How I do it: dual operation channels percutaneous endoscopic far-lateral transforaminal lumbar interbody fusion.

BACKGROUND: The current surgical procedure of interbody fusion in the lumbar spine has several limitations including low efficiency, potential endplate damage, overdose radiation exposure, and failure of fusion. METHODS: Through the endoscopic operating channel, we efficiently removed the superior and inferior articular processes and decompressed the ligamentum flavum. Another operating channel was established under endoscopic monitoring to excise the annulus fibrosus, remove the cartilaginous endplate using open instruments, perform interbody bone grafting, and place a non-expandable polyeth

Bone marrow edema syndrome/transient osteoporosis of the hip joint and management with the utilization of hyperbaric oxygen therapy.

Transient osteoporosis of hip (TOH) is a self-limited condition that affects young and middle aged women or men. The clinical manifestation is characterized by sudden or gradual onset of pain that is exacerbated on weight-bearing and at night. The treatment of TOH is either conservative or surgical. Conservative regimens include restriction of weight-bearing, pharmacological agents such as prostacyclin analogues (iloprost), non-steroid anti-inflammatory drugs (NSAIDs), extracorporeal shock wave and hyperbaric oxygen therapy (HBOT). Surgical intervention includes drilling or core decompression.

Surgical Therapy.

Many prior investigations have indicated the important role of medical treatment to prevent stroke in patients with intracranial atherosclerosis, with angioplasty and stenting occasionally being performed. In a subgroup of patients with severe hemodynamic impairment, extracranial-intracranial (EC-IC) bypass surgery may be considered. Additionally, in patients with massive infarctions due to middle cerebral artery (MCA) occlusion, the use of decompressive craniectomy may lower mortality rates and improve long-term quality of life. However, the benefit of these surgical procedures in patients wi

[Surgical interventions for ameliorating of equilibrium disorders refractory to conservative therapy].

In cases of vertigo resistant to conservative treatment, surgery can be valuable for labyrinthine fistula, perilymphatic fistula, cupulolithiasis and Menière's disease. For instance, covering a labyrinthine fistula by a fascial graft relieves the vertigo in an amazingly short time. Perilymphatic fistula is verified and treated by tympanotomy and is used for patients with special auditory and/or vestibular features. Cupulolithiasis usually requires no surgical treatment: special postural training will speed recovery, but a few cases need neurectomy of the inferior vestibular nerve. There are se

Back To Basics at The Craniovertebral Junction: Occipito-Cervical Fusion for Basilar Invagination with Atlantoaxial Instability.

INTRODUCTION: Basilar invagination (BI) with atlantoaxial instability is a complex craniovertebral junction disorder that causes progressive cervicomedullary compression and disabling myelopathy. Although reduction-based techniques such as distraction-compression-extension-reduction (DCER) have expanded surgical options, conventional occipitocervical fusion remains an important stabilization strategy in selected patients with rigid deformity, advanced neurological compromise, or infective craniovertebral junction pathology. MATERIALS AND METHODS: This retrospective case series included three

Minimally Invasive Decompression and Fusion With Percutaneous Facet Cages vs Traditional Lateral Mass Screw Fixation for Cervical Spondylotic Myelopathy: A Retrospective Comparative Study.

BACKGROUND Comparative clinical data evaluating percutaneous facet cage (PFC)-based minimally invasive decompression against traditional laminectomy and lateral mass screw (LMS) fixation in patients with cervical spondylotic myelopathy (CSM) remain limited. We compared the clinical and radiological outcomes of these 2 surgical strategies. MATERIAL AND METHODS We retrospectively reviewed 76 patients with CSM operated on between 2020 and 2024. Patients were categorized into 2 groups: the LMS group (n=46): laminectomy with LMS fusion; and the PFC group (n=30): speculum-assisted laminotomy with PF

Factors that influence long term instrumentation stability in patients with unstable thoracolumbar injuries.

BACKGROUND: Pedicle screw fixation is frequently used to treat unstable thoracolumbar injuries; however, the rate of instrumentation failure remains considerable. The primary contributing factor leading to instrumentation failure is poor bone quality. On the other hand, some evidence suggests that surgical tactics can influence long-term instrumentation stability. AIM: To assess factors that influence the stability of spinal instrumentation in patients with thoracolumbar injuries. METHODS: This study is a non-randomized single center ambispective evaluation of 204 consecutive patients (117 m

Emergency surgical intervention for bulbar-cervical spinal cord hemorrhage: a case report and review of management strategies.

BACKGROUND AND IMPORTANCE: Spinal cord hemorrhage (SCH), particularly involving the bulbar-cervical segment (medulla oblongata to C7), is a rare and life-threatening neurological emergency. Due to its anatomical proximity to respiratory, motor, and sensory centers, it often leads to catastrophic neurological deficits. Etiologies include vascular malformations, coagulopathy, or idiopathic causes, yet its low incidence (<1%) poses significant challenges in early diagnosis and management. Current evidence highlights emergency hematoma evacuation, adequate decompression, and spinal stabilization a

Percutaneous pedicle screw placement with a mini-open decompression versus open surgery in the treatment of lumbar spondylolisthesis: one-year results of a randomised controlled trial.

PURPOSE: Symptomatic lumbar spondylolisthesis is usually treated with fusion surgery when conservative methods fail. However, traditional open decompression and fusion involves a large skin incision and muscle detachment. Therefore, minimally invasive techniques have been developed to reduce tissue damage, potentially leading to less postoperative pain and earlier resumption of activities. The purpose of this study was to compare percutaneous versus open pedicle screw placement in patients receiving lumbar midline decompression due to symptomatic lumbar spondylolisthesis focusing on short-term

Spinal amyloid deposits are common among older patients undergoing spinal stenosis decompression surgery.

BACKGROUND: Recent advances in the management of transthyretin amyloid cardiomyopathy (ATTR-CM) have highlighted the need for early identification. Studies have demonstrated amyloid deposits in orthopedic surgical specimens, prompting a diagnosis of concurrent ATTR-CM. We sought to determine the prevalence of spinal amyloid deposits among patients undergoing spinal stenosis decompression surgery and whether the presence of deposits was associated with ATTR-CM. METHODS: Patients >60 years of age undergoing spinal stenosis decompression surgery were enrolled as part of a prospective, single-cen

Perioperative Microcirculatory Changes Detected with Gastroscopy Assisted Laser Doppler Flowmetry and Visible Light Spectroscopy in Patients with Median Arcuate Ligament Syndrome.

PURPOSE: Physiological tests may aid in diagnosing median arcuate ligament syndrome (MALS). MALS is a symptomatic compression of the celiac artery causing symptoms similar to chronic mesenteric ischemia (CMI) of atherosclerotic etiology. Simultaneous use of visible light spectroscopy (VLS) and laser doppler flowmetry (LDF) during upper endoscopy may detect microcirculatory changes in these patients. PATIENTS AND METHODS: In a single-center, prospective comparative cohort, 25 patients were evaluated for MALS. Patients with a consensus diagnosis of MALS (n=15) underwent a gastroscopy assisted,

What is the incidence and severity of dysphagia after anterior cervical surgery?

BackgroundExisting studies suggest a relatively high incidence of dysphagia after anterior cervical decompression and fusion (ACDF). The majority of these studies, however, are retrospective in nature and lack a control group.Questions/purposesWe therefore (1) prospectively determined the incidence and severity of dysphagia after ACDF using lumbar decompression patients as a control group; and (2) determined which factors, if any, are associated with increased postoperative dysphagia.MethodsPatients undergoing either one- or two-level ACDF (n=38) or posterior lumbar decompression (n=56) were p
