A controlled trial of a dissolving oral patch concerning glycyrrhiza (licorice) herbal extract for the treatment of aphthous ulcers.

Recurrent aphthous ulcers (RAU) are a common and painful condition. This article describes a randomized, double-blind clinical trial of an over-the-counter medicated intraoral adhesive patch for treatment of RAU. Subjects were randomly assigned to either an active drug (patch with extract of glycyrrhiza root) or placebo patch treatment group (both n=23) at onset of a lesion. Lesion size and pain report (unstimulated and stimulated) were assessed at intervals. A no-treatment group (n=23) also was recruited and assessed similarly. By the eighth day, the ulcer size for the active treatment group

Factors influencing the incidence and severity of oral mucositis in patients undergoing autologous stem cell transplantation.

This retrospective descriptive study documented the frequency of oral mucositis and examined the impact of certain variables in the development of oral mucositis in autologous stem cell transplants. Oral mucositis occurred in 90% of the patients, but 53.57% developed ulcerations. On average, oral mucositis started on day seven, lasted for six days and resolved on day 13 after the initiation of chemotherapy. Sodium bicarbonate mouthwash and mucositis mouthwash were commonly used interventions; 72.92% of the interventions were used as secondary prevention. Oral mucositis was significantly associ

Cancer therapy-related oral mucositis.

Oral mucositis is a common side effect of cancer therapies, particularly radiation therapy for head and neck cancer and various forms of chemotherapy. It commonly results in severe oral pain that can compromise the duration and success of cancer management. Hospitalizations are common because patients lose the ability to take anything by mouth due to severe pain and must have alimentation supported during this period. Pain management usually requires potent narcotic analgesia. Cancer therapy-related oral mucositis is commonly described as the most significant and debilitating acute complicatio

Incidence of mucocutaneous reactions in children treated with niflumic acid, other nonsteroidal antiinflammatory drugs, or nonopioid analgesics.

BACKGROUND AND OBJECTIVE: Results from a relatively small case-control study recently showed that niflumic acid increases the risk of serious mucocutaneous reactions in children. As a consequence, the Italian Ministry of Health sent a "Dear Doctor" letter in June 2001 to warn pediatricians about the alleged adverse effects. The objective of this study was to estimate and compare the incidence of mild and severe mucocutaneous reactions among children using niflumic acid, other nonsteroidal antiinflammatory drugs (NSAIDs), or nonopioid analgesics. DESIGN: Retrospective cohort study. SETTING: I

The head and neck radiation oncology patient.

Radiotherapy may be either an effective alternative to surgery or a valuable adjunct to surgery and/or chemotherapy in the locoregional treatment of malignant head and neck tumors. The care of patients with head and neck cancer undergoing radiotherapy or who have completed treatment is a multidisciplinary effort. Oral health care providers can expect to be called upon to care for patients with head and neck cancer undergoing radiotherapy. Early, active participation in developing preventive and therapeutic strategies, in implementing the plan, and in the education and rehabilitation of the pat

Thalidomide use in pediatric patients.

OBJECTIVE: To provide a detailed overview of thalidomide use in pediatric patients. DATA SOURCES: English-language articles were identified through a MEDLINE search (1966-February 2001); key terms included thalidomide, child, graft-versus-host disease, cancer, HIV, Crohn's disease, Behçet's disease, and lupus erythematosus. References cited in those articles were also evaluated. DATA SYNTHESIS: Thalidomide appears to be effective in patients with chronic, not acute, graft-versus-host disease (GVHD) and in healing aphthous ulcers in patients with HIV infection. Limited case reports suggest ef

Viral infections of the oral mucosa and perioral region.

Viral infections of the oral mucosa and perioral region are commonly encountered in the practice of dentistry. The accurate and timely diagnosis of such infections, coupled with the institution of appropriate treatment, can often permit quick resolution of the condition with minimal discomfort and anxiety for the patient (and carers) and prevent the spread of infection to others, especially immunocompromised individuals. This article outlines the clinical presentation and appropriate management of common viral infections of the oral mucosa and perioral region.

[Aphthae or painful ulcers induced by nicorandil].

