Increased concentrations of mast cell tryptase are a highly sensitive indicator of anaphylactic reactions during anaesthesia. We obtained serum specimens from 350 patients after possible anaphylactic reactions during anaesthesia. Serum was collected from patients in our own institution (27), and transported by mail and courier from other hospitals in response to a request in the medical literature (323). A 54 years old female patient with a mitral valve prolapse and a rheumatoid arthritis treated with steroids was admitted with dyspnea and hypotension, that started 30 min after taking a pill containing enapril and hydrochlorothiazide. Hemodynamic monitoring with a Swan-Ganz catheter showed a pulmonary capillary pressure of 5 mm Hg, a systemic vascular resistance of 887 (dyn sec)/cm5 and a cardiac output of 10 l/min. The patient had a history of adverse reactions to thiazides and responded to volume replacement, dopamine and steroids.

This study investigated the use of measles vaccine in children with a history of egg protein hypersensitivity, referred between 1987 and 1993 to a specialist immunization facility. Two hundred children were seen of whom 199 were immunized without any adverse effect; one child had an anaphylactic reaction. The proportion experiencing an anaphylactic reaction was 0.5% (95% CI 0.01-2.75%). It is believed that a history of egg protein allergy should no longer be regarded as a contra-indication to vaccination. Skin testing prior to measles immunization should, however, continue.

We report four anaphylactoid reactions after intravenous administration of a cremophor-containing multivitamin hydrosol (Hydrosol Polyvitaminé Roche), observed over an 8-month period in a small region of France. The reactions occurred immediately following intravenous infusion of either ionic solutions or parenteral nutrition mixture containing the multivitamin solution. In three cases, erythema and dyspnoea appeared within minutes. The infusion was rapidly stopped and corticosteroids were given to two patients. Calf ears which were sensitised to horse serum and others which were unsensitised were catheterised and perfused at a constant flow rate of 2 ml min-1 with Krebs-Henseleit solution using a peristalic pump. The ear preparations were then challenged with whole horse serum, the sensitising antigen. Vascular responses were monitored as changes in perfusion pressure. A potentially serious complication of epidural bupivacaine used for a repeat, elective Caesarean section is presented. The main features of this reaction were erythema, itching, generalised and pulmonary oedema. In addition, pulmonary oedema was subsequently noted in the baby. An immunological hypersensitivity to bupivacaine or lignocaine is the likely cause.

Topical application of a clioquinol and Bacitracin powder resulted in contact urticaria and an anaphylactoid reaction. Prick tests with clioquinol and bacitracin solution produced a wheal and flare response. This problem is unusual, but has been reported and previous reports are summarized.

Thirteen patients with severe reactions to sweat bee stings were evaluated. Eleven patients had had systemic reactions and two, large local reactions. Skin testing and RAST with venom of other Hymenoptera and whole body extract of sweat bees disclosed negative reactions to both skin tests and RAST of all other insects tested in eight, while three showed sensitivity to honey bee. Intracardiac repair in a 7-year-old girl with tetralogy of Fallot was hampered because of an anaphylactic reaction with cardiopulmonary collapse caused by porcine gut heparin. This patient had an extremely rare case of hypersensitivity to porcine gut heparin but not to bovine lung heparin. Uneventful radical operation could be performed with the use of bovine lung heparin for systemic heparinization during cardiopulmonary bypass.
