[Comparative study of the acceptability of the SpeediCath Set and Actreen set catheterization sets in patients performing self-catheterization].

OBJECTIVE: To compare SpeediCath Set to Actreen Set in terms of performance, acceptability and safety, in patients performing self-catheterization. MATERIAL AND METHODS: Four questionnaires were completed during this multicentre, randomized, crossover study: initial, after having tested each of the two products and patient preference at the end of the study. RESULTS: Analysis was based on 29 men performing self-catheterization. The SpeediCath Set was found to be superior to the Actreen Set in terms of ease of introduction (p=0.0055), ease of emptying (p=0.0157), quality of lubricant (p<0.000

Acute urinary retention: a review of the aetiology and management.

Acute retention of urine (AUR) is a common urological emergency characterised by a sudden inability to pass urine associated with lower abdominal pain. In recent years, the natural history and incidence of AUR has become better understood, however, further research into methods to prevent it and evaluation of the impact an episode of AUR has on the patient is required. This review provides an overview of the current management of AUR in men and the impact of the condition on patients' quality of life.

Continuous bladder infusion methods for studying voiding function in the ambulatory mouse.

OBJECTIVES: To develop a method for chronic cannulation of the mouse bladder that would enable repeated intravesical drug delivery and measurement of voiding patterns in unrestrained mice under controlled infusion conditions. METHODS: Fifteen female mice were anesthetized with halothane and implanted with a 3F polyurethane bladder catheter. The catheter exited from the back through mesh and a polysulfone button into a spring coil that protected the catheter and tethered the mouse. A counterbalanced swivel and infusion pump permitted unencumbered mobility during continuous intravesical perfusi

Coated catheters for intermittent catheterization: smooth or sticky?

OBJECTIVE: To evaluate the current range of hydrophilic-coated catheters for intermittent self-catheterization, focusing on the adherence of the catheter to the urethral mucosa at the end of catheterization. PATIENTS AND METHODS: In a prospective randomized study, 61 community-based men tested each of four different hydrophilic-coated catheters available in the UK at the time. Subjects used each of the four test catheters for 1 week in a random order, and were provided with the number and size of catheter they normally used. To assess the products, the subjects: (i) timed seven catheterizatio

Early evolution of bladder emptying after myelomeningocele closure.

OBJECTIVES: To analyze bladder emptying in newborns with spina bifida after closure of the myelomeningocele defect. Manipulation of the spinal cord and nerve roots results in temporary changes in bladder function in many settings. METHODS: We reviewed 62 consecutive newborn patients who underwent closure of a myelomeningocele defect at our institution from January 1990 to December 1997. We examined the catheterized urine volumes obtained before and after closure, radiographic and urodynamic studies, and the subsequent need for intermittent catheterization. Poor bladder emptying was defined as

Management of men with a first episode of acute urinary retention due to benign prostatic enlargement.

OBJECTIVE: To determine the effect of the existing management protocol of patients presenting with acute urinary retention due to benign prostatic enlargement on clinical efficacy and surgical practice. DESIGN: Prospective study. SETTING: The Urology Unit at the Teaching Hospital, Karapitiya, Galle. PATIENTS: 100 consecutive patients with a first episode of acute urinary retention due to a clinically benign enlarged prostate. MEASUREMENTS: Success of voiding urine after one week of treatment with an alpha adrenoceptor blocker (prazosin). Incidence of subsequent urinary retention during the

Reflex interaction between the proximal urethra and the bladder. A clinical experimental study.

OBJECTIVE: Animal experiments have shown that the bladder-cooling reflex is activated by cold stimulation of the bladder and urethra, causing a reflex bladder contraction. In this clinical experimental study, the bladder reflex responses to distension and cooling of the bladder neck and the proximal urethra were investigated. MATERIAL AND METHODS: Twenty-one patients with overactive bladders and documented positive ice water tests were studied. RESULTS: Three patients (14%) responded with reflex bladder contraction by urethral infusion of ice water only, and another 4 patients responded to u

Voiding function study with ultrasound in male and female neonates.

