New clinical finding in vestibular neuritis: high-frequency audiometry hearing loss in the affected ear.

Twenty-one patients with established vestibular neuronitis were examined otoneurologically and audiologically. High-frequency audiometry up to 20,000 Hz was performed on all the patients. In 17 cases a high-frequency hearing loss was greater in the ear with vestibular neuritis. The mean hearing loss difference varied from 14 dB to 24 depending on the frequency. Our results reveal that in most cases of vestibular neuritis there are no isolated vestibular lesions, but most cases also involve an auditory end-organ lesion. This finding is in unison with the anatomic conditions of the cochlea and t…

Structural defects as consequence of late intrauterine constraint: craniotabes, loose skin, and asymmetric ear size.

Those fetal structures that are normally marked by passive growth in response to neighboring tissues show the effects of extrinsic deforming forces in distinctive ways. An appreciation for the mechanisms of deformation in these structures allows for more accurate clinical diagnosis, provides guidance for prognostic counseling and treatment, and may help eliminate unnecessary testing in the newborn period.

Genetic aspects of the BOR syndrome--branchial fistulas, ear pits, hearing loss, and renal anomalies.

A pedigree of branchio-oto-renal dysplasia (the BOR syndrome) is reported, including the documentation by serial audiometric studies of the onset and rapid progression of hearing loss in the twin sister of an affected child. The literature on this syndrome is analyzed to derive some figures for use in genetic counseling of such families. Branchio-oto-renal dysplasia is an autosomal dominant disorder in which affected individuals may have preauricular pits, lachrymal duct stenosis, hearing loss, branchial fistulas or cysts, structural defects of the outer, middle, and inner ear, and renal anoma…

[Middle ear deafness and noise trauma. Animal studies with the surface specimen technique (author's transl)].

The pertinent clinical literature concerning the research project is reviewed. The middle ear of guinea pigs was first damaged on one side (inflicting a subtotal perforation of the tympanic membrane, dislocation of the incudo-stapedial joint followed by otitis media) and the animals were then exposed to a specified narrow-band noise. The induced inner ear changes were quantified by means of cochleographic studies using the surface specimen technique. In spite of the inner ear changes due to middle ear manipulation prior to the noise exposure, the cochleographic studies revealed an obvious prot…

Eosinophil-Mast Cell Inflammation in Middle Ear Effusion Associated With Chronic Rhinosinusitis With Nasal Polyps.

Chronic rhinosinusitis with nasal polyps (CRSwNP) is predominantly associated with type 2 inflammation, although the inflammatory profile may involve both eosinophils and mast cells. Middle ear disease in CRSwNP is usually attributed to Eustachian tube dysfunction, but inflammatory involvement of the middle ear may also occur. We report the cytological evaluation of middle ear effusion from a patient with CRSwNP. May-Grünwald-Giemsa staining revealed eosinophils, mast cells, and extracellular granules consistent with degranulation. The simultaneous presence of these cellular components suggest…

Visualizing the inner ear: Historical perspectives and future directions to elucidate labyrinth structure and function.

The inner ear is among the most intricate and complex structures in the body. Housed deep within the petrous temporal bone, it contains the organs for audition and balance. Our understanding of its structure and function has relied heavily on advances in imaging and novel visualization modalities. This review synthesizes the historical and technological progression of research on inner ear structure, tracing developments from early philosophical speculation and gross dissection to microscopy, serial histology, radiographic imaging, and contemporary high-resolution digital modalities. We outlin…

Effects of ear canal temperature gradients on altitude-related resonance shifts and hearing aid output.

Understanding how altitude influences hearing aid performance is increasingly important as more users are exposed to high-altitude environments. A previous computational model predicted substantial output reductions near the first-resonance region but assumed that ear canal temperature followed ambient temperature, potentially overestimating resonance-related effects. The present study incorporated an ear canal temperature-gradient sensitivity model and evaluated its effects on predicted hearing aid output and speech audibility at sea level (0 m) and 4572 m using two representative hearing-los…

Effects of inner ear malformations on auditory perception and language development in cochlear implant users.

