Update in the Treatment of Chronic Pain within Pediatric Patients.

Pediatric chronic pain is a challenging entity to evaluate and treat as it encompasses a wide variety of presentations often with overlapping psychosocial implications. Chronic pain may have significant effects upon a child's involvement in academic, athletic, and social participation. If unrecognized, it may have deleterious effects upon family interactions and stability. The treatment of pediatric chronic pain is focused on not only providing analgesia, but also on assisting the child and family with reintegrating into a more functional lifestyle. Given the complex multifactorial causes of p

Occipital nerve block is effective in craniofacial neuralgias but not in idiopathic persistent facial pain.

Occipital nerve block (ONB) has been used in several primary headache syndromes with good results. Information on its effects in facial pain is sparse. In this chart review, the efficacy of ONB using lidocaine and dexamethasone was evaluated in 20 patients with craniofacial pain syndromes comprising 8 patients with trigeminal neuralgia, 6 with trigeminal neuropathic pain, 5 with persistent idiopathic facial pain and 1 with occipital neuralgia. Response was defined as an at least 50% reduction of original pain. Mean response rate was 55% with greatest efficacy in trigeminal (75%) and occipital

Long-lasting orofacial pain--a study of 109 consecutive patients referred to a pain group.

The aim was to register and evaluate the orofacial pain condition, with special focus on the equalities/inequalities between the sexes, in consecutive patients referred to the pain group at the Centre for Oral Health Sciences in Malmö, Sweden. The 109 patients, 85 (78%) women and 24 (22%) men, were referred to the pain group during the years 1988 to 1993 and were clinically examined using a standardised procedure. Pain duration of one year or more was experienced by 83% of the patients. The pain onset was related to dental care or toothache by 56% of the patients. Temporomandibular disorders w

Outcomes for surgical management of orchalgia in patients with identifiable intrascrotal lesions.

OBJECTIVE: The outcome of surgery for relief of orchalgia in patients with identifiable intrascrotal pathology is not well defined. We evaluated the success of commonly performed surgical procedure indicated for pain relief in patients with specific intrascrotal lesions. METHODS: Surgical cases performed for relief of painful scrotal pathology were reviewed, including ligation of internal spermatic vein, hydrocelectomy, spermatocelectomy, and orchiopexy for suspected intermittent torsion. Relief of pain as reported to the physician and time for return to full activity were determined. Pain re

Onychoplasty with 1064-nm Laser: Matrixectomy for Treatment of Ingrown Toenails.

Onychocryptosis is a common pathology treated by podiatry medical services, and in a considerable percentage, surgical procedures are required to achieve a solution. There are multiple surgical approaches for ingrown toenails, both incisional procedures and nonincisional procedures, such as chemical matrixectomies and physical matrixectomies using carbon dioxide laser. This study presents a surgical procedure for onychocryptosis using a physical matrixectomy with a 1064-nm laser applied by means of a 400-μm optical fiber and surgical removal of the posterior cauterized tissue to achieve healin

Emergency management of sigmoid volvulus--institutional experience over four years.

Sigmoid volvulus is a serious surgical emergency and a common cause of large bowel obstruction in India. Patients present with abdominal pain, distension and obstipation and abdominal skiagram usually reveals the characteristic omega sign. Non-operative detorsion with early elective sigmoidectomy is the procedure of choice where gut viability is not in doubt and features of peritonitis are absent. The objective of this study was to demonstrate the most suitable procedure for management of patients with sigmoid volvulus needing emergency surgery. Results revealed a high incidence of burst abdom

Documented preoperative depression is not significantly associated with pain outcomes following microvascular decompression for Type 1 trigeminal neuralgia.

BACKGROUND: Depression is a common comorbidity among patients with trigeminal neuralgia (TN). Although microvascular decompression (MVD) provides durable pain relief for medically refractory TN, prior studies have associated depression with poorer outcomes in heterogeneous TN cohorts. Whether this relationship exists specifically in Type 1 TN (TN1) remains unclear. This study evaluated whether documented preoperative depression was associated with early or long-term pain outcomes following MVD for TN1. METHODS: We retrospectively reviewed MVD procedures performed for classical TN1 at a single

Updated Analysis of Decline of 16.8% in Utilization of Interventional Pain Management Techniques Among Traditional (Fee-for-Service) Medicare Beneficiaries from 2019 to 2024.