BACKGROUND: The neonatal period has been characterized as a time when males have a much higher incidence of urinary infection and severe ureteral reflux than females. However, little information about the voiding function in the neonatal period is available. METHODS: The bladder urine volumes, before and after voiding, and urinary flow rates were determined with the use of noninvasive voiding-provocation maneuvers and ultrasound in the apparently normal neonates. RESULTS: There was no significant difference in the prevoid bladder urine volume between the two sexes. After they were stimulated

Hematuria and clot retention after Burch colposuspension and Cystofix suprapubic catheterization: suprapubic cystotomy as an alternative suprapubic drainage method.

Suprapubic catheterization is a common procedure in continence surgery and hematuria is a common complication after suprapubic catheterization. The authors report an unusual case of acute clot retention after the removal of a Cystofix suprapubic catheter in a patient who had had a Burch colposuspension. Suprapubic cystotomy as an alternate suprapubic method is also discussed.

Prevention of urinary tract infection in patients with spinal cord injury--a microbiological review.

The importance of urinary tract infection (UTI) in patients with spinal cord injury cannot be understated. Many patients with significant bacteriuria are considered to be colonised rather than infected, and treatment should be reserved for those with clinical symptoms or other signs of infection. Published research on the prevention and management of UTI in patients with spinal cord injury often has limitations due to differences in definitions of UTI, studies on groups using different urinary drainage appliances, the mixture of newly injured and longstanding injured patients and studies being

[Ureteral ectopia in pediatric age: treatment and evolution in 55 cases].

In the last twenty years 55 children with ureteral ectopia were diagnosed and treated in our Hospital. Thirty seven of them were female (67.3%) and they presented the urinary tract infection like the most frequent symptomatology. Seven children were diagnosed during the first days after being born because the detection of hydronephrosis in the antenatal ultrasound; 36 were associated with complete ureteric duplication (65.4%) and the other 19 with single system (34.6%). Congenital abnormalities associated were seen in 14 patients with more frequency in those which presented single-system ectop

Complications of invasive, urodynamic examinations and prostate biopsies in patients with benign prostatic hyperplasia.

We studied 85 men with moderate to severe symptoms of benign prostatic hyperplasia (BPH) who completed two placebo-controlled studies of drug therapy. During the 48 week period of treatment and follow-up the patients underwent 164 procedures which included urethral instrumentation, predominantly without antibiotic prophylaxis, and 187 procedures of urethral instrumentation in combination with transperineal prostate biopsy with antibiotic prophylaxis. The risk for a patient to acquire clinically significant urinary tract infection was 2.4% after urethral instrumentation alone and 7.5% when uret

Postoperative hospitalization of children undergoing cross-trigonal ureteroneocystostomy.

The hospital records of 145 girls and 41 boys who had undergone ureteroneocystostomy for vesicoureteral reflux were retrospectively reviewed to determine the duration of urethral catheterization and postoperative hospitalization. Children with a complicated urological history or the need for ureteral tapering and stent placement were excluded. The trend in urethral catheter removal was from 2.71 days postoperatively during the first year of the study to 1.18 days during the last year. Postoperative hospitalization decreased from 4 to 2 days. It is now routine practice to remove the urethral ca

Experience with the Memotherm permanently implanted prostatic stent.

OBJECTIVE: To evaluate the Memotherm (Angiomed) permanently implantable prostatic stent. PATIENTS AND METHODS: Forty-eight men (mean age 70.3 years, range 57-86) with symptoms and urodynamics suggestive of bladder outflow obstruction had a Memotherm stent inserted into the prostatic urethra. Subjective and objective follow-up was performed at 1, 3 and 6 months. RESULTS: All but one patient was eventually able to void. Ten stents have been removed and three patients are awaiting stent removal. Subjective benefits bore no correlation with the maximum urinary flow rate and the findings on cysto

The Prostakath stent: four years' experience.

The Prostakath stent is recommended as an option in the treatment of acute urinary retention in the patient at high risk from a transurethral resection of the prostate. In 57 of 64 patients with acute retention, voiding was re-established. A stent-related problem developed in 16 patients and further treatment was usually required. Patients with satisfactorily functioning stents have been followed up for 12.0 +/- 9.9 months. The most frequent complication was displacement of the stent. Infection and stone formation were uncommon. Our results do not support the use of the Prostakath in patients
