[Emergency cesarean in a patient with Coffin-Siris syndrome].

Coffin-Siris syndrome is a rare genetic disease characterized by coarse facial features, sparse scalp hair, hirsutism, hypoplasia of the distal phalanges, hypoplastic nail in the fifth digit, and mental retardation and delayed growth evident in both weight and height. Most cases are sporadic, but the possibility of recessive or dominant autosomal inheritance has been suggested. Facial abnormalities that make intubation difficult and mental retardation that interferes with cooperation are aspects of this disease that can affect the choice of type of anesthesia. We report the case of a parturien

Technique of anaesthesia in pulmonary hypertension and thrombophilia in early pregnancy.

Pulmonary hypertension in pregnancy is a rare condition but is associated with a high mortality. We report the case of a 29 year old female in early pregnancy with Protein C and S deficiency with recurrent deep venous thrombosis and pulmonary embolism and subsequent secondary pulmonary hypertension. The patient was counselled and consented for termination of pregnancy with tubal sterilization. She was administered continuous spinal anaesthesia with invasive monitoring. The successful anaesthetic management of this condition is described.

Neurological long-term sequelae after spinal anaesthesia in a tropical setting: a case control study.

Spinal anaesthesia (SA) is an important form of anaesthesia in tropical countries. It is considered to have few long-term complications or sequelae, although this hypothesis has not been proven in a rural tropical setting. In a case control study we found SA to he a significant risk factor for lower back pain, reflex abnormalities and muscular atrophy and mild impairment of muscle power in patients examined between 3 months and 4 years after SA. These long-term sequelae need to be confirmed in a larger prospective study employing all possible neurophysiological and neuroimaging techniques.

Iatrogenically induced vertebral osteomyelitis due to Pseudomonas aeruginosa.

We report three cases of iatrogenically induced vertebral osteomyelitis due to Pseudomonas aeruginosa. One case was a post-nucleotomy and post-operative complication of herniated disk treatment, one was related to infection of an epidural catheter in a patient suffering from rheumatoid arthritis and the third followed urinary investigation in a patient with prostate cancer. Infection was cured in all patients with antibiotic treatment. These case reports shed light on the possibility of infections with pseudomonas aeruginosa, in addition to the more common infections such as by staphylococci,

Neural mechanisms accounting for the increase in blood pressure and heart rate during vagino-cervical stimulation.

The rise in blood pressure and heart rate produced by the mechanical stimulation of the uterine cervix (VS) was examined after adrenalectomy, after pelvic or hypogastric neurectomies or after spinal cord transection in anesthetized rats. Neither adrenalectomy, nor hypogastric neurectomy prevented the rise in heart rate and blood pressure produced by VS. After the spinal cord transection at T6 level, VS was still able to produce the rise in blood pressure. However, the rise in blood pressure was significantly lower than that produced in the same animals before the transection. No changes in hea

[Postoperative lesions of the peripheral nerves. 8 years' analysis].

Damage to a plexus or peripheral nerve is a rare and avoidable complication of surgical anesthesia. We reviewed 2,750 case histories of patients who underwent surgery between 1985 and 1992, finding 6 cases of nerve lesions presenting postoperatively. Sequelae involved 1 abdomino-genital neuralgia, 1 case of post-epidural radicular pain, 2 cases of peroneal nerve palsies and 2 of cubital paresis. Three of these cases were related to position during surgery, 1 to position during a prolonged period in bed in the intensive care unit, 1 to the anesthetic technique and 1 to surgical manipulation. Responses of cat C1 spinal cord dorsal and ventral horn neurons to noxious and non-noxious stimulation of the head and face.

