Photosensitivity dermatitis is a multifactorial dermatologic condition characterized by an exaggerated cutaneous response to ultraviolet (UV) radiation, often exacerbated by exogenous agents, particularly photosensitizing medications. The pathophysiology involves direct phototoxic or immune-mediated photoallergic mechanisms, leading to inflammatory skin reactions upon UV exposure. In individuals with vitiligo, the absence of melanocytes in depigmented areas significantly diminishes photoprotective mechanisms, rendering these regions highly susceptible to UV-induced damage. Consequently, vitili…

Too many patients with photoallergy remain undiagnosed due to unsatisfactory knowledge among doctors and limited access to photopatch testing. The objectives of this study were to analyze the frequency of patients requiring diagnostic work-up for photoallergic contact dermatitis among dermatology patients, and to identify the causative photosensitizers. This prospective study involved 1000 consecutive, first-referred dermatology outpatients. All patients with a history of dermatitis induced or aggravated by exposure to light were qualified for photopatch testing. In the study group, 36 (3.6%; …

Our aim was to study the evolution of clinical pattern of parthenium dermatitis. Patients with clinical picture consistent with parthenium dermatitis for 3 years or more along with positive patch test to parthenium were studied. Patients were questioned regarding the site(s) of dermatitis at the onset and change in localization in the following years. Patients were classified into airborne contact dermatitis, chronic actinic dermatitis (CAD) pattern or mixed pattern dermatitis. There were 74 patients (49 men and 25 women) with an age range of 22-70 years and the mean duration of 7.7 years. 60 …

BACKGROUND: Photoallergy to ketoprofen and cross reactivity of ketoprofen with diphenylketones are well known; here are some cases of photoreaction to ketoprofen and unusual substances.
PATIENTS AND METHODS: Eleven photoallergic to ketoprofen patients were photo patch tested with Ketum(R), ketoprofen, oxybenzone, tiaprofenic acid, fenofibrate, and also chlorphenesin, sunscreen series, and photobiologists series (without lichen mix and benzocaine). We performed tests at day 0, we irradiated them with UVA at day 2, and control series remained closed. We performed readings at day 3 and 4 accordi

We report a case of combined contact and photocontact allergic dermatitis to etofenamate in Flogoprofen gel (Chiesi Wasserman, Barcelona, Spain). Patch test results were positive at the nonirradiated site, but there was a stronger reaction at the irradiated site with etofenamate 0.05% in petrolatum (pet.) at d2 and d4. The use frequent of topical agents containing etofenamate and sun exposure can result in a predisposition to contact photoallergy. Clinical findings caused by etofenamate are uncommon. Allergic contact dermatitis is the most common cutaneous reaction reported. In American studie…

Allergic and irritant contact reactions were evaluated in the recently identified hairless guinea pig, Crl:IAF(HA)BR, a mutant from the Hartley strain. The cutaneous changes were observed macro- and microscopically. The irritant contact dermatitis was induced by croton oil, 2,4-dinitrochlorobenzene (DNCB), or anthralin. Both hairless and hairy guinea pigs developed similar reactions to these chemicals. The density of the epidermal Langerhans cells (LC) of hairless guinea pigs was significantly higher than that in the hairy strain. Allergic contact sensitization was easily induced with DNCB. Ph…

BACKGROUND: Chronic photosensitivity dermatitis (CPD) (also named actinic reticuloid) is an unusual disease classically referred often in elderly men. Affected patients have severely itchy, thickened dry skin in areas exposed to the sun throughout the years.
METHOD: A Caucasian female patient who worked most of her life outside who had "chronic dermatitis" in her neck started planting chrysanthemum in her garden on a sunny day. Later, she presented edema, erythema, papules, and a few vesicles in her neck with severe pruritus.
STUDIES: A skin biopsy revealed the diagnosis of CPD, along with p…

A topical application of a nonsteroidal anti-inflammatory drug (NSAID) may induct an adverse reaction of photoallergic contact dermatitis. The occlusive usage may provoke concomitant photoallergic sensitizations to an NSAID and other ingredients. We describe a 58-year-old woman with photoallergic contact dermatitis from ketoprofen and hydrogenated rosin glycerol ester in the applied compress. Our case indicates that photopatch testing with all ingredients is required to verify the actual photoallergen(s).

