Older Adults' Perspectives on Clinical Research: A Focus Group and Survey Study.

OBJECTIVES: Clinical trials can benefit from patient perspectives to inform trial design, such as choice of outcome measures. We engaged older adults in focus groups and surveys to get their perspective regarding needs in clinical research. The goal was to inform the development of a new clinical trial of medication strategies for treatment-resistant depression in older adults. METHODS: Older adults with depression participated in focus groups and a subsequent survey in St. Louis and New York. They were queried regarding research design features including outcomes, clinical management, mobile

The UK clinical research network--has it been a success for dermatology clinical trials?

BACKGROUND: Following the successful introduction of five topic-specific research networks in the UK, the Comprehensive Local Research Network (CLRN) was established in 2008 in order to provide a blanket level of support across the whole country regardless of the clinical discipline. The role of the CLRN was to facilitate recruitment into clinical trials, and to encourage greater engagement in research throughout the National Health Service (NHS). METHODS: This report evaluates the impact of clinical research networks in supporting clinical trials in the UK, with particular reference to our e

Oral lichen planus interactome reveals CXCR4 and CXCL12 as candidate therapeutic targets.

Today, we face difficulty in generating new hypotheses and understanding oral lichen planus due to the large amount of biomedical information available. In this research, we have used an integrated bioinformatics approach assimilating information from data mining, gene ontologies, protein-protein interaction and network analysis to predict candidate genes related to oral lichen planus. A detailed pathway analysis led us to propose two promising therapeutic targets: the stromal cell derived factor 1 (CXCL12) and the C-X-C type 4 chemokine receptor (CXCR4). We further validated our predictions a

Reassessing Viral Gene Therapy for Glioblastoma: Translational Opportunities and Challenges.

Glioblastoma is a highly invasive primary brain tumor with a remarkably poor prognosis. Resistance to the current gold standard treatment, maximal safe surgical resection followed by radiation and chemotherapy, has led to an investigation into novel therapeutic alternatives, notably gene therapy. This perspective evaluates the translational potential of three prominent viral gene therapies for glioblastoma, highlighting the preclinical challenges facing CRISPR-Cas9-mediated gene editing, suicide gene therapy, and oncolytic virus therapy. We assert that clinical translation depends strongly on

Immune and inflammatory radiogenomics of radiotherapy-induced normal-tissue toxicity: genetic susceptibility, mechanisms, risk prediction, and clinical translation.

Radiotherapy is indispensable for cancer control, yet acute and late normal-tissue toxicities remain major limits to treatment intensity and survivorship. Considerable interindividual variation persists after clinical and dosimetric factors are considered, motivating radiogenomic studies of inherited susceptibility. This review compares common and rare germline variation, association and sequencing designs, polygenic risk scores, and functional evidence through an immunological framework. Radiation-induced DNA damage and organelle stress release danger-associated signals and cytosolic DNA that

Research Progress on BRCA1/2 Mutations in Sporadic Gastric Cancer: Risk Stratification, Surgical Prognosis, and Individualized Treatment.

Gastric cancer is one of the most prevalent malignant tumors worldwide. Sporadic gastric cancer accounts for more than 80% of all gastric cancer cases, and its pronounced heterogeneity underlies the substantial variability in clinical outcomes and the complexity of treatment strategies. The breast cancer gene 1 (BRCA1) and breast cancer gene 2 (BRCA2) are core regulators of the DNA damage homologous recombination repair (HRR) pathway, and their pathogenic mutations are closely associated with hereditary breast and ovarian cancer syndrome. Recent evidence has shown that BRCA1/2 mutations also e

Non-coding RNAs as biomarkers and therapeutic targets in pancreatic cancer: Clinical implications and translational perspectives.

Pancreatic cancer remains one of the most lethal malignancies, largely due to late diagnosis, marked tumor heterogeneity, and profound resistance to therapy. Beyond recurrent alterations in protein-coding genes, growing evidence identifies non-coding RNAs (ncRNAs) as key regulators of pancreatic cancer biology and promising tools for clinical translation, particularly in pancreatic ductal adenocarcinoma (PDAC). ncRNAs, including microRNAs, long non-coding RNAs (lncRNAs), circular RNAs (circRNAs), and emerging short RNA species such as small nucleolar RNAs, PIWI-interacting RNAs, and tRNA-deriv

Dopamine D2 Receptor Agonists as Modulators of VEGF-A-Driven Angiogenesis: Mechanisms, Clinical Evidence, and Translational Opportunities.

Angiogenesis mediated by vascular endothelial growth factor A (VEGF-A) is essential for physiological vascular remodeling but also drives pathological processes, including tumor growth, ocular neovascularization, and inflammation. Emerging evidence has revealed that dopamine D2 receptor (DRD2) activation is a key inhibitory pathway that counterbalances VEGF-A-dependent endothelial activation and vascular permeability. This review integrates current mechanistic insights into the effects of DRD2 agonists on endothelial signaling, focusing on their ability to suppress VEGF-A-induced proangiogenic

Comparing the Limbic-Frontal Connectome across the Primate Order: Conservation of Connections and Implications for Translational Neuroscience.

