BACKGROUND: The most common treatment for old calcaneal fractures accompanied by subtalar joint injury is the use of subtalar in situ arthrodesis and subtalar distraction bone-block arthrodesis or osteotomy. PURPOSE: Traditionally, valgus knee deformity is predominately corrected by stabilisation with a plate inserted via the medial approach to the supracondylar region of the femur. A retrospective review was conducted to examine rates of malreduction and nonunion in ipsilateral femoral neck and shaft fractures using different fixation strategies. In this case report, we describe a complication of occlusion of a low-flow artery related to traction and compression against the center post of a fracture table during trochanteric femoral nailing. The ischemic limb subsequently underwent urgent revascularization by the vascular surgery team. The patient was placed on anticoagulation and recovered. Radiographically visible vascular plaques should alert the surgeon to potential vascular complications of traction and center-post compression.

Seventy-three children (48 boys and 25 girls; mean age, 5.7 years) with unilateral femoral or tibial shaft fractures were treated using elastic intramedullary nails at the authors' institution.
