Relationship between tubal function, craniofacial morphology and disorder of deglutition.

26 children without hearing impairment have been examined by otolaryngologists and orthodontists. According to the tubal function test in a pressure chamber they were classifed into a group with good and poor tubal function. The E.N.T. examination was unconclusive for a possible relationship between rhinological findings, mode of breathing, sinusitis, size of tonsils, nasal airway resistance and tubal function. Adenoids proved to be a mechanical impairment for active tubal function as stated by many authors. The cephalometric analysis of lateral head films combined with a static and dynamic-fu

Large Palatal Pleomorphic Adenoma Complicated by Dysphagia: A Case Report.

A 60-year-old Japanese woman presented with a palatal pleomorphic adenoma measuring 5 cm in size. In addition to impairments during the oral preparatory and oral transport phases, dysphagia with nasopharyngeal closure disorder was observed in the pharyngeal phase. After resection of the tumor, dysphagia resolved, and the patient was immediately able to eat a standard meal. A videofluoroscopic swallowing study confirmed improvement in the movement of the soft palate compared with the pre-operative condition.

Laparoscopic Heller Myotomy and Dor Fundoplication: How I Do It?

Achalasia is a primary esophageal motility disorder characterized by lack of esophageal peristalsis and partial or absent relaxation of the lower esophageal sphincter in response to swallowing. Available treatment modalities are not curative but rather intend to relieve patient' symptoms. A laparoscopic Heller myotomy with Dor fundoplication is associated with high clinical success rates and low incidence of postoperative reflux. A properly executed operation following critical surgical steps is key for the success of the operation.

[Role of functional rehabilitation in preventive and interceptive orthodontic management].

Orofacial dysfunction and parafunctions are responsible for many orthodontic abnormalities right from the stage of deciduous teeth.Incisor malocclusion, dental malposition and alveolar arch malformations are the most common abnormalities.Functional rehabilitation for prevention or as part of interceptive orthodontics significantly reduces the risk of appearance or progression of these abnormalities to more extensive malocclusions that are more difficult to manage.Functional rehabilitation consists of re-educating and restoring physiological orofacial functions, such as speech and swallowing, b

Esophageal Apoplexy.

Oesophageal apoplexy or intramural haematoma of the oesophagus (IHE) is a rare condition with a pattern of presentation that can mimic sinister pathologies. Diagnosis requires a high index of suspicion, and the exclusion of an oesophageal perforation or oesophageal malignancy is important. Investigations include computer topography (CT) and contrast swallow studies as well as a gastroscopy. Management is mainly supportive with resolution of symptoms typically occurring rapidly. We present two cases of this rare condition, with accompanying images.

The diagnosis and treatment of Rapunzel syndrome.

Trichobezoars are hairballs or hair-like fibers formed by chewing and swallowing hair or any other indigestible materials. Trichobezoars usually form in the gastric body and are thus prepyloric. However, trichobezoars may rarely pass through the pylorus into the duodenum, jejunum, ileum, and even the colon, in a condition referred to as Rapunzel syndrome. Here, we present a case of a 13-year-old girl with this rare syndrome and discuss the diagnosis and treatment of the disease.

Otolaryngologic challenges in cleft/craniofacial care.

An otolaryngology evaluation is an important aspect of care for any patient with craniofacial conditions. Otolaryngologists are commonly involved in the care of these patients to treat chronic otitis media, which frequently requires placement of pressure equalization tubes; however, there are many other otorhinolaryngologic issues that are addressed. Patients with craniofacial conditions may struggle with nasal obstruction, airway compromise, obstructive sleep apnea, dysphonia, swallowing, chronic sinonasal disease, and otologic disease.

Treatment of skeletal open-bite malocclusion with lymphangioma of the tongue.

Lymphangioma of the tongue causes massive tongue enlargement, leading to difficulties in swallowing and mastication, speech disturbances, airway obstruction, and skeletal deformities such as open-bite malocclusion. Early reduction of tongue volume improved the excessive open bite in a young girl, but it was not sufficient to redirect the original hyperdivergent growth pattern. Orthodontic camouflage treatment was therefore rendered. Long-term evaluation after tongue-reduction surgery and orthodontic treatment is presented.

Uncommon case of symmetrical fibrous hyperplasia of the hard palate.