Previous anatomical studies have shown that trigeminal and cervical afferent nerve fibers project to the upper cervical segments of the spinal cord. To determine the response properties of neurons in the upper cervical spinal cord, we studied the response of C1 dorsal and ventral horn cells to electrical and graded mechanical stimulation of the face, head and neck in anesthetized cats. Neurons were classified as low-threshold-mechanoreceptive (LTM), wide-dynamic-range (WDR), nociceptive-specific (NS) or unresponsive, based on their responsiveness to graded mechanical stimulation. Extracellular

Radiation Disrupts the Protective Function of the Spinal Meninges in a Mouse Model of Tumor-induced Spinal Cord Compression.

BACKGROUND: Recent advances in multidisciplinary treatments for various cancers have extended the survival period of patients with spinal metastases. Radiotherapy has been widely used to treat spinal metastases; nevertheless, long-term survivors sometimes undergo more surgical intervention after radiotherapy because of local tumor relapse. Generally, intradural invasion of a spinal tumor seldom occurs because the dura mater serves as a tissue barrier against tumor infiltration. However, after radiation exposure, some spinal tumors invade the dura mater, resulting in leptomeningeal disseminatio

Analgesic efficacy and functional outcomes of 10 versus 20 mL bupivacaine for popliteal plexus block after total knee arthroplasty: a randomized clinical trial.

BACKGROUND AND OBJECTIVES: The popliteal plexus block is a motor-sparing regional anesthetic technique used as an adjunct to multimodal analgesia after total knee arthroplasty. Clinical studies have used varying local anesthetic volumes for the block, reflecting uncertainty regarding the optimal volume. This randomized clinical trial aimed to determine whether using 20 mL of local anesthetic for the popliteal plexus block provides superior analgesic efficacy and early functional outcomes compared with 10 mL after total knee arthroplasty. METHODS: In this single-center, blinded, randomized con

Intraoperative Lateral Hinge Fractures and Early Postoperative Pain Burden After Medial Opening Wedge High Tibial Osteotomy: Implications for Augmented Postoperative Analgesia.

Background Intraoperative lateral hinge fractures (LHFs) are common complications during medial opening wedge high tibial osteotomy (MOWHTO). While their radiological outcomes are well-studied, their impact on immediate postoperative pain remains underexplored. Purpose The aim of this study was to evaluate the relationship between LHFs and early postoperative pain intensity (visual analog scale [VAS] scores) following MOWHTO. Materials and methods We retrospectively reviewed 244 patients undergoing MOWHTO under spinal anesthesia. Patients were divided into a hinge fracture group (n = 95) and a

Anterior peri-iliopsoas muscle space block for analgesia in lower-limb orthopedic surgery: A randomized controlled trial.

BACKGROUND: A combined lumbar and sacral plexus block may offer unilateral analgesia for the entire lower limb. However, the lumbar plexus block presents challenges due to the plexus's deep location and proximity to vital structures. Moreover, this technique often requires repositioning the patient, which can be painful or risky, especially in cases of multiple or spinal fractures. This study evaluated a novel anterior peri-iliopsoas muscle space block using a single skin puncture for perioperative analgesia in lower limb orthopedic surgery. Perfusion index was assessed as a supportive physiol

Anesthetic considerations in surgery for early onset scoliosis: Challenges and advancements.

Scoliosis correction surgery in pediatric patients poses complex anesthetic challenges due to significant anatomical deformities, cardiopulmonary compromise, and the potential for substantial blood loss. Patients frequently exhibit significant cardiopulmonary compromise due to severe thoracic curvature and rib cage distortion, which impairs pulmonary gas exchange and increases the likelihood of perioperative complications. The severity of Cobb's angle, the number of affected vertebrae, and coexisting syndromes like kyphoscoliosis influence these cardiopulmonary changes and can lead to abnormal

Association between pericapsular nerve group (PENG) block and postoperative inflammatory indices and analgesic outcomes in total hip arthroplasty: a randomized controlled trial.

