While transcatheter aortic valve replacement is considered a viable alternative to traditional surgery for patients with critical aortic stenosis, it is still a cardiac surgical procedure with a steep learning curve. Space consideration is a key aspect of the procedure's success. A TAVR program requires the commitment from and investment of institutional resources, the outfitting of an appropriate procedure room, and meticulous training of a multidisciplinary TAVR team. Careful integration of the various imaging modalities, medical specialties, and equipment is necessary to ensure the safety a

Cardiac rhabdomyoma is frequently associated with tuberous sclerosis. However, very few cardiac malformations have been described with tuberous sclerosis. We report a rare case of coexistent cardiac tumor with mitral valve anomaly in a patient with tuberous sclerosis, who developed mitral regurgitation and required a surgical procedure with histological confirmation of cardiac rhabdomyoma.

Two-dimensional echocardiographic study was performed to detect left atrial thrombi in 33 patients with a prosthetic mitral valve. The subjects were 9 males and 24 females and their ages ranged from 21 to 61 years (average 43.2 years). Their prosthetic valves consisted of 18 Hancock, three Carpentier-Edwards, four Björk-Shiley, four Starr-Edwards and four St. Jude Medical valves. Two-dimensional echocardiograms were obtained using a Toshiba SSH-11A echograph with an electronic phased-array scanner. The left parasternal, apical, subcostal and right parasternal windows were used to detect left a

BACKGROUND: Various conduits for right ventricular outflow tract reconstruction have been reported, but most of them are not available in China. The study investigators developed a simple handsewn valved conduit using expanded polytetrafluoroethylene (ePTFE). This study evaluated the midterm outcomes for this conduit.
METHODS: This retrospective study included a total of 72 patients who underwent right ventricular outflow tract reconstruction with ePTFE valved conduits between January 2014 and June 2020. During follow-up, echocardiograms were performed for all patients, and magnetic resonance

To avoid the negative effects associated with pacing, pacemakers are designed to achieve a pacing cadence as close to physiological pacing as possible. In closed-loop stimulation (CLS; a type of rate-responsive functionality used in pacemakers), the changes in impedance (which correlates with the contractility of the myocardium around the lead tip electrode) are tracked, and the paced heart rate is adjusted accordingly. We herein report a case in which we implanted a pacemaker in a post-tricuspid valve replacement patient. A ventricular lead positioned in the coronary vein exhibited good CLS f

BACKGROUND: Carcinoid heart disease (CHD) is common in patients with carcinoid syndrome (CS). Surgical treatment improves the poor prognosis of CHD, although the reported peri-operative mortality is high (∼17%). We attempted to improve outcomes by implementation of a protocol for the management of patients with CHD at a UK Neuroendocrine Centre of Excellence and report our experience.
METHODS: All patients treated for CHD between 2008 and 2015 were included. Peri-operative treatment included surgical features such as invasive pulmonary valve (PV) inspection and preservation of the tricuspid s

The clinical significance of pannus detected on computed tomography (CT) has not yet been investigated. The purposes of this study were to investigate the clinical significance of pannus detected on cardiac CT in patients who underwent aortic valve replacement (AVR) with mechanical valves, and to determine predictors for pannus severity. A total of 92 patients who underwent cardiac CT and TTE and who had undergone mechanical AVR were included. The geometric orifice area (GOA), the presence of limitation of motion (LOM) and pannus were evaluated on CT. The GOA, presence of LOM, and presence and

OBJECTIVES: Mitral valve replacement with bioprosthetic valves is becoming more common. The incidence of structural valve deterioration and the need for reoperative mitral surgery are expected to increase. The operative mortality and morbidity associated with redo mitral surgery remains high. Transapical transcatheter mitral valve-in-valve implantation might offer an alternate and safer approach for high-risk patients.
METHODS: From July 2007 to April 2010, 11 patients with symptomatic mitral prosthetic valve dysfunction underwent transapical transapical transcatheter mitral valve-in-valve im

BACKGROUND: We have retrospectively analyzed the results of the operations made for aortic infective endocarditis with mitral involvement in a single center in 19 years.
METHODS: From May 1992 to January 2011, we have operated on 72 patients with infective endocarditis of the aortic valve with mitral valve involvement. Fifty-two patients (72.2%) were male and the mean age was 40.5 ± 15.5 (9-73) years. The blood cultures were positive in 33 patients (45.8%) and the most commonly identified microorganism was Streptococcus. Nine patients (12.5%) had prosthetic valve endocarditis. The mean durati

While the Ross procedure provides optimal clinical outcomes for young patients with aortic valve disease, the presence of preoperative aortic regurgitation (AR) with a dilated aortic annulus have long been recognized as independent predictors of early autograft failure. While this had led many to abandon the Ross procedure in this setting, we sought to specifically address the clinical and anatomic features that are unique to patients with severe AR, namely a dilated aortic annulus, aortic/pulmonary annular mismatch, ascending aortic dilatation and persistent hypertension early after surgery.

Transcatheter balloon valvuloplasty for the treatment of aortic and pulmonary valve stenosis was first described nearly 40 years ago. Since that time, the technique has been refined in an effort to optimize acute outcomes while reducing the long-term need for reintervention and valve replacement. Balloon pulmonary valvuloplasty is considered first-line therapy for pulmonary valve stenosis and generally results in successful relief of valvar obstruction. Larger balloon to annulus (BAR) diameter ratios can increase the risk for significant valvar regurgitation. However, the development of regurg

PURPOSE OF REVIEW: This focused review summarizes key insights from the past 12 months of basic science and clinical research on bicuspid aortic valve (BAV)-associated aortopathy.
RECENT FINDINGS: Recent studies in BAV-associated aortopathy support a heterogeneous spectrum of disease with distinct phenotypes. Basic science studies provide further support for the concept of regional differences in the severity of aortopathy within the aorta of BAV patients. Clinical studies compared outcomes of BAV patients after isolated aortic valve replacement and showed that those with primarily valvular i

Valvular heart disease (VHD) encompasses a number of common cardiovascular conditions that account for 10% to 20% of all cardiac surgical procedures in the United States. A better understanding of the natural history coupled with the major advances in diagnostic imaging, interventional cardiology, and surgical approaches have resulted in accurate diagnosis and appropriate selection of patients for therapeutic interventions. A thorough understanding of the various valvular disorders is important to aid in the management of patients with VHD. Appropriate work-up for patients with VHD includes a

The assessment of the severity of a valvular lesion traditionally relies on the evaluation of valvular anatomy and on the demonstration of resting hemodynamic disturbances. In patients with stenotic lesions, this is usually achieved by measuring the trans-stenotic pressure difference and by calculating the surface of the stenotic orifice. In patients with regurgitant lesions, assessment of lesion severity involves the demonstration of a significant volume or pressure overload of the various chambers participating in the process. In many instances, however, discrepancies are found between the s

Ultrasonic Doppler analysis of intracardiac blood flow is a technique complementary to echocardiographic imaging, and an important tool in non-invasive diagnosis. Doppler ultrasonography enables the evaluation of systolic murmurs, particularly the calculation of pressure gradients and valve areas in stenotic lesions. In patients with aortic stenosis, there was a very close correlation of invasive and non-invasive pressure gradients (r = 0.92). In patients with mitral stenosis, invasive and non-invasive valve areas were correlated as well (r = 0.78). In addition, regurgitant and shunting jets c