The fibrous hyperplasia of the palate (fibroma) is a benign tumor which has its origin in the soft tissue and whose frequency is estimated to be 1.2% on adult subjects. Rarely the size exceeds the cm, but in these cases a complaint of increasing difficulty of mastication and swallowing appears. We describe the case of a young white woman affected with two peculiar symmetric lesions of the secondary hard palate, that could be the consequence of an abnormal answer to a chronic inflammatory stimulation.

Central pontine myelinolysis.

In 1949, Victor and Adams observed an alcoholic patient who developed quadriplegia and pseudobulbar palsy, and inability to chew, talk or swallow. A post-mortem confirmed their suspicion of 'a large, symmetrical, essentially demyelinative lesion occupying the greater part of the basis pontis.' This paper follows the historical evolution of central pontine myelinolysis and the changing concepts of its metabolic aetiology. Too rapid a rate of correction of hyponatraemia is the most common, but not invariable aetiology.

Impact of birthing practices on the breastfeeding dyad.

For breastfeeding to start and continue, the newborn must be able to suck, swallow, and breathe; the mother must be able and willing to let her infant breastfeed; and surroundings must support the biological unit: the mother-baby dyad. This article reviews how birth practices, including epidural anesthesia, cesarean surgery, forceps, and vacuum extraction, can affect the newborn's ability to feed, the mother's motivation and lactation capacity, and the mother-baby relationship.

Tongue piercing: case report and ethical overview.

A case of infection following piercing of the tongue is presented. A dentist allegedly provided local anesthesia to the tongue prior to piercing. To avoid criticism from the first dentist, the patient saw a different dentist to treat the infection. The patient had symptoms of pain, inflammation, purulence, salivary incontinence, and difficulty speaking and swallowing. The ornament was discarded and the infection was resolved with antibiotic therapy. Ethical issues of administering anesthesia prior to piercing also are discussed.

[Application of prosthetic replacement for bilateral maxillectomy defect].

A 59-year-old woman with large hard palate cancer underwent surgical resection. We constructed a maxillary prosthesis to be connected to the patient's mandibular denture. Bilateral maxillectomy resulted in total loss of the hard palate and bilateral alveolar and partial loss of the soft palate. A obturator prosthesis was prepared beforehand to fit the surgical cavity. The patient recovered functional speech and swallowing at a relatively early stage, achieving satisfactory cosmetic results.

Elongated uvula with a pleomorphic adenoma: a rare cause of obstructive sleep apnea syndrome.

The authors encountered a case of elongated uvula with a pleomorphic adenoma originating from the minor salivary gland, causing frequent upper airway obstruction. A 75-year-old woman had obstructive sleep apnea syndrome brought about by the swallowing impact of the pendulous tumor into the hypopharyngeal and supralaryngeal spaces. Although there are some case reports about tumors originating from the uvula, this is the first reported to cause obstructive sleep apnea syndrome.

The role of fiberoptic endoscopy in dysphagia rehabilitation.

The clinical bedside swallow evaluation and videofluoroscopy routinely have been used for dysphagia management. This article explores clinical issues for dysphagia management in the population with brain injury in the acute rehabilitation setting. Citing the dysphagia, cognitive, and other relevant issues present in the population with brain injury, the use of fiberoptic endoscopy to address dysphagia assessment and intervention is presented. Factors for decision making, a retrospective review of recent studies, and relevant cases are discussed.

Atypical herpes simplex can mimic a flare of disease activity in patients with pemphigus vulgaris.

We present a 69-year-old white woman with pemphigus vulgaris limited to the oral mucosa who presented with oral pain and difficulty swallowing of 2 days duration, followed by multiple irregular ulcers arising from normal mucosa with no grouping of individual lesions--herpes simplex should be considered in the differential diagnosis of lesions that appear suddenly in patients with PV, particularly if the lesions fail to respond to an increased dose of corticosteroids.

Intraoral cheek transposition flap for primary reconstruction of the soft palate.

A method for primary reconstruction of the soft palate following radical tumor resection is described. The resulting defects can be repaired with 2 layers of tissue by combination of a cranial pedicled pharyngeal flap and a cranial pedicled intraoral cheek transposition (IOCT-) flap. For additional resection of the lateral pharyngeal wall the technique can be combined with a masseter crossover flap. The method allows satisfying restoration of swallowing and speech with relatively small operative effort.
