, limited transport, digital exclusion, and caregiver mental health conditions—undermine caregivers’ capacity to respond to FN episodes effectively. METHODS: This study aimed to identify and operationalize key SDoH influencing FN management and integrate them into the co-design of the Salud

Hormone-induced thrombosis is mediated through non-canonical fibrin(ogen) aggregation and a novel estrogen target in zebrafish.

Venous thrombosis is a well-known complication of sex hormone therapy, with onset typically within weeks to months after initiation. Worldwide, more than 100 million pre-menopausal women use combined oral contraceptives, with tens to hundreds of thousands developing thrombosis annually, resulting in significant morbidity and mortality. Although it is known that estrogens can alter expression of coagulation factors, the pathways and mechanisms that connect the two systems, as well as the proteins involved in progression to thrombosis, are poorly understood. Identification of these mediators are

Can the risk of obstetric anal sphincter injuries (OASIs) be predicted using a risk-scoring system?

BACKGROUND: Perineal trauma involving the anal sphincter is an important complication of vaginal delivery. Prediction of anal sphincter injuries may improve the prevention of anal sphincter injuries. Our aim was to construct a risk scoring model to assist in both prediction and prevention of Obstetric Anal Sphincter Injuries (OASIs). We carried out an analysis of factors involved with OASIs, and tested the constructed model on new patient data. METHODS: Data on all vaginal deliveries over a 5 year period (2004-2008) was obtained from the electronic maternity record system of one institution i
