Early traumatic experiences impair the functioning of both components of the endogenous stress response system in adult people with eating disorders.

Childhood trauma is a non-specific risk factor for eating disorders (EDs). It has been suggested that this risk is exerted through trauma-induced long-lasting changes in the body stress response system. Therefore, we explored the activity of the hypothalamus-pituitary-adrenal axis and of the sympathetic nervous system in adult ED patients with or without a history of childhood trauma exposure. Salivary cortisol and alpha-amylase, a marker of the sympathetic nervous system activity, were measured at awakening and after 15, 30 and 60 min in 35 women with EDs. The Childhood Trauma Questionnaire (

Body mass index kinetics around adiposity rebound in Anorexia nervosa: A case-control study.

BACKGROUND AND OBJECTIVE: Anorexia nervosa (AN) is associated with parameters involved in body mass index (BMI) regulation. Contrary to obesity, BMI kinetics around the adiposity rebound is not documented in AN. This study aimed at investigating which characteristics of BMI kinetics around the adiposity rebound are associated with AN. METHODS: Multicentre case-control study with 101 inpatient women with AN onset after 10 years of age, and 101 healthy women, all free of overweight history and matched for age, level of education and fathers' socio-professional status. Age at adiposity rebound,

Recollections of pressure to eat during childhood, but not picky eating, predict young adult eating behavior.

Picky eating is a childhood behavior that vexes many parents and is a symptom in the newer diagnosis of Avoidant/Restrictive Food Intake Disorder (ARFID) in adults. Pressure to eat, a parental controlling feeding practice aimed at encouraging a child to eat more, is associated with picky eating and a number of other childhood eating concerns. Low intuitive eating, an insensitivity to internal hunger and satiety cues, is also associated with a number of problem eating behaviors in adulthood. Whether picky eating and pressure to eat are predictive of young adult eating behavior is relatively uns

[A pilot study on the specificity of body image disturbance in anorexia nervosa].

OBJECTIVE: Body image disturbance (BID) is a central feature of anorexia nervosa (AN), but evidence for bodily-related disorders also exists for patients with cystic fibrosis (CF), who are frequently underweight. A comparison of BID in patients with AN, CF and controls serves to clarify the specificity of BID for AN. METHOD: 22 patients with AN, 10 patients with CF, and 23 controls were tested with regard to perceptual and cognitive-affective components of BID. Further data concerning eating-disorder-related psychopathology were assessed. RESULTS: BID occurred in all patients with AN. Patien

Stress: from symptom to disease.

AIM: To evaluate the presence and role of stress in causing diseases. MATERIALS AND METHODS: We conducted a 6-month study in patients who presented for the first time with a series of symptom and tried to look for an organic cause of those symptoms. We wanted to see the impact of stress in everyday life and its role in causing various diseases. RESULTS: The obtained results showed a female predominance in terms of seeking medical advice and that the most affected age group was 20-30 years, accounted for by the start of socio-professional issues. The psycho-behavioural symptoms were the most

[The link between ambivalent-preoccupied mode of attachment and narcissistic vulnerability in adolescence].

Attachment research has deepened our understanding of the essential continuities and discontinuities of psychological development from childhood to adolescence and to adulthood. This paper considers the relevance of ambivalent-preoccupied attachment, one of the two types of insecure attachment, for understanding emotional vulnerability during adolescence. How current social and economic conditions have exacerbated the effects of ambivalent-preoccupied attachment in the attainment of the developmental tasks of adolescence is considered. By describing three clinical vignettes clinical recommenda

Rumination and body dissatisfaction interact to predict concurrent binge eating.

Based upon the escape theory (Baumeister, 1991; Heatherton & Baumeister, 1991) and the emotional cascade model (Selby, Anestis, & Joiner, 2008), it was hypothesized that body dissatisfaction and rumination tendencies would interact to predict concurrent binge eating symptoms. This hypothesis was tested in a sample of 780 undergraduate students. The results conformed to prediction, in that individuals with high levels of body dissatisfaction were most likely to report binge eating behavior when they also had a tendency to ruminate. This interaction had a significant association with binge eatin

Enhancing motivation to change in eating disorders with an online self-help program.

Ambivalence to change is a major obstacle to treating people with eating disorders. Enhancing motivation to change can contribute to recovery from the disorders. This study used an Internet-based self-help program developed in the Asia-Pacific region to identify the motivational stages of change in people with eating disorders. It explored their perceptions of the benefits and costs of taking action against their eating disorders, and assessed their involvement in motivational enhancement exercises and their improvement in eating disorder psychopathology. A total of 185 participants, aged 16-5

Eating patterns in youth with and without loss of control eating.

OBJECTIVE: To compare the characteristic meal patterns of adolescents with and without loss of control (LOC) eating episodes. METHOD: The Eating Disorder Examination was administered to assess self-reported LOC and frequency of meals consumed in an aggregated sample of 574 youths (12-17 years; 66.6% female; 51.2% Caucasian; BMI-z: 1.38 ± 1.11), among whom 227 (39.6%) reported LOC eating. RESULTS: Compared to those without LOC, youth with LOC were less likely to consume lunch and evening meals (p's < .05), but more likely to consume morning, afternoon, and nocturnal snacks (p's ≤ .05), accoun

Body-related behaviours and cognitions in the eating disorders.

