Rehabilitation Approach After Surgical Repair of Spontaneous Bilateral Quadriceps Tendon Rupture in a Healthy Mason.

Quadriceps tendon rupture is a rare lesion, occurring most frequently in males older than 60 yrs after a fall or trauma and associated with systemic diseases and risk factors. Bilateral injury is even more uncommon and disabling. The authors report a case of spontaneous bilateral quadriceps tendon rupture, nonsimultaneous (8 days apart), in a healthy 54-yr-old black male mason, that may be work related. The patient was included in a 6-mo rehabilitation program after surgical repair. Pain control and gait reacquisition were particularly difficult. Following the customized and phased rehabilitat

Double Versus Single Tendon Transfers to Improve Shoulder Function in Brachial Plexus Birth Palsy.

BACKGROUND: In children with brachial plexus birth palsy (BPBP) undergoing tendon transfers to augment shoulder external rotation, it is unclear whether transfer of the latissimus dorsi with its combined latissimus dorsi and teres major (cLT) versus isolated teres major (iTM) tendon transfer yield different outcomes. METHODS: Records of patients with BPBP who underwent shoulder tendon transfers to augment external rotation were retrospectively reviewed. Transfer type (cLT or iTM) was considered indiscriminate by virtue of surgeon preference. Modified Mallet Scale (mMS) and Active Movement Sca

Management of chronic distal biceps tendon ruptures: primary repair vs. semitendinosus autograft reconstruction.

BACKGROUND: Delayed presentation of distal biceps tendon ruptures can make primary repair difficult, in which case reconstruction using a tendon graft is an option. The aim of this study was to compare outcomes and complications between delayed distal biceps tendon ruptures managed with repair vs. semitendinosus autograft reconstruction. METHODS: Nineteen delayed distal biceps tendon rupture cases treated with a tendon reconstruction were compared with 16 delayed primary repair cases (>21 days). The reconstructions were performed using a semitendinosus autograft looped through a transosseous

The fasciocutaneous iliotibial band perforator flap in soft tissue and tendon reconstruction of the foot: A case report.

In cases of large defects of the limbs, post-traumatic deformity and disability can have devastating effects on patients' quality of life. The purpose of this report is to describe the technique for raising a fasciocutaneous iliotibial perforator flap and present its application in the reconstruction of a complex soft tissue defect of the foot. The patient was a 13-year-old male who had suffered a crush injury to the foot in a motor vehicle accident 5 years earlier. Due to retraction of the skin, together with the extensor tendons of the digits, the patient could not flex the digits II, III, I…

Relationship between the architecture and function of ankle plantar flexors with Achilles tendon morphology in ballet dancers.

Achilles tendinopathy is the most frequent foot overuse injury in ballet dancers and knowledge of clinically modifiable factors related to tendon structure in a population at risk, such as ballet dancers, would be important for the development of preventive programs. Therefore, the present study aimed to assess relationships of gastrocnemius muscle architecture and ankle plantar flexors function with Achilles tendon morphology in ballet dancers. Fifty-four measures from 27 ballet dancers were collected. Tendon morphology (thickness, echogenicity, hypoechoic areas and neovascularisation) and mu

Location and Extent of A1, A2 Release and Its Impact on Tendon Subluxation and Bowstringing-A Cadaveric Study.

Surgical treatment of trigger finger involves release of A1 pulley. Some authors have theorized that the loss of A1 pulley can lead to ulnar subluxation of flexor tendons, which can be prevented by release of A1 pulley radially, even in a nonrheumatoid hand. However, there is no evidence in literature to either support or oppose this hypothesis. Occasionally, difficulty is encountered to precisely identify where A1 ends and A2 begins. While incomplete release of A1 can cause relapse of triggering, release of substantial A2 can cause bowstringing. Knowledge of the safe limit of concomitant A2 r…

Giant cell tumor of tendon sheath in the hand: analysis of risk factors for recurrence in 50 cases.

BACKGROUND: Giant cell tumor of the tendon sheath is the most common form of giant cell tumors and is the second most common soft tissue tumor of the hand region after ganglion cyst. Magnetic resonance imaging is the diagnostic tool of choice for both diagnosis and treatment planning. The current standard treatment of choice is simple excision. The main concern about the treatment is related to the high recurrence rates. Besides incomplete excision, there is no consensus concerning the effect of other risk factors on recurrence. The literature lacks detailed reports on surgical excision of the

Non-uniform Stiffness within Gastrocnemius-Achilles tendon Complex Observed after Static Stretching.

Higher stiffness of the Achilles tendon (AT) and gastrocnemius have been found to be risk factors associated with Achilles tendinitis. Static stretching (SS) is one intervention that has been investigated to improve the flexibility and therefore reduce injury risk. Previous studies have investigated the acute effect of SS on one region for AT and gastrocnemius morphology and stiffness; however, there is a lack of knowledge about the effect of SS on other regions of the AT and gastrocnemius (e.g., proximal vs. distal, within gastrocnemius). The aims of the present study were: (1) to investigate

Incidence, Risk Factors, and Treatment of Achilles Tendon Rupture in Patients With End-Stage Renal Disease.

BACKGROUND: Dialysis-dependent patients and kidney transplant recipients may be at increased risk for Achilles tendon rupture (ATR). METHODS: We studied Medicare patients with end-stage renal disease (ESRD) from 1999 through 2013. Patients were categorized as waitlisted for a transplant, not waitlisted, or received a transplant. We performed multivariate negative binomial regression using demographic characteristics, comorbidities, and year of study entry to estimate adjusted incidence rate ratios (aIRRs), identify ATR risk factors, and determine treatment patterns and outcomes. RESULTS: We

Anatomic Basis for Minimally Invasive Flexor Hallucis Longus Transfer in Chronic Achilles Tendon Rupture.

The present cadaveric study was designed to measure the flexor hallucis longus (FHL) tendon length and obtain anatomic data regarding the graft-to-tunnel length ratio in an interference screw fixation model for the FHL short-harvest single-incision technique to the calcaneus. Ten fresh-frozen paired cadaveric specimens were used for the FHL short-harvest technique. The length of the osseous tunnel in the calcaneus was measured. At harvesting of the FHL tendon, the length of the tendon that traverses the osseous tunnel was measured with the ankle in neutral and maximal plantarflexion from the t

Suture and Splint Compared With K-Wire Fixation for Open Zone 1 Extensor Tendon Injuries.

Extensor tendon injuries are classified as per the zone of injury. Zone 1 injuries disrupt the lateral bands with resultant inability to extend the distal interphalangeal joint. Open Zone 1 injuries have many well-described treatment options. To date, none of these have been compared directly. Our aim was to compare the benefit of additional Kirschner-wire fixation with suture repair and splinting of open Zone 1 extensor tendon injuries. We performed a retrospective cohort analysis comparing 2 different surgical procedures, "Suture and Splint" versus "Suture, Splint, and Kirschner wire." The 2

Flexor pollicis longus tendon rupture by sandwiched underlying volar locking plate and distal radius.

Flexor pollicis longus (FPL) tendon rupture is a major complication of volar locking plate fixation for distal radius fractures. The tendon rupture is usually caused by friction between the distal edge of the plate and the FPL tendon, and has been well detected recently with ultrasonography. Rarely, the volar locking plate itself entraps the FPL tendon, leading to its rupture. A 63-year-old man was consistently unable to flex his right thumb after previous surgery for a distal radius fracture at another hospital. Ultrasonography demonstrated loss of tendon gliding and unusual patterns of the F
