Acute scaphoidectomy and four-corner fusion for the surgical treatment of trans-scaphoid perilunate fracture dislocation with pre-existing scaphoid non-union.

This paper presents a rare case of trans-scaphoid perilunate fracture dislocation with concurrent scaphoid non-union of the left wrist following a motorcycle accident. Emergent CT identified scaphoid non-union advanced collapse and an acute scaphoidectomy, four-corner fusion, denervation and radiocarpal ligament repair was performed.

An alternative patellar fracture fixation: a biomechanical study.

OBJECTIVES: The purpose of this study is to compare open reduction and internal fixation of the patella with a locking plate and tension-band construct (PF) versus cannulated screws and tension-band fixation (SF). The hypothesis is that both constructs will have similar failure loads with simulated extension loading. METHODS: Transverse patellar fractures were created in 10 cadaveric pairs of legs and were fixed with either PF or SF. Dual-energy X-ray Absorptiometry (DXA) scans of all calcanei measured bone mineral density (BMD). Using an MTS 810 servohydraulic testing machine, each leg cycle

The role of locking technology in the hand.

Locked fixed-angle plating in the hand and wrist helps to optimize outcomes following surgical fixation of select acute fractures and complex reconstructions. Select indications include unstable distal ulna head/neck fractures, periarticular metacarpal and phalangeal fractures, comminuted/multifragmentary diaphyseal fractures with bone loss (ie, combined injuries of the hand), osteopenic/pathologic fractures, nonunions and corrective osteotomy fixation, and small joint arthrodesis. Locked plating techniques in the hand should not be seen as a panacea for wrist and digital acute trauma and dela

In scaphoid non-union, does the source of graft affect outcome? Iliac crest versus distal end of radius bone graft.

Iliac crest bone grafts are sometimes preferred to other bone grafts for the treatment of non-unions of fractures of the scaphoid as they are claimed to have better osteogenic potential and biomechanical properties. We retrospectively studied a consecutive cohort of 68 symptomatic established scaphoid non-unions treated by bone grafting. An iliac crest graft was used in 44 cases and a distal radius graft in the other 24. The two treatment groups were comparable in terms of location of the fracture, duration of the non-union and the fixation implants used. Overall union was achieved in 45 of th

Use of a magnetic intramedullary nail for axial compression in endoprosthetic reconstruction of the humerus: a description of technique.

Pathological fractures, both completed and impending, of the humeral diaphysis from primary or metastatic disease demand a reconstruction that restores stability while preserving shoulder function and alleviating pain. Conventional reconstructive approaches include the use of long-stemmed endoprosthetic devices, plate-and-screw fixation or intramedullary nailing, all of which can be limited in their success when residual bone is short or biologically weak. This case study details the management of a patient with metastatic leiomyosarcoma who presented with refracture through a previously cemen

Treatment of osteoblastoma at C3-4 in a child: a case report.

BACKGROUND: Osteoblastoma is a rare and benign osteoid-producing primary bone tumor that affects mainly the long bones. 36% of these tumors are observed around the spine and the vast majority arises around the posterior. CASE PRESENTATION: This report describes a case of C3-4 osteoblastoma occurring in a 5-year-and-8-month-old Han Chinese child. The pathophysiology of symptom development, evaluations, and management are presented. Because of the close proximity of the osteoblastoma to the vertebral artery canal, the artery suffered a minor laceration intraoperatively. Hemostatic gelatin spong

Transgingival lag-screw osteosynthesis of alveolar process fracture.

Fracture of the alveolar process is a common injury. In some cases, traditional fixation may not be possible. The teeth needed for splinting or mandibulo-maxillary fixation may be missing. The fracture line and soft tissue injury may jeopardize the blood supply of the broken bone. In these extreme and rare situations, the best rehabilitation is needed to avoid the loss of hard and soft tissues, and a secondary reconstruction is required. Between January 2003 and December 2006, of 468 cranio-facial trauma patients studied, alveolar process fracture was reported in 28 (6%) cases. In six (1%) cas

[The influence of the traction on the wrist in the course of healing of the radius distal part comminuted fractures].

Traction and external fixation are the methods of choice by treating the comminuted fractures of the distal part of the radius. Bringing in two screws into the second metacarpal bone and the radius proximity from the fracture makes possible the traction and fixate the fractured bone splinters. The reposition of the comminuted fractures needs a very strong traction and provides to expand the ligaments and the wrist articular capsula. The aim of our study is to explain the mechanical influence of the external stabilization on the wrist and the wrist function after the immobilisation with the dis

Fractures and nonunions of the scaphoid.