BACKGROUND: In recent years, rising costs associated with managing spinal pain and other musculoskeletal disorders have been well documented. Prior to the COVID-19 pandemic, the use of interventional techniques to manage spinal pain and other musculoskeletal disorders had steadily increased. However, the pandemic disrupted chronic pain management, including interventional procedures and opioid use, reflecting a broader reduction in healthcare services. OBJECTIVES: To provide an updated assessment of interventional technique utilization for chronic pain management in the U.S. Medicare populati

Radiation safety and knowledge: an international survey of 708 interventional pain physicians.

INTRODUCTION: Interventional pain procedures have increased in complexity, often requiring longer radiation exposure times and subsequently higher doses. The practicing physician requires an in-depth knowledge and evidence-based knowledge of radiation safety to limit the health risks to themselves, patients and healthcare staff. The objective of this study was to examine current radiation safety practices and knowledge among interventional pain physicians and compare them to evidence-based recommendations. MATERIALS AND METHODS: A 49-question survey was developed based on an extensive review

Treatment of recalcitrant lateral epicondylitis with suture anchor repair.

BACKGROUND: Chronic lateral epicondylitis has been treated with various surgical procedures with varying and often less-than-satisfactory outcomes. HYPOTHESIS: The use of suture anchor repair of the extensor carpi radialis brevis to the lateral epicondyle in the treatment of chronic lateral epicondylitis will result in a more anatomical repair, with resultant improvements in strength, pain, and return to previous sport. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: A total of 20 patients (22 elbows) were treated surgically and were observed for a minimum of 2 years. All patients

A prospective, randomized trial comparing endoscopic and surgical therapy for chronic pancreatitis.

BACKGROUND AND STUDY AIMS: Invasive treatment for abdominal pain due to chronic pancreatitis may be either surgical or endoscopic, particularly in cases of ductal obstruction. To date, the data published on the effectiveness of these two forms of therapy have been mostly retrospective, and there have been no randomized studies. A prospective, randomized study comparing surgery with endoscopy in patients with painful obstructive chronic pancreatitis was therefore conducted. PATIENTS AND METHODS: Consecutive patients with pancreatic duct obstruction and pain were invited to participate in a ran

Acute and chronic peroneal tendon dislocations.

Peroneal tendon dislocation is an infrequent sports injury and can be difficult to diagnose. Posterior fibula tenderness and pain with eversion are useful signs to discriminate this from an ankle sprain. In the acute setting, a direct repair provides good results but may need to be augmented with additional soft tissue or bony restraints. For chronic injuries there are several methods of reconstruction, all with acceptable outcomes, although bone block procedures have the highest rate of secondary procedures.

AAEM case report 32: nerve injury associated with hip arthroplasty.

Hip Arthroplasty to alleviate pain related to arthritic degeneration has become one of the most common orthopedic procedures performed. As the elderly population expands, the number of such procedures can be expected to continue to increase. An electrodiagnostic evaluation can aid in localization, help identify the mechanism of injury, and be used as a tool to identify the nature and severity of the nerve pathology. Electrodiagnosis can also be used to generate a prognosis for recovery from nerve damage following hip surgery.

Acute jejunogastric intussusception: a rare cause of abdominal pain.

We report the case of a 61-year-old man with a history of a Bilroth II procedure who presented to the emergency department after 12 hours of severe epigastric pain and one episode of coffee-ground emesis. Computed tomography and esophagogastroduodenoscopy revealed intussuscepted jejunum through a gastrojejunostomy that required emergency operative reduction. This case illustrates the rare complication of acute jejunogastric intussusception more than 20 years after a Bilroth II procedure.

Arthroscopic treatment of impingement of the shoulder.

Arthroscopic subacromial decompression is an alternative to open anterior acromioplasty in patients who have advanced stage II and selected stage III impingement syndromes. This procedure can be performed in an outpatient setting and postoperative morbidity is minimal; however, the procedure is technically demanding. Because the deltoid is not detached, immediate full range of active motion is possible, allowing an early return to activities of daily living. Substantial pain relief can be anticipated in a large majority of patients.

Pyloric preservation with the Whipple procedure.

The pylorus-preserving pancreatoduodenectomy simplifies resection, allows a satisfactory postoperative weight gain, prevents postgastrectomy symptoms, is followed by a low rate of jejunal ulceration, and can be performed with an extremely low postoperative mortality rate, providing that the pancreatic and biliary anastomoses are constructed so that no leakage occurs. Preliminary data indicate a satisfactory survival rate when this procedure is used for periampullary cancer, and reasonable relief of pain is achieved when the procedure is used in chronic pancreatitis.