Quinolone antibacterial agents are well known to elicit photosensitivity as a side effect. The photoallergenicity of fluoroquinolones, the representative quinolone derivatives, is mainly derived from their photohaptenic moiety. When epidermal cells are irradiated with ultraviolet A light in the presence of fluoroquinolones, quinolone photoadducts are formed in the treated cells. This photomodification is thought to be an initial step for sensitization and elicitation of this photoallergy, and quinolone-photoderivatized Langerhans cells are capable of stimulating immune T cells in mice. In the

We report two patients with bullous phototoxic contact dermatitis caused by Dictamnus albus L. (gas plant) which belongs to the Rutaceae family. In both cases long lasting postinflammatory hyper-pigmentations were observed as a result of the phototoxic contact dermatitis. The phototoxic components of Dictamnus albus L. are the furocoumarins 5-methoxypsoralen (bergapten) and 8-methoxypsoralen, and the alkaloid dictamnin whose phototoxic properties were recently discovered. Even though reactions to the gas plant have been infrequently reported, it is becoming more popular with gardenus and shoul…

Abrocitinib, an oral selective Janus kinase 1 inhibitor, is approved for the treatment of moderate-tosevere atopic dermatitis (AD) in adults, but realworld evidence remains limited. We retrospectively collected data from 187 adult patients (mean age:37.9 years; 60.4% male), with AD and up to oneyear exposure to abrocitinib either 100 or 200 mg, between 2023 and 2025 across 22 Italian dermatology centres. Nearly half of patients were bio-experienced and 80 % had difficult-to-treat areas. Beginning at week 8, with either 100 (59%) or 200 mg, 52 % achieved Investigator Global Assessment 0/1, sign…

A 19-year-old female college undergraduate developed an intensely swollen, erythematous and pruritic rash on the face and hands while working in an optical fabrication lab producing photosensitive polymers. She had no respiratory symptoms. The rash was consistent with contact dermatitis and there was no clinical evidence of respiratory involvement with normal spirometry. A review of the safety data sheets of chemicals used in the laboratory revealed several known sensitizers, including 6-hexamethylene diisocyanate (HDI), dibutyl phthalate, and 2,4,6-tribromophenyl acrylate. Patch testing confi…

The cutaneous lymphocyte-associated antigen (CLA) is the major T cell ligand for the vascular adhesion molecule E-selectin, and it has been proposed to be involved in the selective targeting of memory T cells reactive with skin-associated Ag to cutaneous inflammatory sites. To further investigate the relation of CLA and cutaneous T cell responses, we analyzed the CLA phenotype of circulating memory T cells in patients with allergic contact dermatitis and atopic dermatitis (AD) alone vs in patients manifesting bronchopulmonary atopy (asthma with or without AD) and nonallergic individuals. Signi…

The NLRP3 inflammasome is upregulated by various agents, such as nuclear factor-kappa B (NF-κB), lipopolysaccharide (LPS), and adenosine triphosphate (ATP). The NLRP3 inflammasome facilitations the maturation of interleukin (IL)-1β, a proinflammatory cytokine that is critically involved in the pathogenesis of atopic dermatitis (AD). Although the NLRP3 inflammasome clearly exacerbates AD symptoms such as erythema and pruritus, drugs for AD patients targeting the NLRP3 inflammasome are still lacking. Based on the previous findings that Mentha arvensis essential oil (MAEO) possesses strong anti-i…

BACKGROUND AND OBJECTIVE: Bufexamac is a non-steroidal, anti-inflammatory drug used in the topical treatment of atopic dermatitis, stasis dermatitis and perianal eczema. The substance is known to cause severe allergic contact dermatitis (ACD) as an adverse effect (AE), which may be indistinguishable from the eczema which is to be treated. Hence the diagnosis of this AE is often considerably delayed. In order to estimate the quantitative importance of ACD to bufexamac, data of the Information Network of (German) Departments of Dermatology (IVDK) from July 1999 to December 2004 were analysed.

BACKGROUND: Mitochondrial DNA copy number (mtDNA-CN), a marker associated with mitochondrial function and cellular health, has been implicated in several chronic inflammatory diseases. Atopic dermatitis is a common chronic inflammatory skin disorder, yet the relationship between mtDNA-CN and childhood atopic dermatitis remains unclear. This study aimed to investigate the association between mtDNA-CN and atopic dermatitis in a population-based cohort of school-aged children.
METHODS: We included 1329 singleton children (56.4% boys; mean age ± standard deviation: 6.5 ± 0.5 years) from the Longi…
