Moyamoya disease in an infant with autonomic dysfunction: angiographic and MRI findings.

An infant with persistent Harlequinism went on to develop a hemiparesis secondary to Moyamoya disease at 2 1/2 months of age. The sympathetic nervous system is proposed to be an etiologic factor in the pathophysiology of Moyamoya disease as well as Harlequinism. This is one of the youngest patients reported in the English literature with Moyamoya disease and the only report of the coexistence of Moyamoya disease and atypical Harlequinism. Magnetic resonance imaging led to the diagnosis, which was confirmed by cerebral angiography.

[Malignant neuroleptic syndrome. Literature review and case report].

The neuroleptic malignant syndrome, first described by Delay and Deniker in 1968, is a rare but severe complication of neuroleptic treatment. According to the literature the syndrome particularly affects young men undergoing rapidly intensified neuroleptic treatment ("rapid neuroleptization"). The diagnosis is based on the triad of hyperthermia, extrapyramidal signs, and autonomic dysfunction, once infectious disease has been ruled out. The clinical features, course and therapy of the syndrome are described on the basis of a case report.

Contrasting patterns of autonomic dysfunction in patients with mitral valve prolapse and panic attacks.

Both mitral valve prolapse (MVP) and panic attacks have been reported to be associated with autonomic dysfunction, but previous studies have been limited by the lack of clear separation between patients with MVP and those with panic attacks and the use of noncomparable control subjects. Accordingly, heart rate and blood pressure responses to deep breathing, five minutes' quiet standing, and the Valsalva maneuver were studied in age- and cardiac symptom-matched groups of 33 control subjects, 66 patients with MVP, 20 patients with panic attacks, and 17 patients with both MVP and panic attacks. [Development of an animal model of autonomic over-activity in tetanus].

Although present day progressive respiratory management has decreased the tetanus mortality rate, tetanus-related autonomic overactivity remains a lethal condition. The cause, pathophysiology, and treatment of this condition are controversial and no animal model has ever been reported. So we tried to make a rabbit model of this condition. At first we injected 30 micrograms of purified tetanus toxin intravenously. Blood pressure oscillation occurred, but the degree of oscillation was smaller than that was seen in the clinical cases. The degree of oscillation had not changed by addition of 30 mi

Evidence for a non-cholinergic excitatory innervation in the control of colonic motility.

The colon of the ferret anesthetized with urethane exhibits two distinct types of motility patterns. These were abolished or considerably reduced by blocking nervous conduction in the vagus nerves by cooling to below 4 degrees C. Atropine transiently abolished motility which on its return was also found to be sensitive to vagal integrity. Electrical stimulation of either the cut central or peripheral end of a branch of the abdominal vagus caused large amplitude contractions of the colon which were not blocked by atropine or by atropine and a combination of alpha- and beta-adrenoceptor blocking

Oesophageal function in diabetes mellitus and its association with autonomic neuropathy.

We have evaluated oesophageal function in 34 diabetics and in 16 non-diabetic controls by a timed bolus transit method derived from dynamic oesophageal scintiscans (water transit time: WTT) and barium swallow. The diabetics were screened for autonomic neuropathy using standard cardiovascular responses and 10 patients were classified as abnormal. WTT was significantly prolonged in autonomic neuropaths compared with other diabetics (p less than 0.01) and controls (p less than 0.001). Abnormal peristalsis on barium swallow was seen more frequently in autonomic neuropaths (9/10) than in other diab

Interaction of adrenergic and opioid systems in the short-term regulation of cardiovascular activities.

Endogenous opioid peptides such as methionine enkephalin (met-enkephalin) are known to have a profound cardiovascular action in addition to actions mimicking morphine in many animal models. In the anesthetized rabbit, met-enkephalin, as little as 1 microgram/kg, decreased blood pressure and heart rate upon its intravenous injection. Little is known about the locus of action or the mechanism and the physiological implication of the peptide action. The primary objective of this study was to test influences of adrenergic and cholinergic agents on the cardiovascular effects of the peptide. Pentoba

A critical level of diabetic autonomic neuropathy.

Diabetic autonomic neuropathy was expressed quantitatively using the coefficient of variation of R-R intervals (CVR-R) in ECG and its critical level where diabetics were plagued with various symptoms was investigated. The subjects were 58 diabetics under the age of 40 years. ECGs were recorded in the resting supine position and the degree of autonomic neuropathy was expressed as CVR-R by processing 100 consecutive R-R intervals. Symptoms caused by diabetic autonomic neuropathy were determined using a questionnaire. CVR-R were 1.10 +/- 0.34% (mean +/- SD) and 4.31 +/- 2.01% in patients with and

The anatomy of the lumbar sympathetic trunks in man (with special reference to the question of regeneration after sympathectomy).

Although possible to decentralize the sympathetic outflow to the leg, below the knee, by resecting only the third lumbar sympathetic ganglion, the likelihood of restoration of normal vasoconstrictor activity warrants a more extensive resection to include at least the second and third lumbar ganglia, and preferably the second, third, and fourth lumbar ganglia. When cross-over fibers are suspected or demonstrated following unilateral sympathectomy, the denervation should include the fourth and fifth lumbar ganglia. The first lumbar ganglion should be spared at least unilaterally if interference

NON-IMMUNOLOGIC FACTORS IN ALLERGIC DISEASES.

Many different explanations of the mechanism of asthma and other allergic diseases have been offered. Of these, the immunologic concept has gained the greatest acceptance. During the past 25 years, a renewed attempt to place the diagnosis and treatment of allergic diseases on a firm scientific basis has yielded many fundamental facts and principles in such varied fields as immunology and psychiatry, but no one of these disciplines affords a satisfactory approach to the practical management of the individual patient. There is a vast gap between scientific fact and medical practice. The need at
