Screening for chronic mouth breathing, snoring, and obstructive sleep apnea risks among Palestinian adults in the Gaza Strip during wartime conditions: a cross-sectional study.

OBJECTIVE: Obstructive sleep apnea is a sleep-related breathing disorder characterized by complete/partial upper airway obstruction. The majority of patients with OSA remain undiagnosed. A screening tool is necessary to stratify patients based on their clinical symptoms, physical examination findings, and risk factors. Therefore, this study was conducted using the modified STOP-Bang questionnaire to investigate the risk of OSA in addition to chronic mouth breathing among adults in the Gaza Strip in Palestine who are living in very difficult circumstances as a result of the war. METHODS: 1000

Efficacy of CPAP Therapy on Liver Steatosis and Insulin Resistance in Fatty Liver Patients With Obstructive Sleep Apnea: A 4-Year Follow-Up Cohort Study.

Metabolic dysfunction-associated steatotic liver disease (MASLD) and obstructive sleep apnea (OSA) are comorbid conditions that synergistically increase cardiovascular risk through systemic inflammation and insulin resistance. While continuous positive airway pressure (CPAP) is the standard treatment for OSA, its efficacy in improving hepatic steatosis and insulin resistance in patients with concurrent MASLD remains unclear. This study aims to characterize metabolic profiles in patients with MASLD stratified by OSA severity and evaluate the longitudinal effects of CPAP therapy on hepatic steat

Prevalence and clinical correlates of positive aldosterone-to-renin ratio screening in obese hypertensive patients with obstructive sleep apnea: a Southeast Asian cross-sectional study.

Although routine screening for primary aldosteronism (PA) is increasingly recognized among patients with obstructive sleep apnea (OSA), data regarding aldosterone-to-renin ratio (ARR) screening positivity among Southeast Asian OSA populations remain limited, particularly in resource-limited settings. This study aimed to determine the prevalence and clinical correlates of ARR positivity screening among obese hypertensive OSA patients. In this cross-sectional study, obese hypertensive adults with confirmed OSA were recruited. Blood samples were analyzed for plasma aldosterone concentration (PAC)

Obstructive Sleep Apnea and the Risk of Cardiovascular Disease and Stroke Among Patients With Cancer: Analyses From a Multicenter Electronic Health Care Records-Based Database.

BACKGROUND: Obstructive sleep apnea (OSA) and cancer are independently associated with increased cardiovascular disease (CVD) risk. However, it is unclear whether OSA confers additional CVD risk among individuals with cancer. METHODS: A retrospective cohort study was conducted using the TriNetX database, including adults ≥40 years diagnosed with cancer between 2012 and 2023. Individuals with and without OSA were propensity score matched on demographics and comorbidities. The primary outcome was a composite of heart failure, atrial fibrillation/flutter, myocardial infarction, coronary revascul

A mobile-based daily check-in approach to monitor patient reported outcomes in obstructive sleep apnea: results from a 6-month home-based observation study.

Integrated alongside home-based passive sleep monitoring, high-frequency assessment of patient reported outcomes (PROs) could enhance both research and clinical care surrounding obstructive sleep apnea (OSA). However, traditional approaches to capturing OSA-related PROs are poorly suited for this purpose. Hence, we investigated the feasibility of a smartphone-based check-in approach to monitor daily PROs (e.g. mental, emotional, and physical) over a 6-month period in people referred for suspected OSA. We quantified and assessed change in the proportion of participants (n = 68; 54% female) who,

Usefulness of Blood Biomarkers in Screening Patients with Obstructive Sleep Apnea: Could Albumin Indices and Uric Acid-to-HDL Ratio Be New OSAS Severity Indices?

BACKGROUND AND OBJECTIVES: Hematological parameters are increasingly being investigated as readily accessible biomarkers for the diagnosis of obstructive sleep apnea syndrome (OSAS). In our study, we aimed to investigate the relationship between OSAS and albumin indices and the uric acid-to-HDL ratio (UHR). METHODS: The demographic and laboratory data and AHI (apnea-hypopnea index) values of 613 patients who underwent polysomnography were obtained retrospectively from their files. Blood parameters such as white blood cells (WBCs), red blood cell distribution width (RDW), red blood cells (RBCs

Successful awake tracheal intubation in a morbidly obese patient with severe obstructive sleep apnea-hypopnea syndrome and a giant vocal cord polyp: A case report.