Fractures of the scaphoid are the most common of all fractures of the carpal bones. Scaphoid collapse and angular deformity may develop after fractures. Scaphoid nonunion often complicates scaphoid fractures especially if the diagnosis is missed initially, and adequate timely treatment is not provided. Displaced unstable fractures also have a tendency to develop nonunion. The tenuous vascular anatomy of the scaphoid, the inherent biomechanical instability of scaphoid fractures, and the difficulty of radiographic diagnosis are the culprits of nonunion. Established scaphoid nonunion is a challen

Distal interphalangeal joint arthrodesis: an analysis of complications.

One hundred thirty-nine patients underwent 181 arthrodeses of finger distal interphalangeal joints (144) and/or thumb interphalangeal joints (37). Techniques included (1) crossed Kirschner pins (111 joints), (2) interfragmentary wire and longitudinal Kirschner pin (43 joints), and (3) Herbert screw (27 joints). Each technique had a similar nonunion rate. There were 21 nonunions: 13 were pain free, 6 were successfully fused on the second attempt, 1 was painful (but the patient refused further surgery), and 1 was amputated. Inadequate bone stock, inadequate bone resection, premature pin removal,

Antegrade intramedullary nail and plate combination technique for proximal humeral non-union: A case report.

Proximal humeral nonunion remains a challenging condition, particularly after failed plate fixation, because of the compromised bone stock within the humeral head and loss of medial calcar support. Under these conditions, it is often difficult to achieve sufficient mechanical stability while preserving the humeral head. We report a case of proximal humeral nonunion successfully treated using the nail and plate combination technique. A 61-year-old man sustained a high-energy proximal humeral fracture in a motorcycle accident and underwent initial plate fixation at a different institution. The p

A customized large single-piece bifrontal implant for post-craniectomy defect reconstruction: A case study.

BACKGROUND: Large bifrontal defects pose unique reconstruction challenges due to their complex curvature and mechanical requirements. This case demonstrated how computer-aided design/manufacturing (CAD/CAM) enabled precise single-piece polymethyl methacrylate (PMMA) implant fabrication, thereby overcoming traditional limitations. CASE PRESENTATION: A 25-year-old male who had undergone bifrontal decompressive craniectomy suffered a severe traumatic brain injury. The autologous bone flap had been temporarily stored in a subcutaneous fat area of the abdomen for 3 months to preserve its viability

Impact of augmentation strategy variations on the mechanical characteristics of patients with osteoporotic proximal humerus fractures with medial column instability.

INTRODUCTION: Low bone density and lack of medial support are the two most important factors affecting the stability of locking plate fixation for osteoporotic proximal humeral fractures (PHFs). This study aimed to compare the biomechanical characteristics of PHILOS locking plates combined with calcar screws, bone cement, fibular allografts, and medial locking plate support strategies for treating osteoporotic PHFs with medial column instability. METHODS: A three-part osteoporotic PHF (AO 11-B3.2) model with metaphyseal loss was generated using 40 synthetic humeri and fixed via four distinct

A Cost-Effective Strategy for Mandibular Reconstruction: Fibula Flap With Existing Titanium Plates.

With the rapid advancement of digital and microsurgical techniques, immediate autologous bone grafting following resection of benign and malignant mandibular tumors has become a standard treatment. However, some patients are unable to undergo immediate autologous bone grafting due to various reasons. In such cases, titanium plate implantation (or combined with soft tissue free flap such as anterolateral thigh flap) serves as an alternative to prevent airway compression and maintain facial shape and function. This article reports a case of a patient, referred to as patient 1, who experienced a

Vascularized Bone Grafts for Spinal Fusion-Part 2: The Rib.

BACKGROUND: Pseudoarthrosis, or failure to achieve bony union, is a well-known complication of spinal fusion operations. Rates range from 5% to 40% and are influenced by both patient and technical factors. Patients who do not achieve complete fusion may experience a return or worsening of their preoperative pain. For patients with complicated pathologies, vascularized bone grafts (VBGs) have been shown to provide better outcomes than nonvascularized bone grafts (N-VBGs). OBJECTIVE: To enhance an instrumented spinal fusion by the innovative technique presented herein that utilizes a rotated, p

Fibrous dysplasia of the cervical spine presenting as a pathological fracture.

Fibrous dysplasia (FD) is an uncommon benign fibro-osseous abnormality of bone, of unknown aetiology and equal sex incidence, which is most commonly restricted to one bone (monostotic FD: MFD, 70%). Less commonly multiple bones are affected (polyostotic FD: PFD, 27%). Vertebral involvement is uncommon (4%), but more common with PFD (7 - 24%) than MFD (1%). Of 20 cases of FD involving the cervical spine, only three have represented MFD. Unlike cases associated with PFD, all cases presented with acute neck pain without significant neurological impairment after minor trauma. We present the case o
