Over a period of 10 years, a total of 320 limb amputations were performed on adults at the Ahmadu Bello University Hospital, Nigeria. The major indication for upper limb amputation was trauma and post-fracture splintage gangrene (57%). In the lower limb the most common indication for amputation was advanced squamous cell carcinoma of the skin involving the bone. There was no case of peripheral vascular disease in these patients other than diabetic ulcers. The fitting of prosthesis was uncommon. Most of these amputations were certainly preventable. 15 latissimus dorsi free flap transfers were performed at the University of Ioannina Medical School during a 3-year period. Soft tissue defects in the upper (3) and lower (12) extremities resulting from trauma, septic pseudoarthrosis, osteomyelitis, open type IIIb fractures, incomplete amputations and tumor or unstable scar resection were covered by myocutaneous flaps in 6 cases and muscle flaps with split thickness skin grafts in 9 cases. Reciprocal Ia inhibition from ankle flexors to extensors was studied in five patients with spasticity due to incomplete traumatic spinal cord lesions. Nine healthy subjects were tested as controls. Excitability of the soleus motoneuron pool was estimated by H-reflex testing in the resting state. Ia inhibition was activated by conditioning stimuli to the peroneal nerve. A stepwise system of functional management basing on stage-by-stage development of the traumatic disease is suggested. Therapeutic measures under the principle of preventive rehabilitation are to be taken as early as possible. Functional therapy follows the pattern of 4 pathogenetically validated stages. Neuromotor stimulation involving ideomotor exercise, isometric muscular load, orthostatic factor and transcutaneous neuroelectrostimulation present an additional reserve in improving the response to functional therapy of patients with locomotor trauma.

A highly motivated 18-year-old man continued vigorous military training in spite of the presence of 11 stress fractures and subsequently developed two additional stress fractures. Scintigraphy over 27 weeks illustrated simultaneous regression and progression in his individual multiple foci. When stress fracture foci were not in anatomic locations prone to evolve to gross fractures, healing occurred despite vigorous activities and pain.

The peroneal flap has many advantages, such as thin subcutaneous tissue, availability of a large-diameter artery and vein for anastomosis, a long pedicle, and freely designed size and shape. This technique is introduced because it has many outstanding features that allow it to be used to meet a variety of specific needs, such as a free peroneal flap, a peroneal island flap, a free vascularized fibular graft with skin, and a monitoring flap in free vascularized fibular grafts. We have found the peroneal flap to be especially useful.

During the first three days after trauma a pronounced hypotonia of muscles independent of the localization of the vertebral column was observed. Two patients of mathematical estimation of the clinoid form of the vertebra were proposed aimed at objectivization of the roentgenological information. In 90% of cases the methods proved to be effective. Gamma scintigraphic examination of the vertebral column was carried out in 101 patients. A case of traumatic hemipelvectomy with survival is presented, apparently the sixth reported case in the literature. Following complete left hemipelvectomy and fracture of the right acetabulum, the patient recovered, demonstrated voluntary bowel and urinary control, was full weight bearing on crutches (against advice), and refused a bucket seat or prosthesis, as well as neurosurgical consultation.