BACKGROUND: Total hip arthroplasty (THA) induces a pronounced postoperative inflammatory response that contributes to pain, increased opioid requirements, and delayed recovery, particularly in older adults. Although the pericapsular nerve group (PENG) block provides effective analgesia, its impact on systemic inflammation remains insufficiently defined. METHODS: In this prospective, randomized, quadruple-blinded controlled trial, patients aged ≥ 60 years undergoing elective unilateral THA performed under spinal anesthesia were randomized (1:1) to receive an ultrasound-guided PENG block with r

Who Remains Intubated After Adult Spinal Deformity Surgery? A 5-Factor Scoring System With Dual Institution External Validation.

BACKGROUND AND OBJECTIVES: Prolonged intubation is an unwanted occurrence after adult spinal deformity (ASD) surgery, yet its risk factors are poorly understood. For patients undergoing ASD surgery, we sought to: (1) report the rate of intubation until postoperative day 1 (POD 1), (2) create a scoring system to predict it, and (3) externally validate the scoring system. METHODS: A retrospective cohort study (2009-2021) was performed for patients undergoing ASD surgery. Primary outcome was prolonged intubation (POD 1 or beyond). Univariate logistic regression were performed. External validatio

Intrathecal morphine vs. Ultrasound-guided bilateral posterior quadratus lumborum block in caesarean delivery.

BACKGROUND: Effective postoperative pain management is crucial in caesarean delivery (CD) to enhance recovery, minimize opioid use, and improve maternal outcomes. Intrathecal morphine (ITM) is widely used but can cause side effects, such as pruritus and nausea. Posterior quadratus lumborum block (QLB) has emerged as a potential alternative for postoperative analgesia. This study compared the analgesic efficacy and side-effect profiles of ITM and posterior QLB in patients with CD. METHODS: This prospective observational study included parturients who underwent elective CD under spinal anesthes

Comparison between combined suprainguinal fascia iliaca compartment block with intra-articular bupivacaine and suprainguinal fascia iliaca compartment block in total hip arthroplasty: randomized double blinded controlled trial.

BACKGROUND: Hip innervation is complex, with contributions from the lumbar plexus and sacral plexus. In any surgical approach, the nerves are affected in both the lumbar and sacral plexus. Fascia iliaca compartment block (FICB) could be effective analgesia in total hip arthroplasty (THA), but some patients require morphine for breakthrough pain. This may be due to FICB only blocking the anterolateral aspect of the hip joint, failing to block the posterior aspect. However, blocking the sacral plexus may lead to adverse effects, such as delayed ambulation and the risk of foot drop. Intra-articul

The Use of Intraoperative Neuromonitoring in Routine Percutaneous Spinal Cord Stimulator Surgery is Not Associated With Improved Placement, Patient Safety, or Pain Severity Outcomes.

BACKGROUND AND OBJECTIVES: The placement of spinal cord stimulators (SCSs) relies on midline placement to provide bilateral coverage. Implantation under general anesthesia using intraoperative neuromonitoring (IOM) has been shown to be a valid alternative to awake placement to confirm positioning. It has been suggested that placement should be confirmed with awake testing or IOM for reasons of efficacy and safety. No study has compared operative outcomes in percutaneous SCS surgery with our without the use of IOM. METHODS: A single-center retrospective chart review was performed, identifying

Early Postoperative Mortality Risk Factors and Five- and Ten-Year Mortality Rates After Hip Arthroplasty for Femoral Neck Fracture.

Introduction: Femoral neck fractures in older adults are associated with appreciable short-term mortality, yet long-term survival after hip arthroplasty is incompletely characterized. We analyzed early mortality risk factors and 5- and 10-year mortality after hemi-arthroplasty or total hip arthroplasty (THA) for femoral neck fractures. Materials and Methods: In this single-center retrospective cohort, 397 consecutive patients underwent arthroplasty for femoral neck fracture in 2014 and 2015. Mean age was 83.3 years and 70.3% were women. Demographic data, Charlson Comorbidity Index, Parker Mobi
