[Pars interarticularis reconstruction of spondylolysis using plates and screws with grafting without arthrodesis. Apropos of 78 cases].

Between 1978 and 1985, 78 reconstructions of the pars interarticularis were performed for painful spondylolysis in which there had been failure to respond to conservative methods and the subjacent disc was not degenerate. Sixty-five patients were reviewed with a follow-up greater than 2 years and a mean of 4.4 years. The concept of pars interarticularis reconstruction can be attributed to Kimura (1968), Buck (1970) and Nicol and Scott (1986). It consists of a posterior approach with freshening of the margins of the defect, the insertion of cancellous chips taken from the spinous processes, res

Second generation locked plating for complex proximal humerus fractures in very elderly patients.

OBJECTIVES: Humeral head sacrificing procedures are more favored in elderly patients with complex proximal humerus fractures because of high incidence of failures and complications with osteosynthesis. The purpose of this study is to assess the outcome of second generation locked plating techniques in 3 and 4 part fractures in active elderly patients >70years with an emphasis on function and complications. MATERIALS AND METHODS: 29 patients with displaced 3 and 4 part proximal humerus fractures were treated using the principles of second-generation proximal humerus locked plating. Fixed angle

Treatment of humeral shaft fractures with antegrade intramedullary locking nail.

Antegrade interlocked humeral nailing for stabilization of humeral fractures was introduced many years ago, and studies on this method in the orthopedic literature have shown mixed results. The purpose of this investigation was to document the clinical outcome and complications associated with the use of an antegrade intramedullary nail (T2, Stryker) for the humeral fractures. Between 2005 and 2008, 52 fractures of the humeral shaft were treated operatively with this intramedullary nail in our department. Eight patients were polytraumatized, and four patients had an open fracture. The mean age

Immediate implant placement in the esthetic zone with buccal wall defect using the immediate dentoalveolar restoration (IDR) protocol: A clinical report.

Immediate implant placement in the anterior maxilla is indicated for patients with an intact labial plate to achieve esthetics and predictability. This clinical report describes the use of the immediate dentoalveolar restoration (IDR) protocol with a digitally designed tuberosity adjustment guide (TAG) to restore a compromised socket in the esthetic zone. A 47-year-old patient presented with a missing lateral incisor with loss of the labial wall. The tooth had been lost because of trauma. A tuberosity bone graft was shaped using a computer-aided design and computer-aided manufactured (CAD-CAM)

[Surgical Stabilization of Traumatic Multiple Rib Fractures Using Absorbable Osteosynthesis Agent(u-HA/PLLA Composite Material)].

In case that met several indication criteria with 4 or more rib fractures, we performed surgical stabilization of multiple fractured ribs using a plate and screw system( Super FIXORB MX) that was made of uncalcined hydroxyapatite (u-HA)/poly-L-lactic acid (PLLA) composite material with excellent bioactivity and absorbability. We report our clinical experience of 7 cases in which this device was used. Although there is still room for further consideration of the technique and the strength of the device itself, computed tomography( CT) images taken 9 months after surgery showed that the fixative

Arthroscopic Stabilization of Osteochondral Fracture of the Anterior-Inferior Glenoid Using Double-Row Suture Bridge Technique.

Osteochondral fracture of the glenoid can be caused by traumatic damage in the shoulder, and addressing this injury can be challenging due to the complexity of the condition and the various treatment options. Left untreated, this injury can lead to post-traumatic arthritis and shoulder instability. There have been different surgical techniques for addressing osteochondral fracture, including open and arthroscopic methods, such as debridement, screw fixation, and bone graft, but there is a lack of a universally accepted treatment guidelines. In this technical note, the authors introduce a simpl

Osteochondritis Dissecans of the Lateral Process of Talus Involving the Subtalar Joint: An Unusual Case.

Osteochondritis dissecans is a fairly recognized entity affecting the talus dome but subtalar joint involvement is not that common. We report a case of a 34-year-old male with osteochondritis dissecans of lateral process talus which was missed on imaging studies and identified intraoperatively. The patient was treated with curettage of the subchondral cyst at talus, followed by bone grafting and fixation of the osteochondral lesion with a 4-mm partially threaded cancellous screw. At the 1-year follow-up evaluation, there were no signs of recurrence and the patient resumed his complete activiti

Use of an 'osteocompressor' to avoid damage to the inferior alveolar nerve in the mandibular setback procedure: can the positive effects of osteodistraction be used in reverse? An experimental study in dogs.