Giant vocal cord polyps pose challenges in airway management, particularly in patients with severe obstructive sleep apnea-hypopnea syndrome. We present the case of a morbidly obese man in his 20s (body mass index: 50.8 kg/m2) with severe obstructive sleep apnea-hypopnea syndrome and a giant vocal cord polyp. He successfully underwent awake tracheal intubation using video laryngoscopy and high-flow nasal oxygen, which was administered at 30 L/min. Airway anesthesia was achieved with lidocaine, including bilateral superior laryngeal nerve blocks. A 6.5-mm endotracheal tube was placed within 30

Obstructive Sleep Apnea as a Predictor of Inducible Atrial Flutter During Pulmonary Vein Isolation in Patients With Atrial Fibrillation: Clinical Significance and Follow-Up Outcomes.

BACKGROUND: Atrial fibrillation (AF) and atrial flutter (AFL) often coexist in patients and may lead to severe symptoms and complications. Despite their coexistence, prophylactic cavotricuspid isthmus (CTI) ablation has failed to reduce the incidence of recurrent AF or new onset AFL. In contrast, the presence of inducible AFL during pulmonary vein isolation (PVI) has been shown to be predictive of symptomatic AFL during follow-up. However, the potential role of obstructive sleep apnea (OSA) as a predictor of inducible AFL during PVI in patients with AF remains unclear. Therefore, this study ai

Autophagy impairment in patients with obstructive sleep apnea modulates intermittent hypoxia-induced oxidative stress and cell apoptosis via hypermethylation of the ATG5 gene promoter region.

BACKGROUND: Autophagy is a catabolic process that recycles damaged organelles and acts as a pro-survival mechanism, but little is known about autophagy dysfunction and epigenetic regulation in patients with obstructive sleep apnea (OSA). METHODS: Protein/gene expressions and DNA methylation levels of the autophagy-related genes (ATG) were examined in blood leukocytes from 64 patients with treatment-naïve OSA and 24 subjects with primary snoring (PS). RESULTS: LC3B protein expression of blood monocytes, and ATG5 protein expression of blood neutrophils were decreased in OSA patients versus PS

Association of Hypoglossal Nerve Stimulation With Improvements in Long-term, Patient-Reported Outcomes and Comparison With Positive Airway Pressure for Patients With Obstructive Sleep Apnea.

IMPORTANCE: Hypoglossal nerve stimulation (HNS) and positive airway pressure (PAP) have been shown to improve patient-reported outcomes (PROs) in obstructive sleep apnea (OSA). However, to our knowledge, there are no data that compare change in PROs between HNS and PAP or that indicate whether HNS improves comorbid insomnia or depression in the long term. OBJECTIVES: To determine whether HNS is associated with improvements in patient-reported sleepiness, insomnia, and depression in the long term and to compare the respective associations of HNS and PAP with improved PROs. MEntA Program Based on Motivational Interview to Improve Adherence to Treatment of Obstructive Sleep Apnea With Continuous Positive Airway Pressure (CPAP): A Randomized Controlled Trial.

INTRODUCTION: The first-line treatment for obstructive sleep apnoea is (OSA) continuous positive airway pressure (CPAP) therapy, which achieves a high level of efficacy with continuous use. However, daily application of this therapy requires a motivated patient profile, as lack of adherence is the main problem with this therapy. The aim of this study was to determine whether an educational and training program based on motivational interviewing and proper feedback, improved adherence. METHODS: A randomized, controlled, single-center trial design was performed. The interventions were standard

Paradoxical worsening of hypoxemia in a patient treated by noninvasive positive pressure ventilation for obesity hypoventilation syndrome with concomitant obstructive sleep apnea: a case report.

BACKGROUND: Noninvasive positive pressure ventilation is frequently prescribed to obese patients with obstructive sleep apnea syndrome and obesity hypoventilation syndrome. However, mechanical ventilation with a positive end-expiratory pressure can induce or worsen a right-to-left shunt through a patent foramen ovale associated with systemic hypoxemia. Thus, in obese patients treated with noninvasive positive pressure ventilation, a paradoxical worsening of hypoxemia may reveal the existence of a patent foramen ovale. CASE PRESENTATION: A 50-year-old African woman was referred to our sleep ce

Withdrawal of Continuous Positive Airway Pressure Therapy after Malar Advancement and Le Fort II Distraction in a Case of Apert Syndrome with Obstructive Sleep Apnea.

Apert syndrome is a congenital syndrome characterized by craniosynostosis and craniofacial dysostosis, among other features, and is reported to cause obstructive sleep apnea (OSA) because of upper airway narrowing associated with midfacial dysplasia. We recently encountered a case involving a patient with Apert syndrome complicated by OSA who began to receive continuous positive airway pressure (CPAP) therapy at the age of 4. OSA resolved after maxillofacial surgery performed at the age of 11, and CPAP was eventually withdrawn. In pediatric patients with maxillofacial dysplasia complicated by
