The unstable distal radial fracture one year post Kapandji intrafocal pinning.

Kapandji pinning has been proposed as the treatment of choice for unstable Colles' fractures. The aim of this paper is to evaluate our experience treating unstable Colles' type fractures using this technique. Over a nine month period, 36 patients with Colles' type fractures were treated operatively at St. Vincent's Hospital. 22 of these fractures were deemed unstable and were treated using percutaneous intrafocal Kapandji pinning. 20 of these patients were recalled for review at a mean of 11.3 month post injury. At this stage the wrist was examined clinically and radiologically. Initial satisf

Surgical correction of spinal deformity using a unit rod in children with cerebral palsy.

Thirty-one patients with cerebral palsy and neuromuscular scoliosis underwent instrumentation with a unit rod fixed with sublaminar wires and posterior spine fusion. The mean curve measured 79 degrees preoperatively, 19 degrees immediately postoperatively, and 18 degrees at final follow-up of 2.8 years, excluding two patients who died and four who were lost to follow-up after < 12 months. The preoperative pelvic obliquity was 25 degrees, which was initially corrected to 3 degrees and remained unchanged at 4 degrees at final follow-up. Twenty-four patients underwent a one-stage posterior fusion

Treatment of intercondylar and supracondylar distal femur fractures using the GSH supracondylar nail.

From April 1990 to August 1994, the GSH supracondylar intramedullary nail (Smith & Nephew/Richards, Memphis, Tennessee) was used to treat 23 supracondylar femur fractures in 22 patients. Nineteen of the 23 fractures were highly comminuted and intra-articular. Fifteen patients with 16 fractures were followed up to healing or declaration of nonunion for an average of 10 months. All fractures were treated with direct reduction through a median parapatellar incision. Fixation of the intercondylar fractures was with 6.5-mm cannulated screws, and the GSH nail was then inserted between the screws to

Treatment of segmental femoral shaft fractures.

In a prospective study, 35 segmental femoral shaft fractures were treated either with closed intramedullary nails (most were Grosse-Kempf interlocking nails) or an open method technique (most were open Küntscher nails with supplementary wires). The follow-up period was at least one year. There was an 82.9% (29/35) union rate and a union period of 6.1 +/- 1.9 months. Knee range of motion was on average 125 degrees. Although there was no statistical difference, the closed intramedullary nailing group showed superiority to the open method group. Complications were not particularly different from

[Fracture of the femoral diaphysis in children. Ascending or descending centro-medullary nailing? A choice of principle or of necessity?].

Over a period of 4 years, 40 children between the ages of 6 and 14 years (mean: 10 years) were treated for a fracture of the femoral shaft (39 closed fractures and 1 Cauchoix type I open fracture) by flexible medullary nailing, either ascending (5 cases) or descending (35 cases). Although it is classical to use the ascending route described by the Nancy team, the sub-trochanteric descending route was preferred for its simplicity; non-involvement of the knee by the insertion of the nail allows earlier autonomy in children. The results have been found to be satisfactory; no complications related

Intramedullary rod fixation of femoral shaft fractures: comparison of open and closed insertion techniques.

Meta-analysis of published series of intramedullary rod fixation in fractured femurs revealed significantly higher union rates, lower deep infection rates and a better range of knee motion when closed rather than open techniques of insertion were used. A separate retrospective review of 58 femoral fractures at one hospital showed outcomes consistent with those reported in the literature; these results were obtained during a 6-year period when staff were learning closed techniques. Technical failures of the closed technique can be avoided by paying attention to well-established operative detail

Secondary arthroplasty for complications of femoral neck fracture.

We report a series of 640 consecutive cervical hip fractures which were followed prospectively for two years after primary internal fixation with two hook-pins. Secondary arthroplasties were performed as salvage procedures in 75 cases and the early outcome of these was studied retrospectively. The mean time in hospital was 25 days for prosthetic replacement, though 60% of the patients had other medical conditions considered as risk factors. Mortality was 5% after six months and 8% after one year. Dislocation was seen in 11% and additional surgery was required in 4%. There was one case of deep

Tumors of the thoracic and lumbar spine: surgical treatment via the anterior approach.

We are reporting our experience in 23 patients with tumors of the thoracic or lumbar vertebrae treated via surgical anterior decompression and stabilization. Seventeen patients had metastatic disease and were treated with vertebral body resection followed by stabilization with anterior polymethylmethacrylate and threaded Harrington rods with sacral distraction hooks. Six patients had primary tumors and, following tumor resection and partial vertebral body resection, had autogenous bone graft struts placed anteriorly as well as posterior instrumentation. Posterior instrumentation was transpedic

Ender nailing of acute fractures of the humerus. A study of closed fixation by intramedullary nails without reaming.

A prospective study of closed intramedullary fixation of fractures of the shaft of the humerus, using Ender nails, was performed over a six-year period. Eighty-nine fractures in eighty-eight patients were treated with no immobilization postoperatively. Three patients were lost to follow-up. Eighty-five of the remaining eighty-six fractures healed, the average time to clinical union being 7.2 weeks. Non-union of one fracture occurred and there were no infections or malunions. Six of the nine preoperative and two postoperative radial-nerve palsies were lesions in continuity and healed spontaneou

Slipped capital femoral epiphysis: an analysis of 80 patients as to pin placement and number.