BACKGROUND: Different body-related behaviours and cognitions (checking, avoidance, comparison, display) have been shown to be related to unhealthy eating attitudes in a non-clinical sample. AIMS: This study tested whether the use of body-related behaviours is higher in eating-disordered women than in non-clinical women. It also examined whether the use of body-related behaviours is associated with psychological characteristics (particularly anxiety, depression and narcissistic characteristics), controlling for age and eating pathology. METHOD: Ninety-nine adult women with diagnosed eating di

Positive response to binge eating intervention enhances postoperative weight loss.

BACKGROUND: Binge eating disorder has been suggested as a predictor of negative outcomes, including weight regain and poorer weight loss, particularly if the symptoms (e.g., loss of control eating) remain after surgery. Binge eating disorder has been viewed by some as a contraindication for weight loss surgery, and preoperative treatment has been recommended to help reduce binge eating behaviors. The objective of the present study was to evaluate whether the response to a preoperative binge eating intervention related to differential postoperative weight loss at an academic medical center. Preventing eating disorders.

This article reviews eating disorder (ED) prevention programs, highlighting features that define successful programs and particularly promising interventions, and how they might be further refined. The field of ED prevention has advanced considerably both theoretically and methodologically compared with the earlier ED prevention programs, which were largely psychoeducational and met with limited success. Recent meta-analytic findings show that more than half (51%) of ED prevention interventions reduced ED risk factors and more than a quarter (29%) reduced current or future eating pathology (EP

Personality dimensions among women with an eating disorder: towards reconceptualizing DSM.

OBJECTIVE: To evaluate the incremental validity of a dimensional assessment of personality, after controlling for diagnostic category, in accounting for meaningful variation in eating disorder attitudes and behaviours and in current affective distress among a clinical sample of eating disordered women. METHODS: 244 treatment seeking eating disordered women and 116 non-eating disordered women were assessed with the NEO five factor inventory (NEO-FFI), and with measures of eating disorder attitudes and of affective distress using a cross sectional design. RESULTS: As predicted, differences wer

[What can weight losses in anorexia nervosa predict about the premature termination of treatment?].

It was analysed if the weight course in the first half of a four-step psychosomatic treatment program for inpatients with Anorexia nervosa (AN) was useful to predict premature termination of treatment. The weight courses, sociodemographic and disorder-related data of 121 female adult inpatients were investigated. 39% of the studied patients did not reach their target weight (not completers). 61% of the patients completed all four steps of treatment (completers). The not completers showed significant more time with weight losses in the first step and had more and bigger weight losses in the sec

High risk of osteoporosis in male patients with eating disorders.

OBJECTIVE: Osteoporosis has traditionally been considered a female problem. This study's purpose is to evaluate bone mineral density (BMD) in males with eating disorders. METHOD: Charts of 70 consecutive males admitted to an eating disorder program were reviewed. Females admitted during the same time period were used for comparison. BMD was measured by dual-energy X-ray absorptiometry. RESULTS: Thirty-six percent (19/53) had osteopenia and 26% (14/53) had osteoporosis at the lumbar spine. Cycles of abuse nurtured by concealment: a clinical report.

At present, health care staff do not seem to have sufficient knowledge about their patients' abusive experiences. The aim of the present study is to analyze and discuss what the implications might be for the encounter between patients and health care professionals, when experiences of abuse are concealed. The methodology of this article is varied: a personal narrative, medical records, sociological theoretical literature and empirical evidence. From the narrative we learn that concealment of abuse was devastating for the patient. She was "treated" in vain as a correct diagnosis was not made, w

Core beliefs and narcissistic characteristics among eating-disordered and non-clinical women.

OBJECTIVES: Narcissism is a personality trait that can interfere with the application of evidence-based therapies for the eating disorders, influencing collaboration and the patient's willingness to take responsibility for participating in change. In order to understand and work with this personality characteristic, it is important to understand the cognitions that underpin the traits concerned. DESIGN: This study examined the associations between schema-level core beliefs and narcissism in eating disorders. Narcissism was conceptualized in terms of both its core element (entitlement and gran

A 12 to 24 weeks pilot study of sertraline treatment in obese women binge eaters.

Previous studies tested the efficacy of sertraline in Binge Eating Disorder (BED) over a period of 6 weeks. The present open study assesses the efficacy of sertraline over a period of 24 weeks in obese persons with binge eating behaviour (with or without the full criteria for BED) confirmed by high scores on the Binge Eating Scale (BES). Thirty-two obese outpatients (14 with BED and 18 without full criteria for BED), without co-occurring psychiatric comorbidities, were treated with sertraline (dose range 100-200 mg/d). Subjects were assessed at baseline and at 8, 12 and 24 weeks of treatment f