UNLABELLED: A RECOGNIZED CAUSE OF BUCCAL APHTHOSIS: Nicorandil is the leading drug in a new pharmacology class of well tolerated anti-angina products with vasodilating action. The first cases of buccal aphthosis or ulcerations induced by nicorandil were reported in 1996. Among drugs inducing buccal aphthosis or ulcerations, the largest body of information available on reported cases concern nicorandil. CHARACTERISTIC LESIONS: Forty cases of painful buccal lesions induced by nicorandil have been recorded. Among the 8 publications in the literature, 7 were reported by French groups. The prevalen

Mucosal regeneration during radiotherapy. Trans Tasman Radiation Oncology Group (TROG).

BACKGROUND AND PURPOSE: Regeneration of the aerodigestive mucosa is known to occur during conventionally fractionated radiotherapy. The circumstances surrounding its time of onset and magnitude are not well understood, however. MATERIAL AND METHODS: Mucosal reactions were observed in 100 patients undergoing conventionally fractionated treatment at 2 Gy/day over 7 weeks and 88 receiving accelerated treatment at 1.8 Gy twice daily over 3 1/2 weeks on the Trans Tasman Radiation Oncology Group head and neck cancer trials. Similar observations in 61 patients treated palliatively at dose rates betw

Recurrent aphthous ulcers in association with HIV infection. Diagnosis and treatment.

Recurrent aphthous ulcers in patients with HIV infection can cause significant morbidity, which makes successful diagnosis and treatment imperative. We have found that the diagnostic paradigm for recurrent aphthous ulcers in HIV-seronegative patients, which is based on the ulcers' clinical appearance, location, absence of other ulcer-causing pathogens or pathogenic processes, and response to therapy, may be successfully applied to recurrent aphthous ulcers in HIV-infected patients. However, one must be alert for ulcers with uncommon causes as well as ulcers with common causes that have atypica

Epidemiologic studies of oral mucosal conditions--methodologic issues.

The methods used in the international English-language literature of epidemiologic investigations of oral mucosal conditions were reviewed. Methods used to study leukoplakia, lichen planus, recurrent herpes labialis, recurrent aphthous ulcers, geographic tongue and candidiasis are highlighted. In addition, studies of the full range of pathologies documented in a population were reviewed. The methodologic issues raised by the epidemiologic literature as well as those to be considered for future studies of oral mucosal conditions are presented. Emphasis is placed on study population selection, d

Performance of a hydroxypropyl cellulose film former in normal and ulcerated oral mucosa.

Although oral ulcers are commonly encountered in clinical dental and medical practice, current therapeutic options with respect to pain relief are limited. This study evaluated the mucosal binding characteristics and the pain relief and protection properties of Zilactin, a hydroxypropyl cellulose film former. In 12 healthy volunteers, the mean duration of mucosal adherence of Zilactin was 3.92 hours, which was significantly longer than that of a widely available topical preparation (1.38 hours, p = 0.0001). When evaluated in 20 subjects with recurrent aphthous ulcerations, Zilactin demonstrate

Disseminated pustular dermatosis in polycythemia vera. Relationship with neutrophilic dermatosis of myeloproliferative disorders: study of neutrophil function.

An unusual case of disseminated pustular eruption associated with polycythemia vera is described. This eruption can be included in the group of neutrophilic dermatoses of myeloproliferative disorders (pyoderma gangrenosum, Sweet's syndrome). A defect in superoxide anion (O2-) generation is detected. The fact that both the clinical condition and the neutrophil function improved with the use of dapsone suggests that the O2- generation defect may play a role in the pathogenesis of the eruption. From an evolutionary standpoint, the occurrence of a neutrophilic dermatosis in polycythemia vera could

Treatment of recurrent aphthous ulcers with Aureomycin mouth rinse or Zendium dentifrice.

Clinical trials with a cross-over double-blind technique were undertaken to test chlortetracycline (Aureomycin) and the enzyme-containing dentifrice Zendium with regard to therapeutic effects on recurrent aphthous ulcers. Aureomycin was found to reduce the number of ulcers and diminish pain when compared with placebo. When groups of patients treated with Zendium and placebo dentifrice, respectively, were compared, no statistically significant difference could be demonstrated. However, when the pH value of Zendium was stepwise changed from 5.9 to 6.8, an increased fraction of patients reported