The interaction of the limbic system and frontal cortex of the primate brain is important in many affective behaviors. For this reason, it is heavily implicated in a number of psychiatric conditions. This system is often studied in the macaque monkey, the most largely used nonhuman primate model species. However, how evolutionary conserved this system is and how well results obtained in any model species translate to the human brain can only be understood by studying its organization across the primate order. Here, we present an investigation of the topology of limbic-frontal connections acros

Translating Observational Data Into Public Health Action: Reducing Early Childhood Caries Burden Related to Socioeconomic Disparities.

OBJECTIVES: The "valley of death" between research and practice remains a major challenge. The five-stage Translational Research framework (TR1-TR5) provides a pathway, moving evidence into real-world impact. Traditionally, Randomised Controlled Trials (RCTs) dominate TR stages, yet they are often impractical in public health. Longitudinal observational studies (LOSs), combined with advanced causal statistics, can emulate RCTs. Using the relationship between socioeconomic status (SES) and early childhood caries (ECC) as a case study, where SES is a complex, upstream determinant lacking trial e

Histamine H3 Receptor as a target for alcohol use disorder: challenging the predictability of animal models for clinical translation in drug development.

There is an important need to advance medication development for alcohol use disorder (AUD). BP1.3656B, a highly potent and selective histamine H3 receptor inverse agonist/antagonist, has been developed. Preclinical studies revealed high affinity, good pharmacokinetic profile, good brain penetration, and favorable safety. BP1.3656B reduced alcohol drinking and alcohol-seeking behavior in rodents. Phase I studies revealed good tolerability/pharmacokinetic in humans. Positron emission tomography revealed high brain occupancy in humans. Based on this favorable profile, two trials were conducted i

Liver xenotransplantation: Progress, mechanistic barriers and strategies for clinical translation.

Pig-to-human liver xenotransplantation holds promise for alleviating the severe shortage of human donor livers. In recent years, China has achieved milestone progress in this field. Livers from gene-edited pigs have been successfully transplanted into brain-dead human recipients and a living patient with advanced hepatocellular carcinoma. These advancements demonstrate that a combination of genetic engineering and immunosuppressive protocols can effectively mitigate xenogeneic immune rejection. Furthermore, the transplanted porcine livers were able to perform key liver-specific physiological f

Patient Trajectories with High-Frequency PROMs Integrated into EHR Systems: A Comprehensive Outcome for Clinical Research and Care.

This study assessed the technical and clinical feasibility of capturing detailed, high-frequency Patient-Reported Outcome Measure (PROM) trajectories by integrating short instruments directly into the Electronic Health Record (EHR) system. Current practice uses infrequent PROMs, failing to capture the dynamic nature of a patient's trajectory. We designed and implemented an electronic data capture system that securely sent single-item Quality of Life (QoL) and pain assessments to patients' smartphones for direct EHR integration. The system was tested in cohorts from neurosurgery (182 patients,

Orosomucoid 2 as an immunometabolic regulator in cardiometabolic disease: Molecular mechanisms and translational potential.

Cardiometabolic diseases are driven by persistent crosstalk between metabolic dysfunction and chronic low-grade inflammation. Orosomucoid 2 (ORM2), a highly glycosylated acute-phase protein of the α1-acid glycoprotein family, has conventionally been considered a circulating inflammatory marker. Recent evidence, however, suggests that ORM2 may also function as an active immunometabolic regulator linking hepatic stress responses, adipose tissue inflammation, macrophage polarization, and systemic metabolic homeostasis. This review summarizes the molecular characteristics, tissue distribution, gly

Cholinergic reflex control of inflammation: mechanistic and translational advances in transcutaneous auricular vagus nerve stimulation across rheumatic, metabolic, and postoperative disorders.

Transcutaneous auricular vagus nerve stimulation (taVNS) has recently emerged as a focal noninvasive neuromodulatory approach in anti-inflammatory therapeutics. The vagus nerve functions as a critical neuroimmune interface, tonically suppressing proinflammatory cytokine release via the cholinergic anti-inflammatory pathway (CAP). This mechanism provides substantial therapeutic potential across a spectrum of inflammatory disorders, including postoperative systemic inflammation. Clinical trials have demonstrated the anti-inflammatory efficacy of taVNS, supporting its expanded use in rheumatoid a

Corneal Endothelial Progenitors for Ocular Regeneration: Translating Discovery into Clinical Therapies.

The corneal endothelium is essential for maintaining corneal transparency through active fluid transport and barrier function. Corneal cell loss from disease, ageing or surgical trauma underlies a significant proportion of corneal blindness worldwide, with Fuchs' endothelial corneal dystrophy (FECD) and pseudophakic bullous keratopathy (PBK) representing the dominant clinical indications for corneal transplantation. While Descemet's membrane endothelial keratoplasty (DMEK) has substantially improved surgical outcomes, the procedure remains constrained by global donor tissue shortage. Regenerat