We have designed an osteocompressor to try and avoid damage to the inferior alveolar nerve during sagittal split setback osteotomy of the mandibular ramus, and tested it on the mandibles of 10 dogs. The osteocompressor bears a superficial resemblance to an osteodistractor, but has a different internal structure that allows it to compress rather than distract. We were able to compress the neurovascular canal, the neurovascular bundle, the cancellous bone, and the mandible in dogs at a rate of 1 mm/day by rotating the screw of the compressor 1.5 times. We conclude that in dogs the neurovascular

Experience with scaphoid grafting.

Over a period of 24 years, the author has used five different methods of bone-grafting for ununited scaphoid fractures. The clinical and radiological results have been reviewed, with a minimum follow-up of 1 year. Radiologically the best results (78% definite union) were obtained with a "wedge" graft and Herbert screw, while the worst results followed the original Russe operation. The clinical result often did not coincide with the radiological outcome. All methods led to a decrease in pain in most cases, but little or no pain was achieved most often by the modified Russe graft. With proximal

[Fractures of the inner epicondyle of the humerus associated with dislocation of the forearm bones in children and adolescents].

The authors attract attention to frequent association of fracture of the inner epicondyle of the humerus with dislocation of the bones of the forearm, which is of great practical importance. On this basis classification of fractures of the inner epicondyle of the humerus is presented. The authors believe that in treating this injury the method of choice should consist in open reposition, which allows to achieve accurate reposition, sanation of the ulnar joint cavity and stable fixation of the fragment. Osteosynthesis by screw-awl provides for reliable fixation of the fragment and allows for ea

Design of a porous biodegradable internal fixation for the treatment of pauwels type III femoral neck fractures.

Prolonged internal fixation in the femur can cause issues like osteosclerosis and stress masking, which hinder fracture recovery. AZ31b magnesium alloy has already been widely investigated and shown to be biocompatible and biodegradable. This study used finite element analysis to investigate stress changes during healing, aiming to find the best time for removing internal fixation and focuses on optimizing the structural design of internal fixation, using AZ31b magnesium alloy, to address issues like osteosclerosis and stress shielding in femoral neck fracture healing. This study focuses on op

Open anatomical glenoid reconstruction with an iliac crest bone autograft effectively resolves off-track Hill-Sachs lesions to on-track lesions.

INTRODUCTION: The purpose of this study was to determine if "off-track" Hill-Sachs lesions in patients with dynamic anteroinferior instability were transformed into "on-track" lesions using iliac bone autografts with screw fixation. The secondary purpose was to observe if postoperative bony remodeling would occur over time, resulting in recurrent "off-track" Hill-Sachs lesions with corresponding instability. MATERIALS AND METHODS: We retrospectively reviewed clinical and CT records of 8 patients with an "off-track" Hill-Sachs lesion who underwent open anatomical glenoid reconstruction with an

Endoscopic transforaminal decompression, interbody fusion, and percutaneous pedicle screw implantation of the lumbar spine: A case series report.

BACKGROUND: On the basis of the experiences gained from conventional open spinal procedures, a long list of desirable objectives have emerged with the evolution of the lesser invasive spinal procedures. At the top of that list is the desire to minimize the trauma of surgery. The rest of the objectives, which include reductions of operating time, surgical blood loss, hospital stay, postoperative narcotic medication, convalescence, complication rates, and escalating health care costs, as well as the desire of elderly patients to continue rigorous physical activities, largely depend on the abilit

Plate fixation for Letenneur type I Hoffa fracture: a biomechanical study.

BACKGROUND: A coronal fracture of the posterior femoral condyle, also known as a Hoffa fracture, is an unusual injury, and there are only a handful of case reports or series exploring it. The optimal fixation method of these intraarticular fractures remains controversial; improper or unstable fixation usually lead to an unsatisfactory prognosis. The use of posterior-anterior or reversed lag screw fixation is still a popular method. Additional buttress plating is also recommended for fixation of these difficult fractures. The purpose of this study was to compare the mechanical strength of four