OBJECTIVES: Inner ear malformations (IEMs) represent a major structural cause of congenital sensorineural hearing loss. This study aimed to examine the impact of IEM subtype on auditory perception and language development outcomes in pediatric cochlear implant (CI) users. DESIGN: A total of 51 children with IEMs who underwent cochlear implantation were included. Of these, 12 (23.5%) had an incomplete partition type 1 (IP-1), 21 (41.2%) had incomplete partition type 2 (IP-2), 11 (21.6%) had enlarged vestibular aqueduct (EVA), and seven (13.7%) had cochlear aperture abnormalities. STUDY SAMPLE…

The Cxcl14 chemokine defines pioneer axon guidance and early circuit assembly in the inner ear.

The nervous system wiring requires the precise coordination of axon guidance, neuronal migration, and target cell recognition. Here, we show that inner ear circuit formation, relies on pioneer cells extending an axonal scaffold that selectively target nascent hair cells. High spatiotemporal imaging of pioneer axons reveal how they navigate through the cranial environment, establish dynamic cell-cell contacts with other axons to finally stabilize in target cells. These pioneer axons are required not only for follower axon growth but also for coordinated migration of follower neurons, revealing …

Relationship Between External Ear Severity and Temporal Bone Malformation in Microtia: A Computed Tomography Study.

Background/Objectives: Microtia is frequently associated with temporal bone malformations, but the extent to which external ear phenotype reflects underlying CT-defined anatomy remains uncertain. This study aimed to compare the relationships of Marx severity grading and Nagata morphological classification with temporal bone anatomy in patients with microtia. Methods: This retrospective study included 26 patients with 27 affected ears who had both preoperative external ear photographs and temporal bone computed tomography (CT) examinations. Each affected ear was classified according to the Marx…

Eustachian tube balloon dilation for middle ear effusion in children with cleft lip and palate.

PROBLEM: Eustachian tube balloon dilation is increasingly recognised as a minimally invasive option for middle ear effusion. However, its role in children with cleft lip and palate remains under-explored. METHODS: We prospectively evaluated 14 cleft lip and palate children (28 ears) with middle ear effusion. Group 1 (intervention) comprised 14 ears that underwent Eustachian tube balloon dilation with intranasal corticosteroids, while Group 2 (control) included 14 ears treated with intranasal corticosteroids alone. Tympanometry, pure-tone audiometry and Eustachian Tube Dysfunction Questionnair…

Injectable glycol chitosan-dexamethasone prodrug thermogel for otoprotective therapy against radiation-induced middle ear injury.

Radiation therapy for head and neck cancer often causes otologic complications due to inflammation and mucosal damage in the middle ear. To address this problem, an injectable glycol chitosan-dexamethasone prodrug thermogel (HGC-Su-DEX) was developed for localized and sustained otoprotective therapy. Dexamethasone (DEX) was covalently linked to hexanoyl glycol chitosan (HGC) via a biodegradable succinate linker, yielding an amphiphilic conjugate capable of sol-gel transition near physiological temperature. The HGC-Su-DEX formulations exhibited tunable thermogelling behavior and comparable rele…

Simulation-driven deep learning for the diagnosis of middle ear pathologies using wideband acoustic immittance.

Wideband acoustic immittance (WAI) provides comprehensive frequency-dependent information for diagnosing middle ear pathologies. However, the scarcity of clinical data and complex response patterns significantly hinder automated diagnosis, particularly in data-limited scenarios. To address this issue, this study proposes a simulation-driven computer-aided diagnosis framework for WAI based on finite element (FE) modeling. Latin hypercube sampling was employed to systematically perturb key physiological parameters of the human ear FE models, generating a standardized virtual WAI dataset comprisi…