A retrospective study of 80 patients with chronic slipped capital femoral epiphysis was performed, analyzing the clinical results with regard to pin placement and pin number. Follow-up averaged greater than 2 years, with 86% of the 80 patients obtaining a satisfactory clinical outcome. Serious complications occurred in 10 patients. The severity of complications increased as the number of pins used increased (p less than 0.07). A varus pin position resulting in a more inferior pin placement in the proximal femoral epiphysis was found to be associated with the fewest complications. When the pin

Biomechanical evaluation of biodegradable zinc and polylactide pins for radial head fracture fixation: a human cadaver study.

BACKGROUND: Displaced or unstable radial head fractures are commonly treated with fragment-preserving fixation to restore joint congruity, maintain stability, and allow early mobilization. Biodegradable implants have become attractive alternatives to metallic implants. However, polymer-based pins may be limited by inferior mechanical strength and a higher risk of secondary loss of reduction. Recently, zinc pins have emerged as a potential alternative, but clinical data and biomechanical testing in human bone models are scarce. This study aimed to compare the biomechanical performance of zinc p

Anatomical structure of the medullary cavity of proximal femur with three-dimensional computed tomography.

BACKGROUND: The lateral femoral wall is an important anatomical parameter of the proximal femur, but intramedullary nail fixation for intertrochanteric fractures may cause iatrogenic lateral wall fractures due to population-based design differences. This study aims to measure the anatomical parameters of the proximal femoral medullary cavity and provide data to help design intramedullary nails tailored to the Chinese population to reduce the risk of complications such as lateral wall fractures. METHODS: Consecutive patients undergoing full-length or upper half CT scans of the femur were inclu

Transtrochanteric vs. posterolateral approaches in hemiarthroplasty after proximal femoral nail complications: A comparative study.

BACKGROUND: Proximal femoral nailing is a widely used technique for treating intertrochanteric femur fractures but may result in complications requiring revision surgery. Hemiarthroplasty is a common solution, with the transtrochanteric and posterolateral approaches being two surgical options. This study aimed to compare these approaches in terms of surgical outcomes and complication rates. METHODS: A retrospective analysis was conducted on 79 patients who underwent revision hemiarthroplasty for complications related to proximal femoral nailing between 2019 and 2022. Patients were divided int

Mortality following periprosthetic and peri-implant femoral fractures: comparative analysis according to previous implant type.

INTRODUCTION: Periprosthetic and peri-implant femoral fractures (PPF) include periprosthetic hip fractures (periPC), periprosthetic knee fractures (periPR), as well as peri-implant fractures associated with an osteosynthesis nail (periCL). Although risk factors for PPF are widely studied, there is scarce evidence on differential risk factors for each subtype. The objective of this study is to compare 30-day and 1-year mortality rates among the three PPF subtypes and identify differential risk factors between them. METHODOLOGY: A retrospective cohort study conducted in a tertiary hospital, inc

Comparison of transverse and longitudinal incisions in tibia intramedullary nailing: Implications for pain, numbness, and functional recovery.

BACKGROUND: The current standard of care for surgically managed tibia shaft fractures is intramedullary nailing. Chronic postoperative knee pain is the most common complication of the infrapatellar technique in intramedullary nailing. Literature suggests it may be related to infrapatellar nerve injury. OBJECTIVE: This study compares the incidence of anterior knee pain after tibia nail insertion between conventional longitudinal and transverse incisions. METHODS: This clinical trial study was conducted in our Level I trauma center from February 2022 to August 2024. Patients with closed diaphy

Does a gentamicin-coated intramedullary nail prevent postoperative infection in Gustilo type I and II tibial open fractures? A comparative study and retrospective analysis.

PURPOSE: Tibial open shaft fractures are very common and susceptible to infection, which can lead to significant morbidity especially infection and non-union. Antibiotic coated nail is one option for fixing open shaft tibial fractures to minimise infection. This study aimed to compare the clinical outcome of Gentamicin-coated tibial nails versus regular unreamed interlocking tibial nails in the treatment of type I and II tibial open fractures. METHODS: Between 2013 and 2020, in a retrospective study of 124 patients with Gustilo type I and II tibial fractures compared non-antibiotic-coated nai

Pins and Rubbers Traction System for Treatment of Intraarticular Fractures of Distal Interphalangeal Joint.

Background: The pins and rubber traction system (PRTS) has proven effective in managing intra-articular fractures of the proximal interphalangeal joint. However, there is scant evidence in the literature regarding its efficacy in treating distal interphalangeal joint (DIPJ). This study aims to investigate the outcomes of PRTS in the treatment of comminuted intra-articular fractures of the DIPJ. Methods: We conducted a retrospective review of patients with comminuted intra-articular fractures of the DIPJ treated with PRTS between 2017 and 2021. At the final follow-up, we measured and compared t
